BACKGROUND: X-linked hypophosphatemia (XLH) causes rickets, osteomalacia, skeletal deformities and growth impairment, due to elevated fibroblast growth factor 23 and hypophosphatemia. Conventional therapy requires high doses of phosphate salts combined with active vitamin D analogues. Risks of this regimen include nephrocalcinosis and secondary hyperparathyroidism or progression to tertiary (hypercalcemic) hyperparathyroidism.
METHODS: The primary goals were to estimate the prevalence of hyperparathyroidism and to characterize parathyroidectomy outcomes regarding hypercalcemia among XLH patie

Autosomal dominant hypocalcemia with hypercalciuria (ADHH) is a rare genetic disorder caused by gain-of-function mutations in the calcium-sensing receptor (CaSR) gene. It is characterized by hypocalcemia, inappropriately low parathyroid hormone (PTH), and renal hypercalciuria. We describe a 13-year-old boy who was diagnosed with congenital hypoparathyroidism, persistent hypercalciuria, and mild medullary nephrocalcinosis. Despite calcium and vitamin D supplementation, hypercalciuria was aggravated. Genetic testing revealed a heterozygous c.2518G>A (p.Gly840Ser) mutation in the CaSR gene, class

Amelogenesis imperfecta IIA3, caused by mutations in the tryptophan-aspartate repeat domain 72 (WDR72) gene, has recently been linked to distal renal tubular acidosis (dRTA). This genetic cause of dRTA has been rarely reported, and its full phenotypic spectrum is still being explored. This case report aims to share the clinical presentation and genetic findings of a recently encountered patient with this genetic variant. An eight-year-old girl presented with nocturnal enuresis and enamel hypoplasia. Laboratory investigations revealed normal anion gap metabolic acidosis with inappropriately hig

Familial hypomagnesemia, hypercalciuria and nephrocalcinosis (FHHNC) is a rare autosomal recessive inherited disorder that has recently been attributed to a defect in the paracellin-1 ( PCLN-1)gene, encoding for a protein responsible for the tubular reabsorption of magnesium and calcium. Limited information is available on clinical course, therapy and prognosis. We provide information on five patients with FHHNC and their follow-up at our institution. Polyuria, nephrocalcinosis and hyperuricemia were the main clinical findings of a diagnosis at a median age of 4.4 years. The clinical course of

X-linked recessive nephrolithiasis (XRN) is a rare hereditary form of progressive renal failure characterized by (1) proximal tubular dysfunction and low molecular weight proteinuria; (2) hypercalciuria with nephrocalcinosis and nephrolithiasis. Because the clinical features are non-specific and variable, affected families in different parts of the world were initially thought to have several distinct syndromes. However, positional cloning of the relevant gene (CLCN5) demonstrated that these families have, in common, mutations affecting a chloride channel expressed throughout the renal tubule.

Hereditary hypophosphatemic rickets with hypercalciuria (HHRH), caused by SLC34A3 gene mutations, is characterized by hypophosphatemia, hypercalciuria, and low PTH levels. While rickets and skeletal deformities are common, HHRH also poses a significant risk for chronic kidney disease (CKD) due to nephrocalcinosis and recurrent kidney stones. Here, we describe a patient with mild skeletal deformities, who was diagnosed with HHRH during adolescence after undergoing whole genome sequencing (WGS) for suspected myopathy. Despite hypophosphatemia and low PTH, the patient had normal calcium levels an

Enamel renal syndrome (ERS) or so-called amelogenesis imperfecta type IG is a very rare disorder characterized by the triad of amelogenesis imperfecta, gingival enlargement and nephrocalcinosis. It is caused by biallelic mutations in the FAM20A gene. Herein, we report two unrelated patients with ERS. Our patients presented with the characteristic features of the syndrome, and amelogenesis imperfecta and gingival hyperplasia were the main complaint. Strikingly, they both had long face, thick lips, notched upper central incisors, and thick alveolar ridge which have never been reported before in

BACKGROUND: Medullary sponge kidney (MSK) is characterized by malformation of the terminal collecting ducts and is associated with an increased risk of nephrolithiasis, nephrocalcinosis, urinary tract infections, renal acidification defects, and reduced bone density. It has been historically diagnosed with intravenous pyelography (IVP), which is falling out of favor as an imaging modality. CT urography (CTU) performed with multidetector CT (MDCT) has been shown to create images of the renal collecting system with similar detail as IVP; however, its utility in diagnosing MSK has not been define

Bartter syndrome (BS) type 1, also referred to antenatal BS, is a genetic tubulopathy with hypokalemic metabolic alkalosis and prenatal onset of polyuria leading to polyhydramnios. It has been shown that BS type 1 is caused by mutations in the SLC12A1 gene encoding bumetanide-sensitive Na-K-2Cl (-) cotransporter (NKCC2). We had the opportunity to care for two unrelated Japanese patients of BS type 1 with typical manifestations including polyhydramnios, prematurity, hypokalemia, alkalosis, and infantile-onset nephrocalcinosis. Analysis of the SLC12A1 gene demonstrated four novel mutations: N117

We present the case of an adolescent with hypercalcemia secondary to unrecognized hyperparathyroidism, which lead to complications such as pancreatitis, diabetes mellitus, and nephrocalcinosis. Although hypercalcemia is not common in the pediatric age, its early recognition and intervention are crucial for the prevention of highly morbid complications.

BACKGROUND: Familial hypomagnesemia with hypercalciuria and nephrocalcinosis is a rare autosomal recessive tubulopathy caused by mutations in CLDN16 or CLDN19, leading to progressive chronic kidney disease. While kidney transplantation is the definitive treatment, the safety of using living-related heterozygous carrier donors remains debated due to potential subclinical tubular dysfunction. We report the 2-year follow-up of a pediatric patient with FHHNC and a CLDN19 mutation who received a graft from his heterozygous carrier mother.
CASE REPORT: A male patient presented at 3 months of age wi

Post-transplant hyperparathyroidism (PT-HPT) is common in kidney transplant recipients (KTRs) and can cause nephrocalcinosis and graft dysfunction. Cinacalcet is commonly used for treating PT-HPT but may induce calciuria and exacerbate nephrocalcinosis. The concurrent use of bisphosphonates with cinacalcet to prevent this complication has not been reported. We report a case of PT-HPT-associated graft dysfunction. The patient had ESRD due to IgAN. She had been on continuous ambulatory peritoneal dialysis (CAPD) for 7 years before undergoing kidney transplantation (KT). Pre-KT intact parathyroid

Normocalcemic primary hyperparathyroidism (NPHPT) is characterized by elevated serum levels of parathyroid hormone (PTH) with persistently normal serum calcium concentrations after excluding secondary causes of hyperparathyroidism. Urolithiasis and/or nephrocalcinosis may occur in hypercalcemic PHPT, but little is known about these complications in NPHPT. Objectives. To identify occult urolithiasis and nephrocalcinosis in asymptomatic patients with NPHPT and evaluate biochemical markers as risk predictors for the development of renal calcification (RC). Methods. Cross-sectional analysis of 34

CONTEXT: CYP24A1 encodes 24-hydroxylase, which converts 25(OH)D3 and 1,25(OH)2D3 to inactive metabolites. Loss-of-function variants in CYP24A1 are associated with 24-hydroxylase deficiency (24HD), characterized by hypercalcemia, nephrolithiasis, and nephrocalcinosis. We retrospectively reviewed laboratory, imaging, and clinical characteristics of patients with suspected or confirmed 24HD and patients with other vitamin D-mediated hypercalcemia disorders: sarcoidosis, lymphoma, and exogenous vitamin D toxicity (EVT).
OBJECTIVE: To identify features that differentiate 24HD from other vitamin D-

BACKGROUND: Ante/neonatal Bartter syndrome (BS) is a hereditary salt-losing tubulopathy due to mutations in genes encoding proteins involved in NaCl reabsorption in the thick ascending limb of Henle's loop. Our aim was to study the frequency, clinical characteristics and outcome of each genetic subtype.
METHODS: Charts of 42 children with mutations in KCNJ1 (n = 19), SLC12A1 (n = 13) CLCNKB (n = 6) or BSND (n = 4) were retrospectively analysed. The median follow-up was 8.3 [0.4-18.0] years.
RESULTS: We describe 24 new mutations: 10 in KCNJ1, 11 in SLC12A1 and 3 in CLCNKB. The onset of polyhy
