In a series of patients with trigeminal neuralgia (TN) who were treated with radiofrequency electrocoagulation of the gasserian ganglion and retrogasserian rootlets, either alone or with glycerol, 16 patients with TN and multiple sclerosis (TNMS) are compared with 219 patients with TN without MS. Patients with TNMS were younger and more likely to have bilateral facial pain than those with TN alone. Probability of ipsilateral recurrence was calculated on the basis of Kaplan and Meier product-limit estimates and showed no significant differences in the two groups.

Trigeminal evoked potentials and sensory thresholds in response to maxillary gum stimulation were obtained in patients with a complaint of unilateral face pain. Glossopharyngeal neuralgia of traumatic origin is very rare. Two interesting cases are presented, one with a skull fracture involving the jugular foramen and the other associated with an operation for an acoustic tumor. Carbamazepine gave good results and both patients improved within 24 hours. The glossopharyngeal and vagal nerves might have been injured in the region of the root entry zone giving rise to the neuralgia. Pertinent literature is briefly discussed.

A case report is presented of a postoperative patient who experienced severe, paroxysmal, neuralgic pains in and about his eye. He was successfully treated with diphenylhydantoin. The literature concerning the rationale for this therapy is reviewed.

The angle of the petrous portion of the temporal bone was measured at the site where it is crossed by the trigeminal nerve. Measurement was facilitated by using the impression techniques of dentistry to produce a plaster model of the bone, and the model was then sawn across to show the angle clearly. Considerable variation was found. Certain bone irregularities were observed and these could also conceivably be factors in the causation of trigeminal neuralgia.
