Role of neurologists and diagnostic tests on the management of distal symmetric polyneuropathy.

IMPORTANCE: Distal symmetric polyneuropathy (DSP) is a prevalent condition that results in high costs from diagnostic testing. However, the role of neurologists and diagnostic tests in patient care is unknown. OBJECTIVE: To determine how often neurologists and diagnostic tests influence the diagnosis and management of DSP in a community setting. DESIGN, SETTING, AND PARTICIPANTS: In this retrospective cohort study, we used a validated case-capture method (International Classification of Diseases, Ninth Revision screening technique with subsequent medical record abstraction) to identify all p

Non-Mydriatic Ocular Fundus Imaging on Consecutive Neurologic and Neurosurgical Patients in an Emergency Department.

BACKGROUND AND PURPOSE: Examination of the ocular fundus is part of the neurologic examination. However, bedside ophthalmoscopy is rarely performed, especially in emergency departments (EDs), with risk of delayed diagnoses and inappropriate triage. Our goal was to determine how often funduscopic examination using non-mydriatic ocular fundus photography combined with optical coherence tomography (NMPF-OCT) is possible and useful in a consecutive cohort of patients with neurologic/neurosurgical disorders evaluated in a general ED. METHODS: This was a quality improvement project, prospective ove…

Muscle Weakness in Adults: Evaluation and Differential Diagnosis.

Although the prevalence of muscle weakness in the general population is uncertain, it occurs in about 5% of U.S. adults 60 years and older. Determining the cause of muscle weakness can be challenging. True muscle weakness must first be differentiated from subjective fatigue or pain-related motor impairment with normal motor strength. Muscle weakness should then be graded objectively using a formal tool such as the Medical Research Council Manual Muscle Testing scale. The differential diagnosis of true muscle weakness is extensive, including neurologic, rheumatologic, endocrine, genetic, medica

Integrating Standardized Multimedia to Supplement Physical Examination Curriculum: An Assessment of Medical Student and Evaluator Perspective.

INTRODUCTION: As a new generation enters the realm of medical education, so do their preferences for technology integration into didactic curricula. An analysis of 106 LCME-accredited medical schools found that 97 percent of programs utilize supplemental e-learning to enhance traditional, face-to-face education in their physical examination curricula. Of these programs, 71 percent produced their multimedia internally. Existing literature indicates medical students benefit from the utilization of multimedia tools and the standardization of the instruction process in the learning of physical exa

[The patient with chronic and/or complex orofacial pain: history taking and diagnostic examination].

Diagnosing chronic orofacial pain can be a time-consuming, complicated and challenging task. Therefore a structured interview and clinical examination are essential to capture all key elements of the evaluation. A thorough understanding of clinical conditions that can present in the orofacial region is obviously fundamental. Key elements of the history taking include questions about the location, onset, duration, frequency, quality and intensity of the pain, precipitating, aggravating and relieving factors, and associated symptoms. The goal of the clinical examinations is to elucidate the sour

Approach to the patient with spinal disease.

The approach to the patient suspected of having spinal disease should be methodical to assure an accurate diagnosis that is cost-effective with minimal risk to the patient. Using a problem-oriented approach will facilitate attaining this goal. A neurologic examination should enable the veterinarian to localize to one of four spinal cord segments or the cauda equina.

Useful C8 and T1 function seen immediately after a complete cervical spinal cord injury: report of 2 cases.

BACKGROUND: Because of the rarity of cervicothoracic spine injuries, detailed reports on the neurologic examination after complete cord injuries are scarce. A few retrospective case series scantily documented the intrinsic hand function after C7-T1 complete cord injuries. A complete cervical cord injury with useful C8 and T1 motor function present immediately after the injury has not been reported to date. CASE DESCRIPTION: Functional C8 and T1 motor strength at the time of the initial neurologic examination was seen in 2 cases in which a complete C7-T1 injury resulted in paraplegia. Serial n…

Hypertensive encephalopathy and the neurologic manifestations of malignant hypertension.

In a prospective study, 34 patients with 41 admissions for malignant hypertension were examined by a neurologist before and after antihypertensive treatment. Neurologic examination was normal in 17 cases, and neurologic diagnoses in 19 others included acute cerebral infarction (10), uremic encephalopathy (4), dementia of uncertain etiology (3), multiple infarct dementia (1), and chronic paranoid schizophrenia (1). Five patients had a generalized neurologic syndrome consistent with hypertensive encephalopathy, characterized by altered mental function with or without generalized convulsions. In …

Fundoscopy use in neurology departments and the utility of smartphone photography: a prospective prevalence and crossover diagnostic accuracy study amongst neurology inpatients.

BACKGROUND AND PURPOSE: Although fundoscopy is a crucial part of the neurological examination, it is challenging, under-utilized and unreliably performed. The aim was to determine the prevalence of fundus pathology amongst neurology inpatients and the diagnostic accuracy of current fundoscopy practice compared with systematic screening with smartphone fundoscopy (SF) and portable non-mydriatic fundus photography (NMFP). METHODS: This was a prospective cross-sectional surveillance and diagnostic accuracy study on adult patients admitted under neurology in an Australian hospital. Inpatients wer

Copper deficiency myeloneuropathy due to occult celiac disease.

INTRODUCTION: Copper deficiency is an increasingly recognized cause of gait unsteadiness. Recognized causes of copper deficiency include excess zinc ingestion, and malabsorption. Although hematologic abnormalities have been attributed to copper deficiency in patients with celiac disease, myeloneuropathy due to copper deficiency has not been well described in patients with celiac disease. CASE REPORT: A 69-year-old woman was evaluated for a 5-year history of progressive gait unsteadiness and weight loss. She had no other gastrointestinal symptoms. Her neurologic examination revealed a sensory …

Are headache patients who initiate their referral to a neurologist satisfied with the consultation? A population study of 927 patients--the North Norway Headache Study (NNHS).

BACKGROUND: Headache is a common problem in primary care. Although most patients are treated by GPs, many are referred to specialist consultation. Knowledge of how the referrals can be improved is therefore an important issue. OBJECTIVES: The aim of this study was to determine the relationship between self-initiating referral to a neurologist and the patient's satisfaction with the specialist consultation. METHODS: All patients who had been examined by a neurologist for headache within a 2-year period from three neurological centres in North Norway completed a questionnaire. RESULTS: A tota
