How can I manage anaesthesia in obese patients?

The obese patient is at risk of perioperative complications including difficult airway access (intubation, difficult or impossible ventilation), and postextubation acute respiratory failure due to the formation of atelectases or to airway obstruction. The association of obstructive sleep apnoea syndrome (OSA) with obesity is very common, and induces a high risk of per and postoperative complications. Preoperative OSA screening is crucial in the obese patient, as well as its specific management: use of continuous positive pre-, per- and postoperative pressure. For any obese patient, the impleme

The validity of surface EMG of extra-diaphragmatic muscles in assessing respiratory responses during mechanical ventilation: A systematic review.

PURPOSE: Evidence supporting the utilization of surface EMG (sEMG) of extra-diaphragmatic muscles for monitoring of mechanical ventilation (MV) assistance is unclear. The purpose of this review was to assess the quality of literature available on using extra-diaphragmatic sEMG as an assessment technique of respiratory responses during MV. METHODS: Studies using sEMG of extra-diaphragmatic respiratory muscles during MV were selected by two independent researchers after performing a database search of PubMed, CINAHL, GOOGLE SCHOLAR. Exclusion criteria were studies of patients with neuromuscular

Compared efficacy and tolerance of the neuromuscular blockade induced by brand-name (Nimbex®) and generic (Cisatrex®) of cisatracurium in mechanically ventilated critically ill patients: a crossover double-blind randomized study.

INTRODUCTION: use of generic drugs is common. However, there is still concern among patients and physicians that brand name drugs are more efficient. The aim of the study was to compare efficacy and tolerance between two forms of cisatracurium: brand name versus generic name. METHODS: it´s a crossover, randomized, double-blind physiological trial. Patients admitted for hypoxemic acute respiratory failure with PaO2/FIO2 < 200mmHg despite optimized ventilation and sedation thus requiring non-depolarizing neuromuscular blocking agents (NMBAs), were enrolled. Patients received consecutively, in a

Multivariate Neuromuscular Profile and Principal Component Analysis: A Pilot Study in Professional Female Soccer Players.

BACKGROUND: Professional women's football requires precise neuromuscular characterisation because of the high mechanical demands associated with acceleration, deceleration, jumping, braking, landing, and rapid force production. The aim was to characterise the multivariate neuromuscular profile derived from the countermovement jump (CMJ) and isometric mid-thigh pull (IMTP), and to examine neuromuscular components according to tactical positions in professional female football players. METHODS: Twenty-three professional female football players were assessed using the CMJ and IMTP. Force-time, i

[General anesthesia for ambulatory surgery : Clinical pharmacological considerations on the practical approach].

Due to modern surgical and anesthesia techniques, many patients undergoing small or even medium surgical procedures will recover within minutes and can then be discharged after a few hours of monitoring. Aside from an optimized surgical technique, a precise and differentiated anesthesia concept is needed to guarantee rapid recovery and home readiness. Nowadays, remifentanil-propofol represents the standard regime in ambulatory anesthesia. The use of alfentanil, desfluran or sevofluran is also possible whereas other intravenous or inhaled anesthetics or other opioids are rarely used. If endotra

REVersal of nEuromusculAr bLocking Agents in Patients Undergoing General Anaesthesia (REVEAL Study).

Background: Neuromuscular blocking agent (NMBA) monitoring and reversals are key to avoiding residual curarization and improving patient outcomes. Sugammadex is a NMBA reversal with favorable pharmacological properties. There is a lack of real-world data detailing how the diffusion of sugammadex affects anesthetic monitoring and practice. Methods: We conducted an electronic health record analysis study, including all adult surgical patients undergoing general anesthesia with orotracheal intubation, from January 2016 to December 2019, to describe changes and temporal trends of NMBAs and NMBA re

Low dose of sugammadex versus neostigmine for reversal of rocuronium induced moderate neuromuscular block: a randomized controlled trial.

BACKGROUND: Residual neuromuscular blockade (rNMB) after surgery can lead to complications such as respiratory distress, increased length of … eostigmine, with implications for improved operating room efficiency and patient safety. Continuous neuromuscular monitoring remains essential, as a small proportion of patients may still require additional intervention. TRIAL REGISTRATION: Registered retrospectively at ClinicalTrials.gov with registration number NCT05718934 on 2023-02-08.

Kidney Function in Patients With Neuromuscular Disease: Creatinine Versus Cystatin C.

Background: Accurate measurement of kidney function in patients with neuromuscular disorders is challenging. Cystatin C, a marker not influenced by skeletal muscle degradation, might be of clinical value in these patients. Methods: We consecutively enrolled 39 patients with neuromuscular disorders. We investigated the association of the eGFR, based on plasma creatinine and Cystatin C, with clinical and biochemical variables associated with kidney function, namely age and galectin-3. Results: Creatinine-based eGFR was 242 (±80) and Cystatin C-based eGFR was 110 (±23) mL/min/1.73 m2. Cystatin C-

Aquaticity as a Latent Dimension of Aquatic Performance: Conceptual Framework and Application to Breath-Hold Diving.

Sports performance in aquatic environments is governed by biomechanical, physiological, neuromuscular and perceptual-mental constraints that differ fundamentally from those encountered on land. As a result, athletes with comparable general physiological or motor capacities may achieve markedly different performance outcomes in aquatic sports. Within functional kinesiology and sport science, aquatic performance is still frequently interpreted through isolated physiological, biomechanical, or technical variables, which limits both explanatory depth and applied relevance. This Perspective article

The Diagnostic Potential of Urinary Titin Fragment in Neuromuscular Diseases.

Biomarkers are important for the diagnosis and follow-up of neuromuscular diseases. Creatine kinase (CK) is a widely used marker of active muscle damage; however, it is not suitable for assessing muscle mass loss. Therefore, additional biomarkers are required to monitor skeletal muscle damage and loss. Titin plays an essential role in the structure and function of muscle fibers. It provides stability and elasticity to the sarcomeres. During sarcomere damage, fragments of titin and other proteins are released from muscle fibers and can be detected in blood and urine. Urinary titin-N fragment (U

The respiratory system.

Neurological disorders frequently contribute to respiratory failure in critically ill patients. They may be the primary reason for the initiation of mechanical ventilation, or may develop later as a secondary complication. Disorders of the central nervous system leading to respiratory failure include metabolic encephalopathies, acute stroke, lesions of the motor cortex and brain-stem respiratory centres, and their descending pathways. Guillan-Barré syndrome, critical illness polyneuropathy and acute quadriplegic myopathy are the more common neuromuscular causes of respiratory failure. Clinical

Sedation and chemical relaxation in critical pulmonary illness: suggestions for patient assessment and drug monitoring.

Anxiety, fear, and pain are common occurrences for patients with acute respiratory failure. These symptoms may influence patients' ability to tolerate mechanical ventilation because of the development of asynchronous breathing patterns. The administration of sedatives and neuromuscular blocking agents may be necessary to facilitate ventilation and enhance oxygenation. Few published protocols exist to guide clinicians in initiating and effectively monitoring patients receiving these agents. In this article, the author focuses on the identification of patients for whom these drugs may be indicat

Anaesthetic management using remimazolam in a patient with Charcot-Marie-Tooth disease complicated by sarcoidosis.

Charcot-Marie-Tooth (CMT) disease is a progressive hereditary neuropathy that complicates anaesthesia because of neuromuscular-blocker sensitivity. Remimazolam, an ultra-short-acting benzodiazepine, has not yet been reported in CMT. We anaesthetised a woman in her 70s with CMT type 1A and cardiac sarcoidosis for femoral nail fixation under total intravenous anaesthesia with remimazolam and remifentanil. Rocuronium 0.6 mg/kg was given 3 min after induction. Mean arterial pressure ranged from -40% to +5% of baseline and was corrected with 6 mg ephedrine. Anaesthetic depth (bispectral index) and

Hypermagnesemia in Clinical Practice.

Hypermagnesemia is a relatively uncommon but potentially life-threatening electrolyte disturbance characterized by elevated magnesium concentrations in the blood. Magnesium is a crucial mineral involved in various physiological functions, such as neuromuscular conduction, cardiac excitability, vasomotor tone, insulin metabolism, and muscular contraction. Hypomagnesemia is a prevalent electrolyte disturbance that can lead to several neuromuscular, cardiac, or nervous system disorders. Hypermagnesemia has been associated with adverse clinical outcomes, particularly in hospitalized patients. Prom

Chronic respiratory failure and neuromuscular disease.

The outlook for children with respiratory complications of neuromuscular disease has improved significantly in the past 15 years. This has been the result of many advances in clinical care, including improved monitoring of lung function and hypoventilation during sleep; coordinated respiratory care by experienced physicians with access to specialized respiratory services, especially physiotherapy; and, most importantly, the widespread introduction of noninvasive ventilation.

Acute myopathy after status asthmaticus: steroids, myorelaxants or carbon dioxide?

Acute myopathy complicating treatment of status asthmaticus has been increasingly recognized since its original description in 1977. We report a case of an 11-year-old boy with severe asthma requiring mechanical ventilation. He was given high doses of parenteral steroids and neuromuscular blockade with non-depolarizing agents in order to achieve controlled hypoventilation with an ensuing hypercapnoea. He developed rhabdomyolysis with elevated creatinine kinase and renal impairment secondary to myoglobinuria. Electrophysiological studies revealed myopathic abnormalities. The aetiology for this

Suxamethonium: a new look at pretreatment.

Fifty patients were pretreated with either suxamethonium 10 mg or gallamine 20 mg before an injection of suxamethonium 1 mg kg-1. The effect of the pretreatment upon neuromuscular transmission was monitored by recording the mechanical response of the adductor pollicis muscle to indirect stimulation, using a 2-Hz train-of-four stimulus repeated every 12 s. The pretreatment injection of suxamethonium caused fasciculation in one-quarter of the patients studied. Significant neuromuscular blockade occurred during pretreatment in 16 of the 25 patients investigated. Pretreatment with gallamine 20 mg
