Percutaneous endoscopic gastrostomy in the neurosurgical intensive care unit: complications and outcome.

BACKGROUND: Even with a functioning gastrointestinal tract, it is not always easy to initiate oral feeding in some neurosurgical patients because of their persistently depressed neurologic status or severe lower cranial nerve palsies. Percutaneous endoscopic gastrostomy (PEG) may be required for long-term feeding in these patients. The purpose of the present study is to report our experience with PEG chosen for establishing an enteral route in patients of neurosurgical intensive care unit (ICU). METHODS: The outcome and complications of PEG in neurosurgical ICU patients of Marmara University

Choroid plexectomy reduces neurosurgical intervention in patients with hydranencephaly.

INTRODUCTION: The removal of the choroid plexus from the lateral ventricles was attempted by Dandy in the early 20th century but later discarded as complications arose and other methods of cerebrospinal fluid (CSF) diversion were introduced. We compare our experience with a variation of this operation to CSF diversion in patients with hydranencephaly or near hydranencephaly. METHODS: The hospital and office charts of patients with a diagnosis of hydranencephaly were reviewed from the two institutions spanning the career of the senior author. Thirteen patients were identified, of whom 9 underw

Neurosurgery for trigeminal neuralgia: comparison of alcohol block, neurectomy, and radiofrequency coagulation.

OBJECTIVES: We wished to assess the present condition of patients previously treated with a neurosurgical procedure or procedures for trigeminal neuralgia (TN) in 383 patients treated between 1976 and 1991, for TN at the Department of Neurosurgery, Hvidovre Hospital. Of these, 67 were lost to follow-up. The latest surgical intervention performed was radiofrequency coagulation (RFC) (64%), neurectomy (18%), alcohol block (16%), trigeminal tractotomy (1%), and microvascular decompression (1%); 72% of patients underwent only one neurosurgical procedure. METHODS: Questionnaires were sent to 316 p

[Intracranial epidural abscess associated to sinusitis with positive response to medical treatment].

Epidural abscesses are rare intracranial infections. They can occur from neurosurgical procedures, or head and neck infections. Successful treatment usually requires a combination of drainage procedure and antibiotic therapy. We present the case of a young patient with an intracranial epidural abscess and meningitis secondary to sinusitis who received only corticosteroids and antibiotic therapy, with good evolution.

Stenotrophomonas maltophilia meningitis: to look for in post-neurosurgical state.

Stenotrophomonas maltophilia is a less frequently reported pathogen associated with meningitis but it's involvement cannot be ignored in nosocomial settings, especially post interventional procedures. Managing these cases is difficult, owing to the organism's intrinsic resistance to large number of antibiotics. Here, we report three cases in which patients presented with meningitis caused by Stenotrophomonas maltophilia post neurosurgical procedure.

A Rare Case of Bacterial Meningitis Caused by Hematogenous Spread of Vancomycin-Resistant Enterococcus faecium in an Immunocompromised Patient.

Vancomycin-resistant Enterococcus faecium (VRE) meningitis is rare. It is usually associated with neurosurgical procedures or devices. We describe a case of VRE meningitis from hematogenous spread during persistent bacteremia in an immunocompromised patient who received haploidentical bone marrow transplant for VEXAS syndrome. The bacteremia and meningitis were successfully treated with combination of intravenous (IV) daptomycin, ceftaroline, and linezolid.

[Remote cerebellar hemorrhage].

Intracerebral hemorrhage remote from the site of surgery is an infrequent complication after neurosurgical procedures. Cerebellar hemorrhage after supratentorial craniotomy is the most commonly described pattern of remote hemorrhage unknown by the anesthesiologist. It is linked with a frequent morbidity and mortality. We report a case of this complication after meningioma surgery with a favorable outcome.

Vancomycin-resistant enterococcal meningitis treated with intrathecal streptomycin.

Enterococcal meningitis is a rare complication of neurosurgical procedures. We present a patient who developed vancomycin-resistant enterococcal ventriculitis - meningitis after a brain tumor resection and ventriculoperitoneal shunt placement, treated successfully with intrathecal streptomycin through bilateral cerebrospinal fluid drainage catheters in addition to systemic antibiotics. This is the first report of such treatment for this resistant organism.

Persistence of Bioglue in spinal dural repair.

Bioglue is an organic surgical adhesive commonly used in vascular and cardiopulmonary repair surgery. It has been used in neurosurgical procedures to minimise the risk of cerebrospinal fluid leak. We report a case which demonstrates persistence of Bioglue at the repair site two years after its successful use in aiding dural closure after a lumbar decompressive procedure.

Monitoring of cerebral hemodynamics with jugular bulb catheters.

Jugular venous oxygen saturation (SjvO2) monitoring is useful for detecting episodes of cerebral hypoxia/ischemia in patients with head injury, patients undergoing neurosurgical procedures, and patients undergoing cardiopulmonary bypass. The use of SjvO2 monitoring can direct the treatment of ischemic episodes and identify the optimal level of cerebral perfusion pressure and PCO2 for the individual patient.

Vascular malformations as a cause of spontaneous spinal epidural haematoma.

Seven cases of spinal epidural haematoma occurred in a series of 2,500 neurosurgical procedures. Five of these seven cases were non-traumatic in origin. In four cases the bleeding could be ascribed to various types of vascular malformations. The clinical and histological features are described, and the relevance of these findings for surgical management is discussed.

Neurosurgical strategies for Gilles de la Tourette's syndrome.

Tourette's syndrome (TS) is a neurological disorder characterized by motor and vocal tics that typically begin in childhood and often are accompanied by psychiatric comorbidities. Symptoms of TS may be socially disabling and cause secondary medical complications. Pharmacological therapies remain the mainstay of symptom management. For the subset of patients in whom TS symptoms are medically recalcitrant and do not dissipate by adulthood, neurosurgery may offer an alternative treatment strategy. Greater understanding of the neuroanatomic and pathophysiologic basis of TS has facilitated the deve

Thalamic Deep Brain Stimulation for Writer's Cramp.

Writer's cramp is a type of idiopathic focal hand dystonia characterized by muscle cramps that accompany execution of the writing task specifically. There has been renewed interest in neurosurgical procedures for the treatment of dystonia over the past several years. In particular, deep brain stimulation (DBS) has received increasing attention as a therapeutic option for patients with dystonia. However, to date, limited reporters made investigations into DBS in relation to the Writer's cramp. In this case, unilateral Ventro-oralis complex (Vo) DBS resulted in a major improvement in patient's f…

Neurosurgical control of chronic pain.

The neurosurgeon can play a significant role in the management of chronic pain. This potential is all to often unrealized or distorted by a lack of knowledge about patients who are suffering from chronic pain. Patient selection is the most important determinant of successful surgical therapy. Patients with chronic pain due to a neoplasm should be offered prompt, definitive neurosurgical intervention. Neurosurgical therapy is less likely to be successful if the pain is secondary to benign disease; tic douloureux is an exception to this rule. If chronic pain behavior is not primarily due to noxi

Recombinant activated factor VII for the rapid correction of coagulopathy in nonhemophilic neurosurgical patients.

OBJECTIVE: Coagulopathy is a significant contraindication for neurosurgery. Unfortunately, many coagulopathic patients require urgent neurosurgical intervention. Standard use of blood products, including fresh-frozen plasma or prothrombin complexes, to correct the coagulopathy often leads to significant delays in treatment. Recombinant activated factor VII (rFVIIa) is a medication originally designed to treat bleeding in hemophiliacs but also seems to correct a wide variety of coagulopathies rapidly and safely in nonhemophilic patients. METHODS: The medical records of nine patients with coagu

Intraventricular tension pneumocephalus after endoscopic skull base surgery.

BACKGROUND AND STUDY AIMS: Postoperative pneumocephalus is commonly observed after neurosurgical intracranial procedures and is usually of little consequence. Intraventricular tension pneumocephalus, however, is a rare complication and a neurosurgical emergency that requires immediate intervention. We describe the first case of intraventricular tension pneumocephalus that developed 1 week following an expanded endoscopic endonasal approach for resection of a suprasellar mass. PATIENT: A patient who underwent an endoscopic transplanum transtuberculum approach for resection of a suprasellar epi

W. W. Keen and the dawn of American neurosurgery.

Before the turn of the century, W. W. Keen was the most celebrated neurosurgeon in the United States. During the Civil War he served as a surgeon in the Union Army. He collaborated with Mitchell and Morehouse in clinical studies that culminated in their publishing Gunshot Wounds and Other Injuries of Nerves. In 1887, he was the first surgeon in the Americas to remove a benign brain tumor. He perfected a technique for ventricular puncture, devised operations for spasmodic torticollis, microcephalus, and tic douloureaux, and introduced many European neurosurgical techniques to the United States.
