The experience of using noninvasive ventilation (NIV) in 113 adult cystic fibrosis (CF) patients with chronic respiratory failure, during episodes of acute deterioration in respiratory function is reported. The patients aged 15-44 yrs were divided into three groups. Group A consisted of 65 patients (median forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) 0.7/1.4 L) who were on a lung transplant waiting list. Group B consisted of 25 patients (median FEV1/FVC 0.7/1.4 L) who were being evaluated for lung transplantation. Group C consisted of 23 patients (median FEV1/FVC 0…

Studies have shown that noninvasive positive pressure ventilation (NPPV) is well tolerated and safe, and that it improves oxygenation in some patients with acute respiratory failure. By obviating the need for endotracheal intubation in certain conditions, it results in fewer complications, shorter hospital stays, and consequently, lower mortality rates and costs of care.

Methods of noninvasive pressure support ventilation (NIPSV) are not always easy to apply in patients with acute exacerbations of chronic obstructive pulmonary disease (COPD). The assistance time spent by nurses in relation to ventilatory time was prospectively studied, when NIPSV was used, in a sequential mode, in COPD patients with either acute exacerbations (58 patients, group I) or postextubation hypercapnic respiratory insufficiency (42 patients, group II) in a medical intensive care unit. During the first 24 h after enrolment, NIPSV was used for 6.7+/-3.2 h (mean+/-SD) in group I and 5.6+

OBJECTIVE: To determine the value of noninvasive mechanical ventilation in patients with acute respiratory insufficiency.
DESIGN: Descriptive.
METHODS: Noninvasive mechanical ventilation was considered in all patients with acute respiratory insufficiency in the intensive care unit of the Rijnstate Hospital, Arnhem, the Netherlands, between 1 June 1998 and 31 January 1999. Indication for mechanical ventilation was: respiratory frequency > or = 28/min, and PaCO2 > 6.0 kPa, pH < 7.35, and/or PaO2 < 8.0 kPa. Patients were intubated immediately in case of systolic blood pressure < 90 mmHg, cardia

Recent studies suggest that noninvasive positive pressure ventilation (NPPV) administered by nasal or oronasal mask avoids the need for endotracheal intubation, rapidly improves vital signs, gas exchange, and sense of dyspnea, and may reduce mortality in selected patients with acute respiratory failure, but few controlled trials have been done. The present study used a randomized prospective design to evaluate the possible benefits of NPPV plus standard therapy versus standard therapy alone in patients with acute respiratory failure. Patients to receive NPPV were comfortably fitted with a stan

Previous studies have shown that noninvasive positive pressure ventilation (NPPV) improves gas exchange in acute and chronic respiratory failure. However, some patients are unable to tolerate NPPV due to air leaks around the mask, facial discomfort, and claustrophobia. A new mask that covers the entire face (Total, Respironics, Monroeville, Pa), attempts to overcome these obstacles. We studied the efficacy of NPPV via the Total face mask (TFM) in nine patients with chronic respiratory failure. In three patients, respiratory failure was due to chronic obstructive lung disease, and in six patien

The effects of noninvasive nasal mask-assisted ventilation were studied in two patients with chronic respiratory failure due to Duchenne's muscular dystrophy. Observations were made with continuous recordings of transcutaneous CO2 and O2 and ear oximetry. In one case, the mean tcPCO2 fell from 72 mm Hg to 43 mm Hg. The tcPO2 increased from 38 mm Hg to 62 mm Hg without supplementary oxygen. In the second case, hypercapnia associated with supplementary oxygen was corrected, and at five months' follow-up, hypoxemia was corrected without supplementary oxygen. Prolonged therapy during sleep has res

AIM: This study aimed to develop and evaluate a helmet noninvasive ventilation (NIV) simulation-based training program grounded in the National League for Nursing/Jeffries Simulation Theory (NLN/JST), comparing its effects with lecture-based education on ICU nurses' knowledge, self-efficacy and clinical performance.
BACKGROUND: Although the use of helmet NIV has increased, applying this interface requires complex skills and specific training programs are lacking.
DESIGN: A quasi-experimental, non-equivalent control group, pre- and post-test design was employed.
METHODS: Forty-three ICU nurs…

BACKGROUND: Noninvasive (NIV) failure (progression from NIV to intubation) is associated with high risk of death. Impaired patient-provider communication may increase risk of failure. We investigated associations between primary spoken language (PSL) and NIV failure.
METHODS: In this retrospective cohort study using the 2021 State Inpatient Database from California, Iowa, and Maryland, we identified adult patients treated with NIV. The primary exposure was PSL categorized as English, Spanish, or non-English/non-Spanish. Mixed-effects models were used to determine the adjusted odds (aOR) of NI

PURPOSE OF REVIEW: Helmet noninvasive ventilation (NIV) has gained attention for the management of hypoxemic patients, owing to physiological and potential clinical benefits. We summarize the recent advances on the topic.
RECENT FINDINGS: Compared to facemasks, helmets facilitate application of higher positive end-expiratory pressure (PEEP) for prolonged treatments: this improves oxygenation and may mitigate injurious inflation patterns related to lung heterogeneity. The large, highly compliant interface reduces ventilator triggering performance, causing pressure support to be partially out o…

Noninvasive assessments of lung volume distribution often require inhaled contrast and are limited by low regional resolution. We aimed to examine a noncontrast imaging method of spatial lung volume displacement, adapted to assess changes with noninvasive positive pressure ventilation (NIPPV). This study evaluated regional lung volume displacement in nine healthy volunteers (6 males and 3 females; ages 29-55 yr; body mass index 20.2-31.3 kg/m2) using X-ray velocimetry (XV). Participants were assessed during tidal breathing and, also with 15 cmH2O inspiratory and 5 cmH2O expiratory pressures in

BACKGROUND: Noninvasive ventilation (NIV) has become more studied in immunocompromised patients. However, it has not been studied in hematopoietic cell transplantation (HCT) recipients, who have higher mortality and higher pulmonary complication rates than other immunocompromised patients. This population may be prone to negative effects from this treatment modality. The aim of this study was to determine whether NIV use is associated with worse outcomes in this vulnerable patient population.
METHODS: A secondary analysis of a retrospective multi-center database was performed. Twelve pediatri…

RATIONALE: Little is known on the use of noninvasive ventilation (NIPPV) in pediatric hematopoietic cell transplant (HCT) patients.
OBJECTIVE: We sought to describe the landscape of NIPPV use and to identify risk factors for failure to inform future investigation or quality improvement.
METHODS: This is a multicenter, retrospective observational cohort of 153 consecutive children post-HCT requiring NIPPV from 2010-2016.
RESULTS: 97 (63%) failed NIPPV. Factors associated with failure on univariate analysis included: longer oxygen use prior to NIPPV (p=0.04), vasoactive agent use (p<0.001), a…

INTRODUCTION: During this long COVID-19 pandemic outbreak, continuous positive airway pressure (CPAP) and noninvasive ventilation (NIV) are being widely used to treat patients with moderate to severe acute respiratory failure (ARF). As for now, data on the efficacy of NIV in COVID-19 acute respiratory distress syndrome (ARDS) are lacking, and for this reason it is extremely important to accurately determine the outcomes of this strategy. This study aimed to evaluate clinical data and outcomes of NIV in patients with COVID-19 ARDS.
MATHERIALS AND METHODS: Seventy-nine consecutive patients with…

BACKGROUND: The severity of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection is extremely variable, ranging from asymptomatic patients to those who develop severe acute respiratory distress syndrome (ARDS). As for now, there are still no really effective therapies for coronavirus disease 2019 (COVID-19). Some evidences suggest that tocilizumab (TCZ) may avoid the progression of severe COVID-19. The aim of this retrospective case-control study was to analyze the efficacy and safety of TCZ in patients with COVID-19 ARDS undergoing noninvasive mechanical ventilation (NIV).
