Sudden Unexpected Postnatal Collapse of the Newborn.

Sudden unexpected postnatal collapse is a rare but devastating neonatal event. A well-appearing, full-term newborn with Agpar scores of eight or more suddenly crashes, often with full respiratory and cardiac arrest. Up to half of newborns with sudden unexpected postnatal collapse die, with many survivors suffering serious neurological damage. The first 2 hours of life are the hours of greatest risk, coinciding with the time frame when nurses encourage breastfeeding and uninterrupted skin-to-skin contact between women and newborns. Nursing assessments and measures to promote neonates' optimal t

Supporting breastfeeding mothers in hospital: part 2a.

The first article in this series highlighted the challenges that mothers face in establishing breastfeeding (Wallis and Harper 2007). Breastfeeding a sick infant in hospital presents additional challenges such as small, sleepy babies, babies who may be nil by mouth for prolonged periods and babies who may find breastfeeding difficult due to mechanical problems. There is very little published abut the needs of babies with complex medical or surgical conditions who may have had significant delays in their feeding or numerous starts and stops along the way. This article and Part 2b in the Novembe

Comparing between face-to-face and mobile learning for improving nurses' performance in premature infant positioning: a quasi-experimental study.

BACKGROUND: Proper positioning is widely recognized in neonatal care. Implementing evidence-based education to improve Neonatal Intensive Care Unit (NICU) nurses' competency in infant positioning performance is essential. Evidence-based educational strategies are critical to enhancing NICU nurses' competency in infant positioning. OBJECTIVES: To compare the effect of face-to-face training versus mobile learning (m-learning) utilizing multimedia resources on the positioning of preterm infants among NICU nurses. METHODS: This quasi-experimental pretest-posttest study employed a nonequivalent c

Exploring Perinatal Nursing Care for Opioid Use Disorder: Knowledge, Stigma, and Compassion.

The opioid epidemic has greatly increased the number of pregnant women with opioid use and newborns exposed to opioids in utero. Mothers with opioid use disorder can face stigma by nurses in perinatal care settings, contributing to negative care experiences. A survey was distributed to nurses caring for mothers and newborns exposed to opioids in a large urban hospital in the Pacific Northwest United States (n = 89) from March to July 2019. Survey measures included participant characteristics, attitude toward substance use in pregnancy and postpartum (stigma, compassion satisfaction, comfort, a

Look before you clamp: delivery room examination of the umbilical cord.

The umbilical cord is a critical connection between the embryo (and later, the fetus) and the placenta. The umbilical cord houses the blood vessels that are responsible for nourishing the fetus. Proper umbilical cord function is essential for growth and development before birth. A review of the embryologic origin of the umbilical cord aids in understanding abdominal wall and umbilical cord defects that can present in the newborn period. In the delivery room, inspection of the umbilical cord is an integral part of the first minutes of life. Any abnormality either within the cord structure or in

Systemic lupus erythematosus: perinatal and neonatal implications.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder that can affect almost all organ systems in the body. It is most common in women of childbearing age and may cause multiple peripartum complications. This article reviews the pathophysiology of SLE and the effects of SLE on fertility and pregnancy. The complexities of managing a pregnant patient with SLE are reviewed, and the importance of interdisciplinary collaboration discussed, as well as the effects of SLE on the fetus and a review of neonatal lupus erythematosus. Finally, a case report of a pregnant patient with SLE with

The retinopathy of prematurity screening examination: ensuring a safe and efficient examination while minimizing infant discomfort.

Retinopathy of prematurity (ROP) examinations in the NICU are necessary to protect vision in premature infants, but the examinations are associated with risk and discomfort. ROP examination risks include adverse effects from mydriatic agents, systemic responses to the stress of examination, and nosocomial infection. Infant discomfort may be lessened by limiting examination length and possibly by measures such as topical anesthetic, oral sucrose, and certain nonpharmacologic techniques. A well-organized ROP service facilitates appropriate scheduling of examinations and education of the infant's

Hospitalization for miscarriage and delivery outcome among Swedish nurses working in operating rooms 1973-1978.

All infants born in 1973-1978 to nurses working in anesthesiology or as operating room nurses were identified from a nationwide registry of all births in Sweden, a registry of hospitalized spontaneous and legally induced abortions that covers 70% of Sweden, and a nurse registry (n = 1323). For comparison, a group was formed that consisted of nurses working in medical wards (n = 1382). Delivery outcome was also compared with the estimate expected from nationwide figures. No statistically significant differences were seen, but infants of the anesthesiology/operating room nurses had a slightly hi

Assessing and addressing moral distress and ethical climate Part II: neonatal and pediatric perspectives.

Moral distress remains a pervasive and, at times, contested concept in nursing and other health care disciplines. Ethical climate, the conditions and practices in which ethical situations are identified, discussed, and decided, has been shown to exacerbate or ameliorate perceptions of moral distress. The purpose of this mixed-methods study was to explore perceptions of moral distress, moral residue, and ethical climate among registered nurses working in an academic medical center. Two versions of the Moral Distress Scale in addition to the Hospital Ethical Climate Survey were used, and partici

Development of dome epithelium in gut-associated lymphoid tissues: association of IgA with M cells.

The dome epithelium (DE), which covers gut-associated lymphoid tissues (GALT) and provides both a protective barrier over lymphoid follicles and a route for antigen uptake from the gut, develops in rabbit appendix (caecum) during the first week of neonatal life. To determine if secretory immunoglobulins from maternal milk interact with this developing tissue, their interrelationships in neonatal rabbit appendix were examined by use of immunocytochemical techniques. The glycoprotein, secretory component, was not produced by neonatal rabbits less than 15 days old, since neither the membranous no

Neonatal records and the computer.

To use a combined single document clinical case sheet/computer record which can form the basic document for a life medical record is a practical proposition. With adequate briefing doctors and nurses soon become familiar with the record and appreciate its value. Secretarial and clerical requirements are reduced to a minimum as transcription of medical data is eliminated, so greatly speeding up processing and feed back to the medical services. A few illustrations of trends in neonatal statistics and of computer linked maternal/neonatal data are presented.
