A Systematic Review and Meta-analysis of the Association Between Vitamin K Antagonist Use and Fracture.

BACKGROUND: Vitamin K antagonist (VKA) anticoagulant use is suspected to increase the risk of bone fracture through inhibition of vitamin K-dependent cofactors of bone formation, an effect not seen with non-vitamin K antagonist oral anticoagulants (NOACs). The purpose of our systematic review and meta-analysis is to investigate the association between VKA use and fracture. METHODS: We searched PubMed, EMBASE, and Cochrane Library for studies analyzing fracture in adults using VKAs versus controls. Two authors independently reviewed articles. We assessed for risk of bias using the Newcastle-Ot

Preoperative hormonal stimulation effect on hypospadias repair complications: Meta-analysis of observational versus randomized controlled studies.

INTRODUCTION: Preoperative hormonal stimulation (PHS) is commonly administered to improve hypospadias outcomes. However, current literature provides no clear evidence to support its use, and controversies exist regarding its effect on postoperative complications. Herein we systematically evaluate the effect of PHS on postoperative complications rates following hypospadias repair. MATERIALS AND METHODS: Comprehensive literature search performed on April 2016 including OVID MEDLINE, EMBASE, Web of Science, Cochrane Library, Clinicaltrials.gov, World Health Organization (WHO) International Clini

Up-front combination therapy in pulmonary arterial hypertension: from clinical trials to 'real world' observational studies.

Combination therapy in pulmonary arterial hypertension is widely used in daily clinical practice. There is a wide variation among reference centers, with respect to not only when and how combination therapy should be initiated, but also what constitutes the most effective multidrug regimen. Presently, no combination therapy has proven to be more effective than the other. However, add-on therapy, in case of unsatisfactory improvements after initiation of specific monotherapy, seems to have more evidence of effectiveness than up-front therapy at the diagnosis. Data from national pulmonary arteri

Independent evaluator knowledge of treatment in a multicenter comparative treatment study of panic disorder.

The purpose of this study was to examine independent evaluators' (IEs) blindness to treatment condition during a Multicenter Comparative Treatment Study of Panic Disorder. IEs were 15 doctoral- and masters-level clinicians in psychology, social work, and medicine. They conducted three post-treatment assessments with each patient. Immediately after each assessment interview, IEs completed a form indicating which of the five possible treatments they believed the patient had received and any specific information that provided IEs with information about a patient's treatment condition. These forms

Traumatic Brain Injury in Patients under Anticoagulant Therapy: Review of Management in Emergency Department.

The best management of patients who suffer from traumatic brain injury (TBI) while on oral anticoagulants is one of the most disputed problems of emergency services. Indeed, guidelines, clinical decision rules, and observational studies addressing this topic are scarce and conflicting. Moreover, relevant issues such as the specific treatment (and even definition) of mild TBI, rate of delayed intracranial injury, indications for neurosurgery, and anticoagulant modulation are largely empiric. We reviewed the most recent evidence on these topics and explored other clinically relevant aspects, suc

Association between perinatal intimate partner violence and breastfeeding outcomes: a systematic review and meta-analysis.

BACKGROUND: Intimate partner violence (IPV) negatively affects the health of both the mother and the child. These risks are particularly critical during the perinatal period, a time of heightened vulnerability. However, the relationship between maternal IPV exposure and breastfeeding outcomes remains uncertain, and existing findings are inconsistent and inconclusive. Therefore, this systematic review and meta-analysis aimed to synthesise the existing evidence on the association between perinatal IPV and breastfeeding outcomes. METHODS: A systematic literature search was conducted using releva

Insomnia in spinal cord injury: A meta-analysis of observational studies.

Sleep problems are frequent following spinal cord injury or disease (SCI/D), however the severity of insomnia - including difficulty falling asleep and staying asleep - is not fully understood. Differing assessment methods, variations in diagnostic criteria, and the characteristics of SCI/D may also impact reported prevalence estimates. This PRISMA-guided review synthesizes the available evidence, with consideration of study-level characteristics. The CINAHL, Embase, MEDLINE and PsycINFO databases were searched on March 22nd, 2024 (updated February 12th, 2025), to identify observational studie

Systematic review of the reporting of extrarenal manifestations in observational studies of Saudi patients with systemic lupus erythematosus.

BACKGROUND: SLE is prevalent in Saudi Arabia, with numerous studies focusing on SLE in adult patients. However, there is a lack of comprehensive studies summarising the extrarenal manifestations of SLE in this population. This study aims to assess the variability in the prevalence rates of extrarenal manifestations of SLE across different cities in Saudi Arabia and to emphasise the need for a national registry to better understand the overall disease burden in the region. METHODS: We conducted a systematic review of articles with no time restrictions, including studies from databases such as

The impacts of sarcopenia and its components on heart failure patients: A systematic review and network meta-analysis.

BACKGROUND: Heart failure (HF) and sarcopenia are prevalent, interrelated conditions that significantly impact older adults. Sarcopenia, characterized by progressive decline in muscle mass, strength, and function, is common in HF and linked to higher mortality, hospitalization, and functional decline. Early recognition is hindered by overlap with frailty, variability in assessment methods, and diagnostic challenges in fluid-overloaded patients. These challenges underscore the importance of clarifying the prognostic role of specific sarcopenia-related components in HF. This study aimed to asses
