Treatment of malignant midgut carcinoid tumours with a long-acting somatostatin analogue octreotide.

Treatment with the somatostatin analogue octreotide, SMS 201-995 (Sandostatin), has been carried out in a series of 23 patients with malignant midgut carcinoid tumours. The patients received initially 50 micrograms twice a day for six months, thereafter a median of 100 micrograms twice daily. Six of 22 evaluable patients (28%) showed objective tumour response lasting for 6 to 30 months. Stable disease was observed in 8 of the 22 patients (36%) and progressive disease in a further 8 patients (36%). A subjective response with decrease of diarrhoea or flushing was noted in 11 out of 22 patients (

Long-term use of subcutaneous octreotide reduces variceal bleeding in non-cirrhotic portal hypertension.

BACKGROUND: Non-cirrhotic portal hypertension from a portal vein thrombus can cause variceal bleeding. The use of vasoactive agents such as octreotide is currently limited to acute bleeding episodes. Current strategies to reduce recurrent bleeding events include non-selective beta blockers, treatment of underlying disease including management of risk factors, and periodic endoscopic therapy. There is limited literature on the use of subcutaneous octreotide in the long-term management of recurrent variceal bleeding. CASE: We present a 55-year-old male with progressive non-cirrhotic portal hype

Acute Variceal Bleeding: Does Octreotide Improve Outcomes in Patients with Different Functional Hepatic Reserve?

BACKGROUND: Current guidelines do not differentiate in the utilization of vasoactive drugs in patients with cirrhosis and acute variceal bleeding (AVB) depending on liver disease severity. MATERIAL AND METHODS: In this retrospective study, clinical outcomes in 100 patients receiving octreotide plus endoscopic therapy (ET) and 216 patients with ET alone were compared in terms of failure to control bleeding, in-hospital mortality, and transfusion requirements stratifying the results according to liver disease severity by Child-Pugh (CP) score and MELD. RESULTS: In patients with CP-A or those w

Improved survival with higher doses of octreotide long-acting release in gastroenteropancreatic neuroendocrine tumors.

Gastroenteropancreatic (GEP) neuroendocrine tumors (NETs) represent a heterogeneous group of tumors that is associated with an indolent course. Octreotide has a positive effect on disease stabilization in well-differentiated midgut NETs, but a meaningful survival analysis was not possible due to insufficient events. Higher doses of octreotide long-acting release (LAR) are often used in clinical practice for control of carcinoid symptoms and our objective was to determine if dose of octreotide correlates with survival. We reviewed all patients with advanced GEP NETs who initiated treatment with

Celecoxib and octreotide synergistically ameliorate portal hypertension via inhibition of angiogenesis in cirrhotic rats.

Abnormal angiogenesis is critical for portal hypertension in cirrhosis. Except for etiological treatment, no efficient medication or regime has been explored to treat the early stage of cirrhosis when angiogenesis is initiated or overwhelming. In this study, we explored an anti-angiogenesis effort through non-cytotoxic drugs octreotide and celecoxib to treat early stage of cirrhotic portal hypertension in an animal model. Peritoneal injection of thioacetamide (TAA) was employed to induce liver cirrhosis in rats. A combination treatment of celecoxib and octreotide was found to relieve liver fib

Safety study of somatostatin analogue octreotide for autosomal dominant polycystic kidney disease in Japan.

BACKGROUND: The total kidney volume (TKV) and total liver volume (TLV) increase and renal function decreases progressively in patients with autosomal dominant polycystic kidney disease (ADPKD). Somatostatin analogues, such as octreotide, reduce these increases in TKV and TLV. The aim of this study was to examine the safety of the short-term administration of octreotide long-acting release (octreotide-LAR) in a small number of cases. METHODS: Four ADPKD patients with an estimated glomerular filtration rate (eGFR) > 45 mL/min/1.73 m(2), TKV > 1,000 mL, and TLV > 3,000 mL were enrolled. Two 20-m

Octreotide for esophageal variceal bleeding treated with endoscopic sclerotherapy: a randomized, placebo-controlled trial.

BACKGROUND/AIMS: Endoscopic sclerotherapy is considered a first line therapy to stop bleeding from esophageal varices, but acute variceal bleeding is still associated with high risk of rebleeding and death. We compared the use of octreotide with endoscopic sclerotherapy versus sclerotherapy alone to control acute variceal bleeding and prevent rebleeding in patients with cirrhosis. METHODOLOGY: In a prospective controlled trial, 68 patients with cirrhosis and acute variceal bleeding who underwent emergency sclerotherapy were randomly assigned to receive a continuous infusion of octreotide or p

[111Indium-DTPA-D-phe] octreotide scintigraphy in the diagnostic assessment of palpable breast lumps.

It has been suggested that [111Indium-DTPA-D-Phe] octreotide scintigraphy may be useful in the staging of breast cancer. We evaluated its role in the diagnostic assessment of 40 female patients with palpable breast lumps. All were clinically assessed and imaged by ultrasound or mammography. Thirty patients had adequate FNA cytology performed. Histological examination following core or excision biopsy showed 31 lumps to be benign and 9 to be malignant. All patients with invasive cancer proceeded to wide local excision or mastectomy with an axillary clearance. The mean diameter of malignant lesi

A three-day octreotide-containing helicobacter pylori eradication therapy for cure of peptic ulcers.

BACKGROUND/AIMS: Octreotide is used to arrest peptic ulcer hemorrhage. Since it has anti-secretory properties, it could also be used in Helicobacter pylori eradication therapy, to cure peptic ulcer before discharging patients from hospital. The aim of this pilot study was to determine safety and efficacy of an ultra short quadruple octreotide containing H. pylori eradication therapy in patients with peptic ulcer. METHODOLOGY: Twenty-six consecutive symptomatic H. pylori-positive patients with duodenal (n = 20) or gastric ulcer (n = 6), were treated with a three-day course of octreotide 0.3 mg

Octreotide therapy in patients with a left ventricular assist device and bleeding gastrointestinal angiodysplasias.

BACKGROUND: Angiodysplasias commonly develop in patients with left ventricular assist devices (LVADs) and often bleed recurrently. Whether octreotide reduces the burden of angiodysplasia or the frequency of bleeding in these patients is controversial. Hence, we aimed to study the effect of long-acting octreotide on angiodysplasia bleeding in LVAD patients. METHODS: Between May 2012 and December 2023, 65 patients with LVAD and angiodysplasia were identified at our tertiary care medical center. Of those, 33 patients received octreotide therapy. Complete clinical data were abstracted from the me

Revisiting the usefulness of the short acute octreotide test to predict treatment outcomes in acromegaly.

INTRODUCTION: We previously described that a short version of the acute octreotide test (sAOT) can predict the response to first-generation somatostatin receptor ligands (SRLs) in patients with acromegaly. We have prospectively reassessed the sAOT in patients from the ACROFAST study using current ultra-sensitive GH assays. We also studied the correlation of sAOT with tumor expression of E-cadherin and somatostatin receptor 2 (SSTR2) . METHODS: A total of 47 patients treated with SRLs for 6 months were evaluated with the sAOT at diagnosis and correlated with SRLs' response. Those patients whos

Short-term protective effect of octreotide on the lungs of rats with experimentally induced sepsis.

BACKGROUND: Acute respiratory distress syndrome is a devastating complication of severe sepsis. Preclinical models suggest that direct lung injury begins with attack to the lung epithelium, but indirect lung injury results from systemic endothelial damage due to inflammatory mediators. The aim of the present study was to explore the effect of octreotide on lungs in a surgically induced sepsis model in rats. METHODS: We used 32 male Sprague Dawley rats and divided into four groups. Group 1: Normal (non-operative and orally fed control, n=8); Group 2: Sham operated (n=8); Group 3: Cecal ligatio
