Opsoclonus-myoclonus-ataxia syndrome (OMAS) is an autoimmune central nervous system disorder, primarily manifesting as a paraneoplastic sequalae to neuroblastoma, and characterized by motor disorders and behavioral disturbances. OMAS is typified by aberrant B-cell and T-cell activation. Current treatment involves immunosuppression using corticosteroids, intravenous immunoglobulin, and rituximab. However, these approaches often lead to treatment-related toxicities and symptomatic recurrences with chronic neurocognitive impairment. We treated three children with refractory neuroblastoma-associat

Few hours after birth, a neonate showed abnormal eye movements with episodes of downward gazing with a vertical nystagmus. Electrolytes were normal and CRP was low. Cerebral ultrasound and amplitude integrated electroencephalography showed no abnormalities. The abnormal eye movements resolved spontaneously. The infant was diagnosed with paroxysmal tonic downgaze of infancy.

The purpose of the study was to present a rare case of post-traumatic Parinaud's syndrome with a history of closed head injury. The clinical characteristics, examination, and management are presented in a 12-year-old boy who was a victim of physical violence at the hands of a young boy who punched him in his chin with his fists, with associated paralysis of the upward gaze of the left eyeball, and convergence nystagmus with pupillary involvement. CT examination indicated posttraumatic lesions in the dorsal midbrain and in the cavity of third ventricle, suggestive of acute hemorrhages. Posttrau

INTRODUCTION: Following mild traumatic brain injury, visual dysfunction is a common occurrence, yet the condition often goes undiagnosed. A mobile application was developed to measure aspects of visual acuity and oculomotor function. The aim of this project was to validate the newly developed suite of outcomes conducive for use in the field to detect visual dysfunction.
MATERIALS AND METHODS: A custom mobile application was developed on an Apple iPad using iOS operating system software version 11.0 in Objective C to measure near point of convergence (NPC), distance visual acuity, reading flue

An oculogyric crisis is a tonic conjugated deviation of the eyes, usually upward. We present two cases with a drug induced oculogyric crisis. The differential diagnoses should include epilepsy, a functional neurological movement disorder, ocular tics, ocular dyskinesia or ocular bobbing. Typically, in an oculogyric crisis the patient's awareness is intact; accompanied signs can be blepharospasm, neck flexion, jaw opening with or without tongue protrusion and autonomic symptoms. The underlying pathophysiology seems an imbalance between cholinergic and dopaminergic pathways. Most frequently an o

Blunt trauma in the orbital region often results in fracture of 1 or more bone walls resulting in alteration of the orbital volume with the potential for visual sequelae. This study reports a rare case of displacement of the eyeball into the maxillary sinus after blunt trauma as well as its initial management, treatment, and clinical follow-up. Victims of eyeball displacement have a poor visual prognosis. Even if the patient has already been diagnosed with amaurosis on arrival to the hospital, all measures for rapid eye repositioning and reconstruction of the orbital cavity must be instituted,

BACKGROUND: Eye movements are one of the most complex motor functions of the central nervous system (CNS). Eye movement disorders including nystagmus occur in diseases of the CNS and the vestibular system. A systematic clinical examination often allows a topodiagnostic classification of the lesion.
OBJECTIVE: The basics of eye movements, the role of the cerebellum, the clinical examination of the oculomotor system, and the most important forms of nystagmus and their diagnostic implications are described.
MATERIALS AND METHODS: A literature analysis assessing eye movements, cerebellar control

The brainstem contains ocular motor and vestibular structures that, when damaged, produce specific eye movement disorders. In this review, we will discuss three brainstem syndromes with characteristic ocular motor and vestibular findings that can be highly localizing. First, we will discuss the lateral medullary (Wallenberg) syndrome, focusing on ocular lateropulsion, saccadic dysmetria, and the ocular tilt reaction. Second, we will review the medial longitudinal fasciculus syndrome including the ocular tilt reaction, nystagmus, and the vestibular-ocular reflex. Lastly, we will discuss hypertr

INTRODUCTION: The eyes are a window to the brain' is a maxim that holds true especially in the intensive care setting. Recognising specific eye signs aids rapid decision-making regarding diagnosis or prognosis. Eye signs play a pivotal role in intensive care for the neurologist.
STATE OF THE ART: Eye signs have long been considered the best clinical clue for assessment of a comatose patient. In critically ill patients, the recognition of brainstem involvement hinges primarily on eye signs. The ability to recognise and interpret these signs goes a long way towards ensuring proper care of neuro

Brown's syndrome is related to an abnormality of the superior oblique muscle: it is manifested by an oculomotor disorder with active and passive limitation of elevation in adduction, the field of action of the inferior oblique muscle. The origin is congenital or acquired secondary to multiple causes - inflammatory-infectious, traumatic or iatrogenic. The clinical and paraclinical signs are suggestive. Cerebral and orbital imaging including CT and magnetic resonance imaging (MRI) is essential for the diagnosis and management of congenital or acquired Brown's syndrome. The course may yield impro

BACKGROUND: The brainstem contains numerous structures including afferent and efferent fibers that are involved in generation and control of eye movements.
EVIDENCE ACQUISITION: These structures give rise to distinct patterns of abnormal eye movements when damaged. Defining these ocular motor abnormalities allows a topographic diagnosis of a lesion within the brainstem.
RESULTS: Although diverse patterns of impaired eye movements may be observed in lesions of the brainstem, medullary lesions primarily cause various patterns of nystagmus and impaired vestibular eye movements without obvious o

PURPOSE OF REVIEW: Eye-movement research continues to provide an excellent tool for understanding the central control of motor function, both in health and disease. This article reviews recent findings in relation to saccadic eye movements, particularly antisaccades and microsaccades, with particular emphasis on the control of inaction, something which has recently become topical.
RECENT FINDINGS: Microsaccades are under the control of the cerebral cortex, particularly the frontal and parietal eye fields. Their frequency and direction alters following presentation of visual stimuli. Spontaneo

The European Neuromuscular Centre (ENMC) derived the term Congenital Cranial Dysinnervation Disorders in 2002 at an international workshop for a group of congenital neuromuscular diseases. CCDDs are congenital, non-progressive ophthalmoplegia with restriction of globe movement in one or more fields of gaze. This group of sporadic and familial strabismus syndromes was initially referred to as the 'congenital fibrosis syndromes' because it was assumed that the primary pathologic process starts in the muscles of eye motility. Over the last few decades, evidence has accumulated to support that the

Parinaud's syndrome, also known as dorsal midbrain syndrome, includes multiple clinical signs, with the most prominent being paralysis of upward gaze. This syndrome has most commonly been described in association with pineal region masses. Other relatively common causes of this syndrome include hydrocephalus, intrinsic midbrain masses, and ischemic lesions. MRI is very helpful in determining the cause and thus guiding appropriate treatment.
