Osteoclasts; culprits in inflammatory osteolysis.

Periarticular osteolysis, a crippling complication of rheumatoid arthritis, is the product of enhanced osteoclast recruitment and activation. The osteoclast, which is a member of the monocyte/macrophage family, is the exclusive bone resorptive cell, and its differentiation and activation are under the aegis of a variety of cytokines. Receptor activator of NF-kappaB ligand (RANKL) and macrophage colony-stimulating factor are the essential osteoclastogenic cytokines and are increased in inflammatory joint disease. Tumor necrosis factor-alpha, which perpetrates arthritic bone loss, exerts its ost

[Prognosis of total hip replacement: the importance of an implant register].

The survival of total hip arthroplasty is mainly determined by the loosening of implants. Wear particles generated at the articular contact of head and cup are engulfed by macrophages. These activated macrophages induce the production of RANKL, leading to proliferation and activation of osteoclasts resulting in osteolysis around the hip implant. Higher activity leads to increased particles production, which explains the lower survival rate of total hip arthroplasties in young patients compared to older patients. Implant fixation with bone cement still is an excellent technique, but uncemented

Complex Anterior Cruciate Ligament Revision and Lateral Extra-Articular Tenodesis With Achilles Tendon Allograft: The "Monoloop" Technique.

Revision of anterior cruciate ligament reconstruction (ACL-R) presents many challenges that are not encountered in the primary setting and, therefore, requires thorough preoperative planning. Recently, there has been growing evidence showing that combining the anterolateral ligament (ALL) reconstruction with ACL-R revision reduces the risk of postoperative ACL rupture and meniscal tears, and therefore, the ALL reconstruction becomes essential to a complex ACL revision. The technique that we describe is mainly used in the setting of complex ACL revision with extensive tunnel osteolysis associat

An unusual case of disappearing bone disease in the mandible and literature review.

Disappearing bone disease is a rare disorder that causes spontaneous and progressive osteolysis and proliferation of lymphatic tissue and blood vessels. It is debilitating and there is evidence of mortality in reported cases including those affecting mandible. A 38-year-old man was diagnosed with disappearing bone disease of the mandible in 2013. This had progressed from being in the right ramus only, and now extends to the lower right first molar and affects the left coronoid process and ramus. Mandibular involvement has shown to be high-risk. In older patients, misdiagnosis can occur without

Gorham-Stout disease: good results of bisphosphonate treatment in 6 of 7 patients.

Background and purpose - Gorham-Stout disease (GSD) is a rare mono- or polyostotic condition characterized by idiopathic intraosseous proliferation of angiomatous structures resulting in progressive destruction and resorption of bone. Little is known about the course of disease and no previous study has evaluated patients' quality of life (QoL).Patients and methods - This is a retrospective analysis of 7 consecutive patients (5 males) with a median age at diagnosis of 14 years and a median follow-up of 7 years who were diagnosed with GSD in our department between 1995 and 2018. Data regarding

Primary versus trauma-induced Gorham-Stout disease.

Gorham-Stout disease - also known as "disappearing bone disease" is currently considered a single entity with varying clinical manifestations. We reviewed the existent literature from the earliest historic description(Jackson in 1838) and Gorham and Stout's original series of patients, multiple case reports and series since. After analyzing 212 reported cases, we identified 76 cases with details that recorded either a history of multifocal disease or an identifiable history of preceding trauma. From this review, we have defined two distinct Gorham-Stout entities - those characteristically asso

Gorham disease of the mandible.

Gorham disease, or massive osteolysis, is a rare condition of unknown etiology. The disease is characterized by spontaneous progressive osteolysis of 1 or more skeletal bones. The mandible is the most commonly involved bone in the maxillofacial region. This article reports a case of Gorham disease with mandibular involvement in a 46-year-old male patient with a 7-year follow-up. In this case, we performed lower right mandibular osteotomy and reconstruction with a phased titanium plate. Postoperative follow-up showed continued mandibular bone loss that was progressing to the contralateral mandi

Carpal-tarsal osteolysis with elbow involvement.

Carpal-tarsal osteolysis is a rare condition that manifests as the progressive resorption of carpal and tarsal bones in young children. The diagnosis of this condition is often difficult and delayed as the initial clinical presentation is non-specific. Radiographic changes occur gradually, are often not seen at presentation and depend on recognising loss of bone in the ossification centres of the carpus and tarsus. MRI demonstrates morphological abnormalities in the cartilaginous, as well as the osseous components, of the developing carpal and tarsal bones and therefore may be helpful in preda

Hajdu--Cheney syndrome: evolution of phenotype and clinical problems.

Hajdu-Cheney syndrome is a rare, autosomal dominant disorder comprising acroosteolysis of the distal phalanges with associated digital abnormalities, distinctive craniofacial and skull changes, dental anomalies, and proportionate short stature. The clinical and radiologic characteristics of Hajdu-Cheney syndrome develop and progress with age. Many of the medical problems that arise in this syndrome cluster in specific age ranges. Case reports of six affected individuals in two additional families and a summary of the English literature is presented with emphasis on the changing physical findin

Massive Craniofacial Gorham Disease Treated Successfully by Cisplatin and 5-Fluorouracil With Ten Years of Follow-Up: A Case Report and Literature Review.

Gorham disease (GD) is a rare osteolytic condition of unknown etiology that causes spontaneous, progressive bone resorption. The maxillofacial area is one of the regions most frequently involved in this disease. GD is characterized by its aggressiveness and rarity; therefore, the treatment modalities remain controversial, and no specific treatment method has been proved effective. The present report describes a case of GD with massive craniofacial bone involvement that was treated effectively using a combination of 5-fluorouracil and cisplatin, with 10 years of follow-up.

Current concepts of hip arthroplasty for radiologists: part 2, revisions and complications.

OBJECTIVE: This article reviews the imaging features of revisions and complications of hip replacement arthroplasty and relates these features to the current understanding about how and why these failures occur. CONCLUSION: Short-term failures of hip replacements are most commonly the result of instability and dislocation. Complications ranging from osteolysis caused by granulomatous reaction to particulate wear debris lead to many long-term failures. Attempts to reduce wear debris through changes in design and materials have reduced the rate of some complications but have resulted in new one

Massive femoral osteolytic lesion secondary to disengagement of a total knee tibial locking pin: case report.

The Anatomic Modular Knee (AMK; Depuy, Warsaw, Ind) was one of the first designs to incorporate a modular locking mechanism for the tibial insert; fixation was secured using a screw-in tibial tray-locking pin. This case report describes a patient who presented with instability and worsening knee pain 15 years following a primary total knee arthroplasty using the AMK prosthesis. Radiographs and intraoperative assessment revealed proximal migration of the tibial tray-locking pin into the medial femoral condyle resulting in a large osteolytic defect.

Biparietal osteodystrophy.

Following an exhaustive literature review of 126 cases, a personal series of 8 instances is presented. The following inferences are drawn: 1) the condition is not an anatomical variant, but a pathological lesion. 2) the lesion occurs in all races, and at all latitudes; its prevalence can be estimated to range between 0.4 and 0.5%. 3) the lesion occurs predominantly in females (sex ratio male: female = 1 : 2.5) and after the age of 60. 4) histological evidence indicated erosion of the external table, compensated by remodeling of the diploe, with resultant loss of the diploe. 5) Neither senility

Osteolytic form of Paget's disease. Differential diagnosis and pathogenesis.

Of fourteen patients with Paget's disease for whom adequate roentgenograms and pathological material were available, six had lesions showing significant or predominant osteolysis. Based on a study of these six patients and a review of the literature, the following mechanisms were identified as causes of the so-called osteolytic type of the disease: (1) an "early" destructive Paget's lesion; (2) "advanced" Paget's disease with secondary degenerative changes; (3) "seeding" of an independent osteolytic lesion, particularly tumor in pre-existing Paget's disease; (4) sarcomatous transformation; and
