We have used the intravascular oxygenator (IVOX) in eight patients (14-76 years) with acute respiratory failure. At the time of referral for IVOX all patients were on inotropic support, five had pneumothoraces and two were on haemofiltration. Ventilatory data before IVOX were: ventilated for 2-14 days (median 6), PEEP 5-12 cm H2O (12), PIP 34-95 cm H2O (49), FiO2 0.7-1 (1), PaO2 5.4-26 KPa (8.5) and PaCO2 3.7-23 KPa (8). Transport of critically ill neonates in need of extracorporeal membrane oxygenation can be risky. Their extreme cardiorespiratory instability may delay or even preclude conventional transport to an extracorporeal membrane oxygenation center. We report the use of a UH-1 helicopter specially adapted for mobile extracorporeal membrane oxygenation support to transport a critically ill neonate.

Superoxide production was measured as the superoxide dismutase (SOD)-inhibitable portion of nitro blue tetrazolium (NBT) reduction after cerebral ischemia-reperfusion in anesthetized cats equipped with cranial windows. From 1964-1987 we have examined 31 patients suspected to have gaseous gangrene. The diagnosis was confirmed 13 times on clinical and microbiologic grounds. 61% of the patients underwent amputation, 28% died. On the basis of this experience we have tried to define a therapeutic attitude. We recommend an adequate debridement, i.e. the removal of necrotic tissue and drainage of the wound, microbiological investigations and the administration of antibiotics before hyperbaric oxygen treatment is begun (2-3 treatments over 3 days).

We have studied patterns of oxygen saturation (SpO2) before and after thoracotomy in 20 patients monitored nightly from the preoperative night to the fourth postoperative night. After operation, 10 patients received paravertebral bupivacaine (PVB) infusion and 10 received paravertebral saline (PVS) infusion. Papaveretum was given as required. It depends on the successful interaction of the respiratory and cardiovascular system in order to deliver O2 in sufficient amounts and at a high enough capillary partial pressure to allow for adequate diffusion into the cell. Oxygen saturation and end-tidal CO2 tension were monitored in 15 healthy women during labor. Oxygen saturation was determined with a pulse oximeter and end-tidal CO2 with a CO2 monitor. Evidence is provided that mutagenicity in Salmonella by a chromium(VI) salt and a chromium(III) compound has a differential dependence on the presence of oxygen. The mutagenic chromium(III) compound, cis-dichlorobis(2,2'-bipyridyl)chromium(III), reverted Salmonella strains, TA102 and TA2638, only under aerobic conditions. Extracorporeal membrane oxygenation (ECMO) is used in the treatment of reversible pulmonary disease in the newborn. The ECMO program at Wilford Hall USAF Medical Center began in 1985 and to date, 57 patients have been placed on bypass for a mean of 125 hours. The indications for ECMO are severe, prolonged hypoxemia in patients with an estimated mortality of greater than 90% using conventional ventilator support. Hyperbaric oxygen (HBO) therapy is presently used in clinical practice as a primary mode of treatment or as an adjunct to conventional therapy. In recent years, a number of bewildering array of skin disorders are being treated with HBO. HBO is found useful in chronic non healing ulcers, bacterial infections and fungal infections.
