Bilateral Parotid Tuberculous Lymphadenitis Mimicking Warthin Tumor in an Elderly Male: A Case Report.

Tuberculosis involving the parotid lymph nodes is rare and can clinically and radiologically mimic benign neoplasms such as Warthin tumor, particularly when bilateral. We present the case of a 72-year-old male with bilateral parotid swellings, initially suspected to be Warthin tumors based on magnetic resonance imaging (MRI) findings showing well-defined, cystic lesions with focal solid components and central necrosis. Histopathological examination following ultrasound-guided biopsy revealed caseating granulomas consistent with tuberculous lymphadenitis. This case highlights the importance of

Outcomes and prognosticators in regionally recurrent cutaneous squamous cell carcinoma of the head and neck.

INTRODUCTION: Data regarding regionally metastatic cutaneous squamous cell carcinoma of the head and neck (cSCCHN) is limited and derived almost exclusively from Australian and United States (US) institutions. We report the first United Kingdom perspective, with the aims of benchmarking survival outcomes and identifying clinically relevant prognosticators. MATERIALS AND METHODS: Ninety-one patients with regionally recurrent cSCCHN treated with curative intent over a ten-year period (2009-2018) were studied retrospectively. Time-to-event analyses were used to estimate oncological outcomes, and

Unilateral Cysticercosis of the Parotid Gland: Case Report, Diagnosis, and Clinical Management.

Cysticercosis is a systemic parasitic infection caused by the establishment of the larval form of the parasitic cestode, Taenia solium. Cysticercosis is acquired via the fecal-oral route and is prevalent in low- and middle-income countries (LMICs). Patients typically manifest with skeletal muscle, subcutaneous, or central nervous system involvement. Though there are reports of oral mucosa involvement, solitary involvement of the parotid gland is rare. This is a rare case of a 57-year-old man diagnosed with parotid cysticercosis by imaging and FNA. He was successfully treated by anthelminthic t

125I seed implant brachytherapy-assisted surgery with preservation of the facial nerve for treatment of malignant parotid gland tumors.

The surgical treatment of malignant parotid gland tumors combined with (125)I seed implant brachytherapy and preservation of the facial nerve is described. Tumor and parotid gland resection with preservation of the facial nerve was carried out in 12 patients with malignant parotid gland tumors. (125)I seeds were implanted into the target area intra- or postoperatively. The extent of regional control of the tumor was followed up, and facial nerve function was evaluated. None of the patients had tumor recurrence during the follow-up period of 50-74 months (median follow-up period, 66 months). Fa

Ca2+ release dynamics in parotid and pancreatic exocrine acinar cells evoked by spatially limited flash photolysis.

Intracellular calcium concentration ([Ca(2+)](i)) signals are central to the mechanisms underlying fluid and protein secretion in pancreatic and parotid acinar cells. Calcium release was studied in natively buffered cells following focal laser photolysis of caged molecules. Focal photolysis of caged-inositol 1,4,5 trisphosphate (InsP(3)) in the apical region resulted in Ca(2+) release from the apical trigger zone and, after a latent period, the initiation of an apical-to-basal Ca(2+) wave. The latency was longer and the wave speed significantly slower in pancreatic compared with parotid cells.

Multifocal nodular oncocytic hyperplasia of bilateral parotid glands: A case report with a histological variant of clear cells.

A case of multifocal nodular oncocytic hyperplasia (MNOH) of the bilateral parotid gland is presented. An 80-year-old woman was admitted to hospital because of painless swellings in bilateral parotid regions. Histologically, the nodular lesion had incomplete capsules and engulfed the surrounding parotid gland at the periphery. The lesions were mostly composed of clear cells, while the peripheries of the lesions had typical oncocytic cells with abundant fine granules. The histological existence of the clear cell component in the lesions led to misdiagnoses of other clear cell neoplasms. However…

A Rare Case of Multifocal Cystadenomatous Oncocytic Hyperplasia of the Parotid Gland.

Multifocal cystadenomatous oncocytic hyperplasia of the parotid gland is an extremely rare non-neoplastic lesion. We present a case involving a 73-year-old female with a painless, small mass in her right preauricular region for the past two months. Fine needle aspiration suggested a diagnosis of mucoepidermoid carcinoma. Following further investigations, the patient underwent an uneventful right superficial parotidectomy without neck dissection. Histopathological examination of the excised superficial parotid tissue revealed multifocal cystadenomatous oncocytic hyperplasia with negative surrou

Parotid salivary gland adenocarcinoma with bilateral ocular and osseous metastases in a dog.

A case of primary parotid salivary gland adenocarcinoma is described in a three-year-old entire female great dane. The presenting complaint was sudden onset blindness concomitant with the development of bilateral hyphaema and anterior uveitis. A soft-tissue swelling of the left subaural region and right forelimb lameness were also present. A definitive diagnosis of primary parotid salivary gland adenocarcinoma with bilateral ocular metastases was made on post mortem examination. Metastases were also present in the right proximal humerus, left and right prescapular lymph nodes, left axillary ly

Extraglandular Warthin's tumors.

Warthin's tumor or papillary cystadenoma lymphomatosum (PCL) represents a clinically significant entity, not because of pernicious behavior, but because its cytohistological characteristics tell us much about the development and morphology of the parotid region. Three cases operated on in which PCL developed in an extraglandular location from the parotid, formed the basis for a series of observations on what is known about: 1. etiology of PCL; 2. the nature of the lymphocytic components; 3. the relatively high incidence of bilateral and multiple occurrence, as well as male predominance. These …

Follicular Dendritic Cell Sarcoma of the Parotid Gland: A Case Report and Review of Literature.

Follicular dendritic cell sarcoma of the parotid gland is an extremely rare tumor, with only six cases reported in the literature. A 51-year-old female had a 3.0 cm tumor resected from the right parotid gland. The tumor exhibited solid sheets, whorls, fascicular pattern, and syncytium appearance with an indistinct cell border. The lymphocytic infiltrate was sprinkled throughout the neoplasm, with focal prominent perivascular cuffing. Immunohistochemically, it was positive for follicular dendritic cell markers CD21, CD23, and CD35. We aim to enhance the understanding of this neoplasm and alert

The Youngest Reported and Successfully Treated Patient with a Dermoid Cyst of the Parotid Gland: A Rare Pediatric Case.

Dermoid cysts (DCs) are rare benign, epithelial-lined lesions. Up to 7% of them are found in the head and neck region and 80% of those predominantly occur in the orbit, in the nose, and in the floor of the mouth. The average age of presentation is around the age of six. Dermoid cysts located in the parotid gland have only been published in 19 cases so far. Interestingly, the mean age of occurrence in the parotid gland was much higher (31 years). We report on a four-year-old girl being the youngest patient who had ever been diagnosed with this disease.

Acute suppurative parotitis caused by Streptococcus pneumoniae in an HIV-infected man.

We report a case of a 32-year-old man who presented with progressive unilateral parotid gland enlargement and subsequently tested positive for HIV. A CT scan of the neck performed with contrast showed a phlegmon in the region of the right parotid tail measuring approximately 2.5×2.4 cm. Cultures of the aspirated fluid grew Streptococcus pneumoniae and the S. pneumoniae urinary antigen test was also positive. The patient underwent surgical debridement and received antimicrobial therapy with complete resolution of the parotitis. Parotitis caused by S. pneumoniae is rare, and HIV infection should

Congenital Sialolipoma in an Infant.

Sialolipoma is a newly recognized tumor of the major and minor salivary glands and represents only 0.3% of all salivary gland tumors. Only 3 cases of congenital sialolipoma are available in the literature. In the current case, we performed a total parotidectomy with facial nerve preservation on a 12-week-old infant exhibiting huge mass in the parotid region. Histopathology results showed sialolipoma. There was no recurrence at the 18-month follow-up. Although it is a very rare disease in infants, congenital sialolipoma should be kept in mind in patients with parotid mass. The primary treatment…

An unusual case of epithelial-myoepithelial carcinoma of the parotid gland radiologically simulating a benign lesion.

Epithelialmyoepithelial carcinoma (EMEC) is a rare low-grade malignant salivary gland neoplasm that most commonly occurs in the parotid gland but can also arise in minor salivary glands. Here, we present a case of EMEC in a 60-year-old male patient with a huge swelling in the left parotid gland region. Clinically and radiologically, it simulated a benign salivary gland neoplasm. However, fine-needle aspiration cytology and histologic examination revealed atypical myoepithelial cells in solid sheets or nests suggestive of epithelial-myoepithelial carcinoma. Diagnosis was further confirmed by po

First bite syndrome following ipsilateral carotid endarterectomy.

First bite syndrome (FBS) is characterized by unilateral pain in the parotid region after the first bite of each meal, usually following ipsilateral neck surgery. The proposed mechanism is sympathetic denervation of the parotid gland, from iatrogenic injury to the sympathetic trunk supplying this gland. Local botulinum toxin injection has emerged as a promising treatment option with favorable results. To date, there are 3 published cases in the literature describing FBS after carotid endarterectomy. We present a case of a 75-year-old gentleman who developed FBS after carotid endarterectomy, to…

Hemangiopericytoma of the parotid gland: a case report.

Hemangiopericytomas are uncommon neoplasms of vascular origin that mostly arise from extremities, retroperitoneum, and pelvic fossa. They are rarely found in the parotid gland. A 35-year-old female patient presented with a swelling in the right preauricular region. Fine-needle aspiration biopsy performed twice yielded an extraordinary amount of blood. A cytological diagnosis could not be made. Computed tomography showed a homogenous, well-shaped mass in the parotid gland. Superficial parotidectomy was performed. Histopathological diagnosis was made as hemangiopericytoma. No evidence for local

Shear Wave Elastography for Parotid Glands: Quantitative Analysis of Shear Elastic Modulus in Relation to Age, Gender, and Internal Architecture in Patients with Oral Cancer.

BACKGROUND: Recently, shear wave elastography (SWE) has been recognized as an effective tool for evaluating Sjögren's syndrome (SS) patients. The purpose of this study was to assess the parotid glands with SWE, especially for quantitative analysis of shear elastic modulus in relation to age, gender, and internal architecture in patients with oral cancer to collect control data for SS. METHODS: In total, 124 parotid glands of 62 patients with oral cancer were evaluated with SWE. The parotid glands were examined for the internal architecture (homogeneous or heterogeneous) on B-mode. The SWE all

[Case report: tuberculosis of parotid gland].

Primary tuberculosis of the parotid gland is an unusual clinical presentation. In this report a 32 years old male patient with parotid gland tuberculosis has been presented. The patient has been admitted to Ear, Nose & Throat outpatient clinic of our hospital with the complaint of left facial nodule. Histopathologic examination of the needle aspiration biopsy (NAB) specimen yielded benign necrotic lenfoid tissue, and in cervical ultrasonography cystic formations in left parotid gland were detected. Since the nodule size has increased in the follow-up period, cervical tomography was performed a

First-Bite Syndrome: A Rare Complication of Carotid Endarterectomy.

This report describes the rare complication of first-bite syndrome (FBS) after carotid endarterectomy (CEA). Although FBS is well known in otolaryngologic literature, it is rarely discussed in relation to vascular surgery. FBS is most commonly a postoperative pain syndrome that is thought to be the result of selective parotid gland sympathetic denervation. The resultant facial pain is centered around the parotid region and triggered by initiation of mastication. The pain is severe, but short in duration, and quite specific in pattern. We present a case of FBS after CEA with complex anatomy. Th…

Ultrasound Elastography in Diffuse and Focal Parotid Gland Lesions.

In Europe, ultrasound (US) is the diagnostic gold standard for the visualisation of parotid gland disorders. While high-resolution B-mode US reliably visualises focal lesions, its diagnostic capacity to distinguish tumour entities or to characterise diffuse lesions remains poor. Strain elastography (SE) and shear wave elastography (SWE) are novel imaging techniques that provide additional information about tissue elasticity. SE analyses the strain or displacement in response to force or compression and, therefore, shows the relative tissue elasticity within a selected region of interest (ROI).
