Self-enucleation of the right eye by a 38-year-old woman diagnosed with schizoaffective disorder: a case report.

BACKGROUND: Autoenucleation is a rare form of self-mutilation typically associated with psychiatric disorders such as schizophrenia, substance-induced psychosis and bipolar disorder. The act is usually unilateral, although bilateral attempts are also well documented in the literature. CASE PRESENTATION: It is a case study involving a female patient (NN) diagnosed with schizoaffective disorder who self-enucleated her right eye following sexual intercourse with a fellow patient, and was forcefully prevented by staff from enucleating the second eye. We report recurrent episodes of her illness cu

Perspectives on the HIV continuum of care among adult opioid users in New York City: a qualitative study.

BACKGROUND: Engagement in the HIV care continuum combined with office-based opioid treatment remains a cornerstone in addressing the intertwined epidemics of opioid use disorder (OUD) and HIV/AIDS. Factors influencing patient engagement with OUD and HIV care are complex and require further study. METHODS: In this qualitative study, in-depth interviews were conducted among 23 adult patients who use drugs (PWUD) in an inpatient detoxification program in New York City. The semi-structured interview guide elicited participant experiences with various phases of the HIV care continuum, including fa

Critically Exploring Self-Harm Through Lived Experience Perspectives: A Survivor-Controlled Integrative Review Employing Participatory Methodology.

INTRODUCTION: In psychiatric knowledge, self-harm is commonly understood as an individual, self-destructive behaviour arising from emotional dysregulation where sociocultural context is referred to briefly but typically overlooked. Conversely, many with lived experience (LE) highlight relational and systemic trauma, abuse and violence as inseparable from self-harm. This survivor-controlled integrative review was led by a LE researcher and employed participatory methodology with people with LE to explore self-harm. Our research question was 'How is self-harm conceptualised?'. METHODS: Evidence

Introducing Key Performance Metrics for Assessing the Benefits and Harm of the Prostate Cancer Diagnostic Pathway.

Traditional accuracy metrics no longer suffice for comparing diagnostic pathways for prostate cancer; new comparative performance metrics are needed that prioritize the overall benefit-to-harm ratio. Key performance metrics, such as biopsy efficiency, biopsy (grade) selectivity, and biopsy avoidance safety, enable meaningful patient-level comparisons across studies. Standardized benefit-to-harm metrics reporting enhances quality assurance, informs personalized diagnostics, and supports integration into clinical guidelines to improve the safety of prostate cancer diagnosis.

A continuum of disconnection: the impacts of safer supply program closures on continuity of care, transition, and total loss of service access.

OBJECTIVE: In March 2025, the expiration of federal pilot funding for Safer Supply Programs (SSPs), combined with restrictive provincial legislative changes to harm reduction service provision in Ontario, disrupted care for thousands of clients at risk of toxic drug poisoning. This study examines the impacts of these changes on client health, social stability, and overdose risk across a continuum of access. METHOD: We conducted 30 semi-structured virtual interviews between September and November 2025 with clients who were enrolled in an SSP prior to March 31, 2025. Participants were recruited

Diagnosis and Management of Opioid Use Disorder in Hospitalized Patients.

The diagnosis of opioid use disorder (OUD) is often overlooked or inadequately managed during the inpatient admission. When recognized, a common strategy is opioid detoxification, an approach that is often ineffective and can be potentially dangerous because of loss of tolerance and subsequent risk for overdose. Medication for addiction treatment (MAT), including methadone and buprenorphine, is effective and can be dispensed in the hospital for both opioid withdrawal and initiation of maintenance treatment. Hospitalists should be knowledgeable about diagnosing and managing patients with OUD, i

Implicit cognitions on self-injurious and suicidal behavior in borderline personality disorder.

BACKGROUND AND OBJECTIVES: Performance on implicit measures of suicidality has been associated with suicidal and nonsuicidal self-injury. Despite the high prevalence of self-harm in patients with borderline personality disorder (BPD), no previous study has assessed implicit measures in this patient group. METHODS: Forty patients with BPD and 25 healthy controls completed three implicit association tests (IATs) (Death words - Me/Others words, Self-Harm pictures - Me/Others, and Self-Harm pictures - Good/Bad words) and a subliminal priming task (effect of the primes "dying"/"growing" on the cat

Randomized controlled trial of harm reduction treatment for alcohol (HaRT-A) for people experiencing homelessness and alcohol use disorder.

BACKGROUND: People experiencing homelessness are disproportionately affected by alcohol use disorder (AUD). Abstinence-based treatment, however, does not optimally engage or treat this population. Thus, harm reduction treatment for alcohol (HaRT-A) was developed together with people with lived experience of homelessness and AUD and community-based agencies that serve them. HaRT-A is a compassionate and pragmatic approach that aims to help people reduce alcohol-related harm and improve quality of life (QoL) without requiring abstinence or use reduction. A three-month, two-arm randomized control

Role of primary care in supporting older adults who self-harm: a qualitative study in England.

BACKGROUND: Self-harm and suicide are major public health concerns. Self-harm is the strongest risk factor for suicide, with the highest suicide rates reported in older populations. Little is known about how older adults access care following self-harm, but they are in frequent contact with primary care. AIM: To identify and explore barriers and facilitators to accessing care within primary care for older adults who self-harm. DESIGN AND SETTING: An exploratory qualitative methods study using semi-structured interviews with older adults and third-sector workers in England. Older adults were

Emotional Distress and Attitudinal Changes in Nursing Students During the Psychiatric Practicum: Impact of Patient Aggression.

PURPOSE: This study aimed to examine changes in nursing students' emotional distress and attitudes toward mental illness during their psychiatric nursing practicum, paying particular attention to the influence of students' personality traits, patients' aggressive behaviour and diagnoses on such changes. METHODS: Ninety nursing students completed self-reported questionnaires before and after their psychiatric practicum. This prospective study examined changes in emotional distress, assessed using the Beck Anxiety Inventory (BAI) and the Beck Depression Inventory II (BDI-II), and changes in att

Self-poisoning as a cause of admission in a medical intensive care unit and a question of misuse of prescription medications.

BACKGROUND: Self-poisoning accounts for between 3 and 17% of intensive care unit (ICU) admissions. OBJECTIVES: To display the misused substances, intention, and outcome of patients admitted to the ICU due to self-poisoning. METHODS: A retrospective review of ICU admissions using an electronic patient database was performed. RESULTS: A total of 149 patients were admitted, accounting for 4.6% of ICU admissions. Self-poisoning with self-harm was more frequent than without self-harm intent (62.4% vs. 37.6%). 64.4% of ingested substances were misused prescribed drugs. Females used antipsychotics

Exploring behaviours of concern including aggression, self-harm, sexual harm and absconding within an Australian inpatient mental health service.

OBJECTIVE: Patients admitted to mental health services may exhibit behaviours of concern (BOCs) such as aggression, self-harm, absconding and sexual harm. BOCs can lead to restrictive interventions, which have adverse effects on patients, carers and staff. This paper aims to explore the nature and outcome of BOCs within an adult inpatient mental health setting. METHODS: A retrospective audit was conducted at a metropolitan inpatient service between 1 August 2016 and 31 July 2017. The frequency, nature and outcomes of BOC episodes were described and thematic analysis was used to summarise BOC

Self-harm by Sharp Foreign Body Ingestion.

OBJECTIVE: Ingestion of sharp foreign bodies is a less frequently encountered method of selfharm. This study aims to follow up patients presenting with foreign body (FB) ingestion requiring urgent endoscopic intervention under general anaesthetic in theatre, characterise each patient and establish whether admission or removal of the FB promotes further self-harm. METHOD: Eleven adult patients, totalling 38 presentations over a 28-month period were identified. Each was followed up from an initial presentation requiring endoscopy. RESULT: 9/11 (81.9%) where transferred either from prison or ps

Patient safety incidents in the psychiatric inpatient setting: determinants, consequences, and strategies. A systematic review.

INTRODUCTION: Patient safety in psychiatric inpatient settings remains an underexplored area despite the heightened vulnerability of this population to preventable harm. This review aimed to provide an updated and comprehensive overview of Patient Safety Incidents (PSIs) in psychiatric inpatient settings, identifying their types, contributing factors, preventive strategies, consequences, and mitigating actions. METHODS: A systematic search was conducted in PubMed, Embase and Scopus for primary studies published from 2000 onward. A total of 92 studies were included. Data were synthesized using

Differential effects of manual assisted cognitive behavior therapy in the treatment of recurrent deliberate self-harm and personality disturbance: the POPMACT study.

A total of 480 patients were treated in a large, multicenter randomized trial of a brief form of cognitive therapy, manual-assisted cognitive behavior therapy (MACT) versus treatment as usual (TAU) for recurrent deliberate self-harm. Each patient was randomized after a self-harm episode assessed at an accident and emergency center and followed up over 1 year. The main hypothesis tested was that those allocated to MACT would have a lower proportion of self-harm episodes in the succeeding year. A total of 60% of those allocated to MACT had face-to-face treatment and 430 (90%) of all patients had

A patient-initiated voluntary online survey of adverse medical events: the perspective of 696 injured patients and families.

BACKGROUND: Preventable medical errors continue to be a major cause of death in the USA and throughout the world. Many patients have written about their experiences on websites and in published books. METHODS: As patients and family members who have experienced medical harm, we have created a nationwide voluntary survey in order to more broadly and systematically capture the perspective of patients and patient families experiencing adverse medical events and have used quantitative and qualitative analysis to summarise the responses of 696 patients and their families. RESULTS: Harm was most c

Continuum of hepatitis C care cascade in prison and following release in the direct-acting antivirals era.

BACKGROUND: People with criminal justice involvement contribute remarkably to the rising hepatitis C virus (HCV) burden; however, the continuum of care is a major barrier to prison-based programs. We aimed to evaluate a comprehensive HCV care model in an Iranian provincial prison. METHODS: Between 2017-2018, in the Karaj Central Prison, newly admitted male inmates received HCV antibody testing and venipuncture for RNA testing (antibody-positive only). Participants with positive RNA underwent direct-acting antiviral (DAA) therapy (Sofosbuvir/Daclatasvir). Sustained virological response was eva
