Causes and predictors of 30-day readmissions in patients with cardiogenic shock requiring extracorporeal membrane oxygenation support.

BACKGROUND: Cardiogenic shock is associated with significant mortality, morbidity, and healthcare cost. Utilization of extracorporeal membrane oxygenation in cardiogenic shock has increased in the United States. We sought to identify the rates and predictors of hospital readmissions in patients with cardiogenic shock after weaning from extracorporeal membrane oxygenation. METHODS: Using the 2016 Nationwide Readmission Database, we identified all patients (⩾18 years) with cardiogenic shock (ICD-10 CM R57.0) that have been implanted with extracorporeal membrane oxygenation (ICD-10-PSC of 5A1522

Outcomes and readmissions after transcatheter and surgical aortic valve replacement in patients with cirrhosis: A propensity matched analysis.

BACKGROUND: The data on the comparative outcomes and readmissions after transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) in patients with cirrhosis are limited. We compared mortality, complications, discharge disposition, 30-day readmission rates, length of stay, and cost of hospitalization in cirrhotic patients undergoing TAVR and SAVR. METHODS: The National Inpatient Sample (NIS) and the National Readmission Database (NRD) were used for the study. The International Classification of Diseases-9th version was used to define cohorts of patients undergo

Thirty-Day Readmission Rate Is High for Hospitalized Patients Discharged With Home Parenteral Nutrition or Intravenous Fluids.

BACKGROUND: Reducing hospital readmissions decreases healthcare costs and improves quality of care. There are no published studies examining the rate of, and risk factors for, 30-day readmissions for patients discharged with home parenteral support (HPS). OBJECTIVE: Determine the rate of 30-day readmissions for patients discharged with HPS and whether malnutrition and other demographic or clinical factors increase the risk. MATERIALS AND METHODS: Retrospective review of patients discharged with HPS from the Cleveland Clinic between July 1, 2013, and June 30, 2014, and followed by the Clevela

Multicenter randomised trial on home-based telemanagement to prevent hospital readmission of patients with chronic heart failure.

BACKGROUND: Chronic heart failure (CHF) remains a common cause of disability, death and hospital admission. Several investigations support the usefulness of programs of disease management for improving clinical outcomes. However, the effect of home-based telemanagement programs on the rate of hospital readmission is still unclear and the cost-effectiveness ratio of such programs is unknown. The aim of the study was to determine whether a home-based telemanagement (HBT) programme in CHF patients decreased hospital readmissions and hospital costs in comparison with the usual care (UC) follow-up

Nurse-initiated telephone follow-up on patients with chronic obstructive pulmonary disease improves patient empowerment, but cannot prevent readmissions.

INTRODUCTION: Readmissions reduce quality of life and increase mortality. Furthermore, disease severity and shortened length of stay make it difficult to support disease management during admission. The aim of this study was to explore whether telephone follow-up after discharge may reduce readmission rates, lower mortality and improve disease management in patients with chronic obstructive pulmonary disease (COPD). METHODS: This was a randomised controlled trial (n = 224) with nurse-initiated telephone intervention after discharge. On day 30, questionnaires about health status and perception

Preoperative psychotherapy for primary total hip arthroplasty patients: an evaluation of its modifiability on readmissions and implant complications in patients who have depression.

INTRODUCTION: Few studies have evaluated nonpharmacologic interventions, including preoperative psychotherapy, in patients who have depression. We studied whether psychotherapy visits prior to total hip arthroplasty (THA) in patients with depression were associated with lower: (1) readmissions; and (2) implant complications in patients who have depression. METHODS: A nationwide database was queried from 2010 to 2021. Patients who had depression were 1:1 propensity score matched based on those who had (n = 16,143) and did not have (n = 16,140) preoperative psychotherapy visits within 3 months

Body roundness index as a predictor of 3-month readmission in elderly patients with first-episode acute heart failure.

AIMS: To determine whether admission body roundness index (BRI), a validated anthropometric metric of central adiposity, independently predicts 3-month all-cause readmission and rehospitalization for acute heart failure (AHF) in elderly patients hospitalized for a first AHF episode. METHODS: In this single-center retrospective cohort study, 317 patients aged >60 years with a first AHF hospitalization were enrolled and stratified by BRI tertiles. The endpoints were all-cause readmission and AHF rehospitalization within 3 months. Patients who died during follow-up were retained in the cohort an

Admissions and readmissions among patients with congestive heart failure: the impact of pandemic waves in New South Wales, Australia.

OBJECTIVES: Australia is among the few large jurisdictions to experience more than one pandemic wave preceded by no incident COVID-19 cases. This enables research to estimate the immediacy, extent and duration of impact of those waves on initial admission and readmission rates for patients with congestive heart failure (CHF), as an example of high users of hospital services. METHODS: We used single and controlled interrupted time series designs, segmented regression analyses, and linked administrative data from 2016 to 2021. RESULTS: Patients with CHF experienced immediate and sizable reduct

Antiphospholipid antibodies among hospitalized patients predict mortality and readmission: A real-world 10-years cohort study.

OBJECTIVES: Antiphospholipid antibodies (aPL) are markers of increased morbidity and mortality in patients with antiphospholipid syndrome. However, the prognostic significance of aPL positivity in hospitalized patients without antiphospholipid syndrome remains unclear. This real-world study evaluated the independent clinical implications of aPL positivity in a large hospitalized population. METHODS: We conducted a retrospective cohort study of 4,801 hospitalized patients who underwent aPL testing at Sheba Medical Center between 2006 and 2018, after excluding patients with antiphospholipid syn

Association of glucose-lowering agents with readmission risk in patients with rheumatoid arthritis and comorbid type 2 diabetes mellitus.

This study evaluated the clinical value of a "immuno-metabolic co-management" strategy by investigating the association between glucose-lowering agents (metformin, insulin, and acarbose) and readmission risk in patients with rheumatoid arthritis (RA) and comorbid type 2 diabetes mellitus (T2DM). This study included up to 10 years of longitudinal follow-up data from 616 patients with RA and comorbid T2DM. A shared Gamma frailty model was utilized to analyze unplanned readmission. Furthermore, inverse probability of treatment weighting (IPTW) was employed to control for confounding, followed by

Association Between Complementary Use of Daikenchuto (a Japanese Herbal Medicine) and Readmission in Older Patients With Heart Failure and Constipation.

BACKGROUND: Constipation commonly coexists with heart failure (HF) and can increase blood pressure because of straining during defecation and accompanying mental stress. Daikenchuto, a Japanese herbal medicine to ameliorate gastrointestinal motility, may be effective as a complement to laxatives in improving outcomes in patients with HF and constipation. METHODS AND RESULTS: We used the Diagnosis Procedure Combination database to identify patients aged ≥65 years who were admitted for HF, had constipation, and were discharged alive between April 2016 and March 2022. We divided the 115,544 elig

Impact of sarcoidosis in patients undergoing aortic valve replacement: Insight from nationwide readmission database 2016-2019.

It is not well-known if valve replacement outcomes differ in patients with sarcoidosis, especially in aortic valve intervention, where the pressure gradients are physiologically high. In this retrospective study, we included all patients who underwent surgical/transcatheter aortic valve replacement from the Nationwide Readmission Database (2016-2019), and then divided them into those with and without sarcoidosis. Logistic and cox proportional hazard regression models were used. In-hospital mortality, stroke, acute kidney injury, paravalvular leak, 30-day pacemaker implantation, and 30-day hear

Identifying risk prediction models and predictors for hospital readmission in patients with medical conditions: A systematic review and meta-analysis.

BACKGROUND: Hospital readmission is a frequent adverse outcome among patients with medical conditions, with approximately 20% being readmitted within 30 days of discharge. However, the factors that significantly influence early readmission remain poorly understood, and it is unclear how to comprehensively evaluate predictors to identify patients at higher risk of readmission effectively. OBJECTIVE: To identify internally validated prediction models for all-cause readmission within 28-31 days of patients with medical conditions and to summarize the types of candidate and final predictors as we
