Patient safety incidents in mental health residential services: a multicenter, crosssectional, survey-based study.

INTRODUCTION: While patient safety in general hospitals is well-researched, residential mental health services have received less attention. Psychiatric patients and staff working in such facilities face unique and complex risks. This study aims to explore and describe the frequency and characteristics of patient safety incidents (PSIs) experienced by professionals working in a cohort of Italian mental health residential services. METHODS: A multicenter, cross-sectional, questionnaire-based observational study was conducted involving workers from 68 psychiatric mental health services from sev…

Healthcare Resilience Should Include the Worker-Patient Safety Loop: A Taiwan Psychiatric Hospital Perspective.

Research before and during COVID-19 has framed health-system resilience as the capacity to absorb shocks, adapt, transform and learn while sustaining essential functions. Building on this broader literature, including Meredith et al.'s account of local public-health adaptation under political and resource constraints, we propose a worker-patient safety loop from a Taiwan psychiatric hospital perspective: workforce strain can weaken safety-critical care; near misses, adverse events and continuity failures should trigger reporting, organisational learning and corrective action; and the resulting…

A novel, widespread impurity in mass-compounded tirzepatide/B12 products: potential patient safety implications.

BACKGROUND: Compounded versions of tirzepatide are widely available in the U.S. in the form of fixed‑dose combinations of tirzepatide and various analogs of vitamin B12. These combinations are mass marketed in the U.S. and other countries as comparable to FDA‑approved tirzepatide products even though they undergo no evaluation of their potency or impurity profiles. RESEARCH DESIGN AND METHODS: Samples of compounded tirzepatide combined with B12 obtained from various sources in the U.S. market were tested using various analytical methods. Samples were assessed for unacceptable levels of peptid

Using ambient AI in clinical consultations: reframing policy around clinical audit and patient safety.

Recent work has mapped what is legally permitted, commercially practiced, and technically coming for ambient AI in clinical care, yet it has … nicated clinical reasoning observable at scale, aligning consultation recording with the WHO Global Patient Safety Action Plan 2021-2030. Examining the regulatory, vendor, patient, and clinician perspectives, we argue that European law permits this purpose but does not supply it, and that consent should be reframed around patient protection rather than instituti …

Management of a Hypoglossal Nerve, Upper Airway Stimulator in the Pain Patient: Safety Considerations.

BACKGROUND: Upper airway stimulation (UAS) is an emerging neuromodulatory treatment for obstructive sleep apnea (OSA). It involves stimulating the motor fibers of the hypoglossal nerve to prevent airway collapse. Patients with OSA and pain may require radiofrequency ablation (RFA) or electrocautery during surgery. Due to the proximity of UAS leads to cervical facet joints, careful procedural considerations are necessary to prevent complications. This report summarizes best practices for performing RFA or electrocautery in patients with UAS implants, guided by manufacturer safety recommendation

Patient safety education in undergraduate medical education through a global lens: a scoping review.

BACKGROUND: Patient safety, an organizing framework to minimize risks and harm to patients in healthcare delivery, is broadly accepted as a crucial component of global undergraduate curricula. The incorporation of Patient Safety Education (PSE) into medical curricula, as suggested by the World Health Organization (WHO) can be challenging and has been partially and inconsistently applied. Factors such as densely packed curricula, gaps in the evidence-base, under-prepared faculty, and low levels of organizational support have influenced implementation. This review highlights teaching and learnin…

Osmolality (mosmol/kg H2O) versus osmolarity (mosmol/L): applied physiology to improve patient safety.

Severe osmotic pressure imbalances may be associated with substantial morbidity and mortality. As a consequence, osmolality should be a routine parameter in clinical medicine, but actual clinical practice often looks different, and confusion or inaccurate interpretations are common. This paper therefore reviews the physiological background of osmolality to achieve a better understanding and to prevent complications in hospitalized patients. The use of osmolality (mosmol/kg H2O) instead of osmolarity (mosmol/L) is recommended for clinicians to avoid potentially misleading results when comparing

Ableism and the discourse of risk and safety in patient-facing work-integrated learning.

In many countries, such as Canada, the USA, England, and Australia, to graduate from a regulated profession such as nursing, students must complete a set of work-integrated learning (WIL) hours and demonstrate their ability to safely perform physical skills and apply knowledge in relation to professional standards. For a disabled nursing student (DNS) undergoing training in higher education institutions (HEI), securing proper accommodations to participate effectively in WIL experiences has been difficult due to concerns related to risks to self and patient safety. This study used critical disc…

Safety of treating acute pulmonary embolism at home: an individual patient data meta-analysis.

BACKGROUND AND AIMS: Home treatment is considered safe in acute pulmonary embolism (PE) patients selected by a validated triage tool (e.g. simplified PE severity index score or Hestia rule), but there is uncertainty regarding the applicability in underrepresented subgroups. The aim was to evaluate the safety of home treatment by performing an individual patient-level data meta-analysis. METHODS: Ten prospective cohort studies or randomized controlled trials were identified in a systematic search, totalling 2694 PE patients treated at home (discharged within 24 h) and identified by a predefine
