Abdominopelvic tuberculosis in gynaecology: laparoscopical and new laboratory findings.

INTRODUCTION: Tuberculosis (TB) is a rare curable infective disease, caused mainly by Mycobacterium tuberculosis, which in abdominopelvic (AP) localisation, can mimic a disseminated carcinomatosis. Symptoms of AP-TB are non-specific, so diagnosis is difficult and elusive as the affected patients have normal chest X-ray and elevated levels of CA125. Female ultrasonographic features of AP-TB mimic peritoneal carcinomatosis, and the computed tomography has also been suggested to be helpful, but the final diagnosis was reached by histology and serology. AIM OF THE STUDY: To propose the validity o

Corticosteroid treatment of peritoneal tuberculosis.

Corticosteroids are beneficial as treatment of certain tuberculosis syndromes. We reviewed all cases of peritoneal tuberculosis diagnosed at our institution over 10 years to evaluate the role of corticosteroid administration combined with antituberculous therapy. Nine patients were treated with steroids plus antituberculosis agents (cases), and 26 received antituberculosis treatment only (controls). The two groups were not significantly different in terms of their basic demographics or disease. Nineteen controls compared with one case had recurrent abdominal pain. Seven controls had 17 emergen

[Abdominal TB. Peritonitis due to perforated intestinal TB & upper intestinal fistulas. Surgical treatment].

Nowadays tuberculosis--TB is most popular chronic infections disease in the world. The mortality is high, 3 million approximately death every year in the world. TB affects predominantly the lungs (pulmonary--90%) and exra-pulmonary) skin; bones; joints; urinary tract-UT; gastro intestinal tract-GIT; abdominal TB. There is an increase in diagnosed cases of pulmonary TB last years in Bulgaria. There are very rare cases of diagnosed abdominal TB described. We present a case operated from us in Multi Pro File Hospital Cherven Bryag. A 54 years old male patient with peritonitis due to perforated in

Frequency and predictive factors of granulomatous hepatitis in patients with peritoneal tuberculosis.

AIMS: The aims of our study were to determine the frequency of granulomatous hepatitis in patients with peritoneal tuberculosis, to identify factors for high risk and whether it is associated with higher frequency of antituberculous treatment side effects. METHODS: We carried out a prospective study on patients with histologically proven peritoneal tuberculosis between January 1996 and December 2005. We performed a liver biopsy in all the patients before starting the antituberculous treatment. Granulomatous hepatitis was systematically searched in all patients. RESULTS: The study was conduct

Peritoneal tuberculosis in premenopausal patients with elevated serum CA 125.

INTRODUCTION: Peritoneal tuberculosis predominantly involves the omentum, intestinal tract, liver, spleen, and genitourinary tract and occurs in 1-4% of patients with pulmonary tuberculosis. Peritoneal tuberculosis may mimic a pelvic mass in imaging studies and also may increase CA-125 levels. Peritoneal tuberculosis may also produce massive ascites, and intraperitoneal gross appearance might be similar to the peritoneal carcinomatosis. Therefore, peritoneal tuberculosis is often confused with advanced-stage epithelial carcinoma because of similar clinical, radiologic, and laboratory findings

Inflammatory choroidal neovascular membrane after healed tuberculous choroidal granuloma.

Objective: To present a case of choroidal granuloma masquerading as intraocular tumor that healed on anti-tuberculous treatment but led to the development of inflammatory choroidal neovascular membrane (CNVM). Method: A 42-year-old female patient with past history of hysterectomy presented with diminution of vision in the right eye. Fundus examination in the right eye showed a yellowish white choroidal mass with associated bullous retinal detachment superotemporal to fovea. Left eye fundus was normal. Fundus flourescein angiography showed early and late hyperflourescence with late pooling in s

[Noxytioline in the local treatment of empyema (author's transl)].

The importance assumed by local treatment in non-tuberculous purulent pleurisy has suggested that intrapleural applications of a broad spectrum antiseptic could be as useful as in peritonitis. Fifteen adult patients with empyema secondary, in most cases, to local, usually Gram-negative, infections were treated with combined antibiotic therapy and locally administered noxytioline. The patients underwent 3 to 30 intrapleural lavages (average 9) via a pleurotomy drainage tube (11 cases) or after pleural puncture (4 cases). Results were very satisfactory in 12 out of 15 cases, with mild pleural se

Prevalence and determinants of extrapulmonary involvement in patients with pulmonary tuberculosis in a Sub-Saharan African country: a cross-sectional study.

BACKGROUND: Determinants of extrapulmonary involvement during pulmonary tuberculosis (PTB) have not been extensively investigated. We assessed the prevalence and determinants of extrapulmonary involvement during PTB in a Sub-Saharan African country with a high prevalence of both TB and human immunodeficiency virus (HIV) infection. METHODS: The medical records of patients aged ≥ 15 y, admitted for a first episode of TB to the Pneumology Service of Yaoundé Jamot Hospital, Cameroon, between 2009 and 2010 were considered. Determinants of extrapulmonary involvement were investigated through logist

Imaging pearls and differential diagnosis of encapsulating peritoneal sclerosis: Emphasis on computed tomography.

Encapsulating peritoneal sclerosis (EPS) is a severe peritoneal fibrotic reaction most frequently identified as a complication of peritoneal dialysis. EPS is a complex condition whose management requires multidisciplinary input from radiologists, gastroenterologists, nephrologists, surgeons, and dietitians. EPS carries significant morbidity and mortality, primarily due to bowel obstruction that results in intestinal failure, malnutrition, and sepsis. The nondialysis causes of EPS include tuberculous peritonitis, prior abdominal surgery, beta-blocker medication use, and endometriosis. The clini

Disseminated intravascular coagulation in association with cavitary tuberculosis.

Coagulopathy due to tuberculosis is rare. We believe ours is only the second reported case of cavitary tuberculosis associated with disseminated intravascular coagulation. Our review of all 13 cases to date shows that the patients are generally black, middle-aged, male, alcoholic, and febrile. The tuberculosis is generally military, and is associated with a high mortality. Eight of the patients had associated adult respiratory distress syndrome. Only one (our case) had an acute tuberculous peritonitis. In six cases the coagulopathy began after the start of therapy; steroids did not appear to a

Tubercular abdominal cocoon in children - a single centre study in remote area of northern India.

BACKGROUND: Amongst the numerous causes of intestinal obstruction listed in the literature, sclerosing encapsulating peritonitis also called Abdominal Cocoon (AC) is one of the rarest entities. Its characteristic feature is a thick fibrotic membrane encasing varying lengths of the small and large gut in a cocoon. In India, there is an increasing incidence of tuberculosis, especially in the rural areas. AIMS AND OBJECTIVES: The aim of this study was to investigate the clinical presentation and evaluate the operative findings of tuberculous AC. We also evaluated the outcomes and response to ant

Peritoneal tuberculosis versus peritoneal carcinomatosis: distinction based on CT findings.

OBJECTIVE: The purpose of this study was to determine the utility of CT in distinguishing peritoneal tuberculosis (PT) from peritoneal carcinomatosis (PC). MATERIALS AND METHODS: CT scans were retrospectively reviewed in 19 patients known to have PT and compared with scans in 19 patients known to have PC. CT images were evaluated for thickening (smooth versus irregular), enhancement, presence of nodules, and site of involvement on the parietal peritoneum. The existence of omental caking, nodules, and smudged patterns in the omentum, mesentery, and gastrocolic ligament was noted. The presence,

Lupus enteritis and peritonitis presenting as initial manifestations of systemic lupus erythematosus: diagnostic challenge in tuberculosis-endemic regions - case report.

INTRODUCTION AND IMPORTANCE: Systemic lupus erythematosus (SLE) is a chronic multisystem autoimmune disease with heterogeneous manifestations. Gastrointestinal involvement, such as lupus enteritis, is rare and often presents with nonspecific abdominal symptoms and is associated with significant morbidity and mortality. Peritoneal involvement with ascites as an initial manifestation is also among the rarest presentations of SLE. In tuberculosis (TB)-endemic regions, these presentations may mimic abdominal TB, complicating timely diagnosis. CASE PRESENTATION: A 23-year-old male presented with w

Video-assisted laparoscopic procedures in peritoneal dialysis.

BACKGROUND: Although laparoscopy is commonly adopted for the diagnosis and management of various medical or surgical problems, its use for patients with peritoneal dialysis has seldom been addressed. This retrospective study analyzes the indications and clinical effects of this procedure. METHODS: A retrospective chart was drawn up and a videotape review performed for 18 laparoscopic procedures involving 198 patients receiving peritoneal dialysis on a long-term basis at our dialysis unit from May 1992 to June 2002. The clinical and demographic parameters in this study included gender, age, un
