OBJECTIVES: To describe etiology and clinical characteristics of all patients diagnosed with axillary-subclavian vein thrombosis (ASVT) in our center.
METHODS: Retrospective study during last 4 years (January 1991 to December 1994). Only those patients with a typical phlebography were included.
RESULTS: 42 patients were diagnosed with ASVT. 10 in 1991, 6 in 1992, 10 in 1993 and 16 in 1994. The average age was 56.3 years. About sex, 20 were in men and 22 in women. The right upper extremity was affected in 16 patients, left side was in 24 and 2 were bilateral. Respect the incriminating agent, …

To investigate the possible anatomic and hemodynamic reasons for the variability in response to surgery for venous insufficiency (a 50% ulcer recurrence rate following ligation of incompetent perforating veins), we performed phlebography and venous pressure measurements in 109 legs of 77 patients and in 30 healthy volunteers. Patients were divided into five groups: saphenofemoral incompetence alone (group 1), saphenopopliteal incompetence (group 2), incompetent lower leg communicating veins alone (group 3), calf communicating veins and saphenous incompetence (group 4), and postthrombotic limbs…

In this article, we discuss the approach for diagnosing deep vein thrombosis (DVT) in different patient populations. Clinical features and probability assessment guide further diagnostic tests. D-dimer testing is used as screening test; however, duplex ultrasound remains the primary confirmatory test. Computed tomography and magnetic resonance imaging are used only in select patient populations, such as when ultrasound results are equivocal, in patients suspected of central venous DVT, or as a part of combined protocol for diagnosis of pulmonary embolism. Contrast phlebography and plethysmogra…

The antegrade scrotal sclerotherapy is a minimally invasive surgical technique in the treatment of the varicocele. Through a small scrotal incision a straight vein merging in the spermatic vein is isolated and cannulated. An antegrade phlebography is performed and ethoxysclerol is injected in the vein. We have performed this technique in 35 patients with a follow-up of at least 3 months. All patients except one (97%) show no more varicocele. This technique has a low morbidity, is cheap and as effective as the other treatment modalities. The antegrade sclerotherapy is competitive to the retrogr…

We report the case of a 73 year old man presenting with severe hypoxaemia due to an anatomical right to left shunt from a persistent left superior vena cava (VCSG) anastomosing with the left auricle. This was fortuitously discovered while placing a left sub clavicular catheter; the diagnosis of the VCSG draining into the left auricle was confirmed by superior phlebography and CT scanning. The discovery of this shunt after a pulmonary embolus suggested to us that the rise in venous pressure secondary to the embolus led to both clinical symptoms and hypoxaemia from the shunt, which had until the…

In 20 patients with a clinical picture of primary hyperaldosteronism (5 patients were later found to be suffering from hypertension of other origin), CT showed diagnostic precision higher than 90% in the localization of adrenal adenoma (11 out of 12). On the other hand, this examination scarcely helped in the diagnosis of uncommon gland hyperplasias (1 out of 3). In CT doubtful or negative cases, hormonal measurement in blood samples taken selectively from adrenal veins was more indicative than selective phlebography. CT can therefore be considered as an alternative to iodocholesterol scintigr…

The authors report 4 cases of sciatica from disk herniation occurring in children under 16. It happens rarely in this age range, as is shown in the review of the literature performed for this article. The symptomatology is similar to that in adults. The physical signs are much more prominent than the functional signs. An original traumatism, sometimes severe, is often encountered. The standard X-ray examination is of little use in diagnosis, while contrast studies (opaque myelography, phlebography) clearly show the lesions of compression. Surgical treatment is frequent but not compulsory, foll…

The authors present a series of 18 cases (13 patients) of a venous syndrome of the arm. Three were cases of phlebitis due to effort and 15 were cases of intermittent or permanent oedema of the arms. Four of the patient also presented homo- or contralateral arterial disorders. Dynamics and multiposition phlebography gave evidence on 18 occasions of a branchial compression syndrome. Twelve patients were operated upon. On 16 occasions resection of the first rib was carried out by the axillary route (in 1 case this was combined with the ablation of one cervical rib). All the patients were cured wi…

A 4-year-old girl was studied at the Pediatrics Unit at the Hospital General de México, S.S.A., with a diagnosis of abdominal tumor, probably malignant. After several laboratory tests (blood count, blood chemistry, catecholamines and the following X-ray studies: chest, skull, long bones, Cava's phlebography and excretory urography), she was submitted to surgery without a diagnosis. After this, the final diagnosis was established: mesenteric panniculitis (mesenteric lipodystrophy. This case is published because of its rare occurrence at this age and because it is important to be kept in mind in…

Nutcracker syndrome is a complex of clinical symptoms due to compression of the left renal vein between abdominal aorta and superior mesenteric artery. Hematuria and pelvic or back pain are the most common presenting symptoms with varicocele being an uncommon clinical finding in these patients especially in children. Doppler ultrasound, computed tomography and selective left renal vein phlebography are useful to confirm the diagnosis. Treatment can be conservative or surgical depending on the severity of symptoms. Here, we report a 13-year-old boy with left flank pain and varicocele on examina…

An absent right superior vena cava associated with a persistent left superior vena cava (PLSVC) is a rare and generally asymptomatic congenital malformation. It is usually discovered incidentally during a pacemaker (PM) implantation. We report here a successful PM implantation using a left subclavian approach through the coronari sinus via a PLSVC, with a 2-year follow-up showing a still appropriate function of the device. We discuss the clinical usefulness of peripheral phlebography in the operating room for delineating precisely the venous anatomy, as well as the role of shaped stylets and a…

CASE REPORT: A 65-year-old patient with polycythemia vera (PV) was admitted with a painful edema of the right arm lasting for 24 h. The D-dimer assay was negative. By phlebography the patient was diagnosed with a fresh thrombosis of the right subclavian vein. 1 week later she developed a D-dimer-negative symptomatic pulmonary embolism.
CONCLUSION: If clinical signs of a thromboembolic event are present, the negative predictive value of the D-dimer assay is not sufficient to abandon further definitive diagnosis. Since thromboembolic events are frequent in PV patients, a prophylactic treatment …

The authors have studied two cases of progressive diaphyseal dysplasia (Camurati Engelmann's disease). In one case their investigations included bone marrow pressure tests, phlebography and blood gas analysis of the medullary blood. The BMP was raised, and the phlebogram showed venous stasis. The blood gas analysis revealed reduced oxygenation of the medullary blood. On the hypothesis that the vascular change could be of pathogenetic significance, the authors proceeded in both cases to open the medullary canal. This produced immediate remission of the pain, which still persists in one case aft…

The pathology of the cervico-thoracic channel is now well understood. Intermittent venous compression in the costo-clavicular space by the subclavian muscle can lead to acute occlusion of the venous trunk. It is important to make an early diagnosis of such compression before the stage of occlusive phlebitis. Clinical examination and dynamic phlebography allow the diagnosis to be made. Resection of the first rib is the ideal treatment. The mode of approach to the first rib is debatable since it seems that when venous symptomatology is dominant, the axillary method of Roos is not the best approa…

Besides anticoagulants and thrombolytic drugs, surgery should be included among the therapeutical means to prevent deep phlebitis of the lower limbs. Early phlebography, thoroughly innocuous, is necessary to any coherent therapy. The prevention of pulmonary embolism which can be done by a ligature of the inferior vena cava or by the fitting of an umbrella-shaped filtre, is performed most of the time with a clamp of Adams and de Weese. Its reliability is satisfactory as the percentage of embolic recurrences does not exceed 5%. Early venous thrombectomy prevents in 60 to 80% of cases the post-ph…

Using 13 cases of Budd-Chiari syndrome, the authors describe their method of radiological investigation in order to reach a positive and sometimes aetiological diagnosis. Hepatic venous thrombosis can be confirmed only on the basis of findings at hepatic phlebography. Cavography, which precedes retrograde catheterisation of the hepatic veins, provides only presumptive evidence and sometimes reveals total thrombosis of the inferior vena cava. Coeliac and superior mesenteric, or even hyperselective hepatic arteriography, is of great value if there is inversion of the portal flow. Otherwise it on…

Iliofemoral venous thrombosis is discussed and a technique of iliofemoral venous thrombectomy is presented. Operative phlebography is recommended. The personal recommendations of leading American vascular surgeons in treating the patient with acute iliofemoral venous thrombosis are presented. I recommend thrombectomy for phlegmasia cerulea dolens, and in previously healthy, young ambulatory patients with phlegmasia alba dolens who are seen within 48 hours following thrombosis and have failed to show clinical improvement after a trial of bed rest, elevation of the lower extremities, and intrave…
