A boy with congenital atrichia, ichthyosis follicular, keratitis, cutaneous infections and a huge inguinal hernia, but without deafness is reported. We believe it represents a new case of a rare X-linked recessive syndrome known as ichthyosis follicularis, alopecia, photophobia syndrome (IFAP). The differential diagnosis from keratitis ichthyosis deafness is discussed. The cutaneous infections seen in our case suggest the possibility of considering a genetic link between these syndromes.

Two patients, a 48-year-old woman and a 56-year-old man, had hypocomplementemic cutaneous vasculitis syndrome. The patients had ocular pain and photophobia and were found to have bilateral recurrent iridocyclitis. We treated the iridocyclitis with cycloplegia and topical corticosteroids, which relieved their symptoms and diminished the clinical signs. The bilateral iridocyclitis found in both patients may have been secondary to immune complex deposition in the eye.

Nivolumab and ipilimumab are immune checkpoint inhibitors widely used for the treatment of metastatic melanoma. They can cause various adverse effects in a significant percentage of treated patients, including uveitis. Symptoms include decreased visual acuity, pain, redness, and photophobia, which typically resolve with appropriate anti-inflammatory therapy. The aim of this work is to report a case of uveitis in a patient on ipilimumab and nivolumab for the treatment of metastatic melanoma, as well as the response to therapy with difluprednate.

Albinism is not a single entity but represents a heterogenous group of inherited disorders of pigmentation. Despite a wide variety of manifestations, all forms of albinism are characterized by several ocular features, including: nystagmus, photophobia, reduced visual acuity and a lack of stereopsis. It is the intention of this review to describe and discuss the clinical implications of albinism with particular emphasis placed on these ocular features and their effects on visual performance. Various techniques that may be employed to aid the albino will also be described.

Accidental ocular exposure to doxorubicin was followed by no reaction or rapidly resolving conjunctivitis in 13 of 15 cases (87%). In the two remaining cases, persistent photophobia and chronic inflammation were reported. Of 28 accidental exposures to sites other than the eyes, no reactions or rapidly resolving local reactions were reported in 24 cases (86%). Nurses are at particular risk for accidental exposure to doxorubicin and accounted for 20 of the 43 reported exposures (47%).

Idiopathic intracranial hypertension (IIH) is typically described in a middle-aged female and present with symptoms of headache, transient visual obscuration, photophobia, diplopia, rarely with pulsatile tinnitus. Seizure as a sole manifestation is very rare. Here we present a case of a young non-obese female with epilepsy, and imaging revealed temporal lobe encephalocele with typical imaging features of IIH. She underwent further workup, which confirmed the diagnosis of idiopathic intracranial hypertension.

Traumatic aniridia from combat ocular trauma can cause visual disability. A 41-year-old male Army Veteran was referred for evaluation of light sensitivity and glare secondary to subtotal traumatic aniridia of his left eye from an improvised explosive device blast. A custom-made artificial iris prosthesis was implanted in the ciliary sulcus and secured using Gore-Tex sutures. After surgery, the patient reported improvement of his light sensitivity and quality of life. We report a boy, born to healthy first cousin parents, with diffuse hyperpigmentation of the skin and guttate hypomelanotic lesions, photophobia, abnormal hair, developmental delay, and recurrent bronchitis. Skin histology showed pigmentation incontinence with numerous melanophages. Electron microscopy showed a very high number of melanosomes and some degenerating keratinocytes. These features correspond to a rare genodermatosis, the X-linked reticulate pigmentary disorder with systemic manifestations. Skewed X-inactivation patterns were detected in the mother's lymphocytes.

Many common ophthalmologic conditions may present as a red eye. Most are not vision-threatening and can be easily treated. Vision-threatening symptoms and signs include sudden diminution in visual acuity, ocular pain, photophobia and the presence of circumcorneal injection (the so-called cilary flush) on examination. Corneal, uveal tract and anterior chamber problems often require ophthalmologic consultation, but these conditions are less common than blepharitis, lid problems, dry eyes and conjunctivitis.

We examined two women who had keratic precipitates, minimal intracameral cells and flare, diffuse iris stromal atrophy, posterior subcapsular cataract, and vitreous opacities in their left eyes. No ocular pain, photophobia, or posterior synechia was noted. Their right irises appeared brown. Heterochromia was evident in both patients. Recurrent subconjunctival hemorrhages were also in their affected left eyes. It is possible that subconjunctival hemorrhages in our patients may be associated with Fuchs' heterochromic iridocyclitis.

A 12 year old girl with mitochondrial myopathy, encephalopathy, lactic acidosis and stroke like episodes (MELAS) is reported. After a normal childhood, at 9 years of age she developed generalized and hemilateralized seizures. Posteriorly, these episodes became more frequent and were accompanied by headache, homonimous hemianopsia, ataxia, vomiting, photophobia, left hemiparesis, slurred speech and even convulsive status. Laboratory tests evidenced lactic acidosis, brain lucencies at CT Scan and ragged skeletal muscle fibers at muscle biopsy.

Sporadic hemiplegic migraine (SHM) is a rare form of migraine. I report a 23-year-old Nigerian lady with SHM. She had recurrent attacks characterized by visual aura that progressed to headache, dysphasia, and hemiplegia. Her motor symptom was always followed by development of nausea and photophobia lasting several hours. Computerized tomography of the brain and electroencephalography were normal. She was treated with carbamazepine, aspirin, and codeine. Differential diagnoses of SHM were highlighted.
