Acute physiological and perceptual responses in the FIT FIRST FOR ALL school-based physical activity program.

This study examined acute physiological and perceptual responses to the FIT FIRST FOR ALL program, a structured school-based physical activity intervention designed to promote engagement. In a non-randomized cluster study conducted in the Faroe Islands, 179 pupils aged 7-16 participated in three weekly 40-minute sessions for 10 weeks as part of a school-wide program alongside physical education. Acute responses were assessed in a subsample (n = 53) using heart rate, accelerations, perceived exertion, and enjoyment, with individualized thresholds and stratification based on baseline cardiorespi

[Eccentric muscle work--the unknown side of our movements].

Eccentric muscle work is an important part of our everyday activity, i.e. in breaking and preventing falls. Eccentric phases of movements improve the efficiency of cyclic repetitive activities. The molecular function in eccentric work is not understood, however its characteristics are a low metabolic demand and a high power output. Compared to concentric work there are only small differences in the hemodynamics of eccentric work. The muscle capacity for mechanical load is the limiting factor in eccentric work. Unaccustomed eccentric activity leads to damage of the cytoskeleton, which is the re

Limitations to exercise capacity and endurance: pulmonary system.

The response of the pulmonary system to exercise is well-documented to be quite precise in its homeostatic regulation, highly efficient in terms of its physiologic cost, and operating well within its maximum reserves. Two exceptions to these generalizations are documented here in the highly-trained athlete: 1) the arterial hypoxemia observed in short-term work at a VO2 greater than 4-5 l . min-1, and 2) the tachypneic hyperventilation of long-term, heavy exercise in varying environmental conditions. The potential causes and consequences of these responses are discussed with reference to so-cal

Physical activity in the Fontan population: provider recommendations and patient actions.

BACKGROUND: Emerging evidence suggests that routine physical activity may improve exercise capacity, long-term outcomes, and quality of life in individuals with Fontan circulation. Despite this, it is unclear how active these individuals are and what guidance they receive from medical providers regarding physical activity. The aim of this study was to survey Fontan patients on personal physical activity behaviours and their cardiologist-directed physical activity recommendations to set a baseline for future targeted efforts to improve this. METHODS: An electronic survey assessing physical act

[Exercise-induced laryngeal obstruction : diagnosis and management].

Exercise-induced laryngeal obstruction (EILO) is a condition characterized by temporary narrowing or closure of the larynx during physical activity, in particular during intense physical exertion. It generally affects teenagers and young adults and is often misdiagnosed as exercise-induced asthma. Symptoms include dyspnea, wheezing and a feeling of throat tightness during exercise. The aim of this article is to review this often underrecognized condition, its diagnosis and its management.

A 78-Year-Old Woman With Dyspnea on Exertion, Hypotension, and Telangiectasias.

A 78-year-old woman with gastroesophageal reflux disease was admitted with 2 months of progressive dyspnea on exertion and leg swelling. At baseline, she could walk 10 blocks; at presentation, she managed less than 6 steps. For a month, she had been sleeping upright in a chair. She experienced no fever, cough, palpitations, or chest pain. With prompting, she shared a several-year history of discomfort and skin changes in her fingers; she attributed these symptoms to chemical exposures as a housecleaner.

Exertional Heat Illness in Active Populations.

AbstractExertional heat illness (EHI) is a leading yet preventable cause of morbidity and mortality in active populations. EHI develops when the body's integrated thermoregulatory responses, including cardiovascular adjustments and evaporative cooling, are unable to dissipate the combined burden of metabolic heat production, environmental heat stress, clothing or protective equipment, and other contributing factors. This review examines the physiological mechanisms that underlie EHI and the effects of heat stress on the exercising heart. We also summarize best practices for the prevention, rec

Brain, Metabolic, and RPE Responses during a Free-Pace Marathon: A Preliminary Study.

The concept of the "central governor" in exercise physiology suggests the brain plays a key role in regulating exercise performance by continuously monitoring physiological and psychological factors. In this case report, we monitored, for the first time, a marathon runner using a metabolic portable system and an EEG wireless device during an entire marathon to understand the influence of brain activity on performance, particularly the phenomenon known as "hitting the wall". The results showed significant early modification in brain activity between the 10th and 15th kilometers, while the RPE r

Taking a walk on the heart failure side: comparison of metabolic variables during walking and maximal exertion.

AIMS: Although cardiopulmonary exercise testing (CPET) is the gold standard to assess exercise capacity, simpler tests (i.e., 6-min walk test, 6MWT) are also commonly used. The aim of this study was to evaluate the relationship between cardiorespiratory parameters during CPET and 6MWT in a large, multicentre, heterogeneous population. METHODS: We included athletes, healthy subjects, and heart failure (HF) patients of different severity, including left ventricular assist device (LVAD) carriers, who underwent both CPET and 6MWT with oxygen consumption measurement. RESULTS: We enrolled 186 subj

The intensity of a resistance exercise session can be quantified by the work rate of exercise.

PURPOSE: Athletes regularly perform resistance training, yet it is unknown how best to monitor its intensity. This study compared different resistance exercise intensity metrics to determine their sensitivity to manipulating work rate (via altering inter-set rest and load). METHODS: Following baseline testing for 10- and 3-repetition maximum (RM; squat and bench press), fourteen trained participants completed four volume-matched protocols in a randomised order: 3x10 with 85% 10RM, 60 s rest (3x1060s); 3x10 with 85% 10RM, 180 s (3x10180s); 8x3 with 85% 3RM, 120 s (8x3120s); 8x3 with 85% 3RM, 3

Acute Neuromuscular Fatigability and Perceived Exertion in High-Level University Pianists: An Electromyography and Maximum Force Capacity Study.

PURPOSE: Musicians are labelled 'fine motor athletes' due to the significant demands of practice and performance. In response, sports medical concepts and expertise have been adapted and mobilized to address musicians' health considerations. However, understanding distinct differences between the sports and performing arts medical contexts is critical. Mismanagement of fatigue and acute fatigability are identified as key risk factors for deleterious symptoms and injuries in musicians. Acute fatigability in athletes and musicians have been assumed to occur similarly, although this has yet to be

Exercise Intensity and Pacing Strategy of Cross-country Skiers during a 10 km Skating Simulated Race.

The aim of this study was to observe the cardiovascular demands by monitoring the heart rate (HR) and the pacing strategy by evaluating the speed during a simulated cross-country skiing race. Eleven skiers (16.45 ± 1.67 yrs; 21.32 ± 2.03 kg m(-2); 72.35 ± 4.69 mL min(-1) kg(-1)) competed in a 10 km event divided into four laps. The HR profile was classified into four intensity zones. The skiers spent 66.84 ± 23.64% and 31.82 ± 23.77% of the total time above 90% and at 80-90% of the HRmax respectively, whereas the lower zones were negligible. During the event, a progressive increase in intensit

How to exhaust your bone marrow.

A 32-year-old man was admitted to the hospital because of oedema and 8 kg of gained weight. The oedema decreased spontaneously over weeks and there was no evidence for a nephrotic syndrome; however, the blood tests revealed a moderate pancytopenia. The patient practiced excessive physical activity at work and in his spare time, and kept a very thorough training and weight diary. Owing to a high intake of energy and protein drinks he tried to optimise his physical performance and kept a normal body mass index  at 23.7. A bone marrow biopsy showed gelatinous bone marrow transformation, normally

[Exertion-related abnormalities in the urine].

Strenuous exercise can lead to the occurrence of various abnormalities in the urine. Hematuria and proteinuria are those most frequently described. Other less common possibilities are pigmenturia like hemoglobinuria and myoglobinuria. These urine abnormalities are found not only in sportsmen/women or soldiers, but also in percussionists. Other possible causes are hereditary metabolic myopathy and the use of drugs or medication. Exertion-related urine abnormalities occur frequently and are usually benign. Any abnormalities should disappear after a period of 24-72 h of rest. Exertional rhabdomyo

[Factors limiting exertion in respiratory diseases].

Factors limiting exercise capacity in patients with respiratory disease are determined in part by their response to an exercise test. Exercise tests evaluate the overall oxygen transport chain, i.e. exercise tolerance. The basic parameter is maximal oxygen consumption, VO2max which is very low in respiratory diseases. Decreased exercise tolerance results first from limited ventilatory capacity at maximal exercise: ventilatory reserve in these patients is low. At submaximal exercise, ventilation is on the contrary increased for a given exercise load. This relative hyperventilation is the conseq

Exertional collapse and sudden death associated with sickle cell trait.

Although rare, exertional collapse and sudden death are the most serious potential complications of sickle cell trait. Studies suggest that this condition may occur in susceptible persons when poor physical conditioning, dehydration, heat stress or hypoxic states precipitate sickling of the abnormal erythrocytes. Sickling leads to endothelial damage, which can cause vasoconstriction, disseminated intravascular coagulation and local tissue damage. Cardiac effects include acute ischemia and arrhythmias. Muscle damage results in acute compartment syndromes and release of myoglobin into the circul
