Robotic Mediastinal Surgery in Patients with Suspected Thymic Neoplasms: First Israeli Experience.

BACKGROUND: Thymectomy is a reliable surgical method for treating patients with myasthenia gravis (MG) and benign tumors of the thymus. Despite the advantages of minimally invasive surgical approaches for resection of thymic neoplasms, there are still controversies regarding the superiority of one type of surgery over another. OBJECTIVES: To report the results of our initial Israeli experience with robotic thymectomy in 22 patients with MG and suspected benign thymic tumors. METHODS: We retrospectively analyzed 22 patients (10 men, 12 women) who underwent robotic thymectomy by a left-sided (

Mesothelioma and asbestos-related pleural diseases.

At present, the use of asbestos is not regulated at a worldwide scale. Moreover, there is a latency period between asbestos exposure and the manifestations of asbestos-related diseases. Consequently, pulmonologists are still dealing with consequences of asbestos exposure, which mainly occur at the pleural surface. The aim of this review is to provide an overview of asbestos-related pleural diseases. We summarized the most relevant data for the diagnosis and the management of benign asbestos pleural effusions, pleural plaques, diffuse pleural thickening and rounded atelectasis. Special attentio

The cytological features of effusions with mesothelioma in situ: A report of 9 cases.

INTRODUCTION: The diagnosis of mesothelioma in situ (MIS) is now accepted by the WHO as a pre-invasive neoplastic mesothelial proliferation and considered a diagnosis based on histologic evaluation only. Although the definition of MIS includes recurrent effusions, little is known about the cytologic features of such effusions. Since mesothelioma is usually diagnosed at an advanced stage and has a poor prognosis, early detection of a neoplastic mesothelial population in such effusions can potentially have a positive impact on the management of such a dire disease. MATERIALS AND METHODS: We rev

Adult NTRK-rearranged spindle cell neoplasms of the viscera: with an emphasis on rare locations and heterologous elements.

NTRK-rearranged mesenchymal neoplasms mostly affect the soft tissues of pediatric patients. Given the responsiveness to selective NTRK inhibitors, it remains critical to identify those ultra-rare cases occurring in the viscera of adults. In five females and two males aged 18-53 years, we characterized visceral mesenchymal tumors harboring TPM3-NTRK1 [uterine cervix (N = 2), pleura, prostate], LMNA-NTRK1 (lung), SQSTM1-NTRK3 (heart), and NTRK3 rearrangement with unknown fusion partner (colon/mesocolon) with RNA sequencing, FISH, RT-PCR, and immunohistochemistry. The tumors exhibited spindled to

What every surgeon should know regarding asbestos-related disease.

Long-lasting occupational exposure to asbestos dust may cause pleural disease, asbestosis, bronchogenic carcinoma and mesothelioma. Many years elapse from first exposure to the appearance of symptoms. Almost all these diseases are the result of dusty working conditions 20-40 years ago. The pathological sequelae are usually irreversible and can progress after exposure ceases. There is no satisfactory treatment. Prevention is the only practical way. In spite of the fact that the general public is invariably exposed to small amounts of the material, asbestos is not a public health problem. Exempt

Adrenal solitary fibrous tumor: A case report.

Solitary fibrous tumor (SFT) is a rare spindle cell neoplasm, originally described in the pleural cavity. It may also occur in extrapleural organs like adrenal gland. We present a case of a fifty two-year old Arab man who has been admitted in urology department for right lumbar pain. A suspect malignant adrenal mass has been detected. He underwent right adrenalectomy and histologically it was SFT. The follow-up three years after surgery is unremarkable. The prognosis of SFT after surgery is unpredictable. A long term follow-up is mandatory to detect local recurrence or distant metastasis.

Synchronous lung adenocarcinoma in situ and type B2-B3 thymoma in a frozen hemithorax: a case report and multidisciplinary approach.

BACKGROUND: Synchronous primary lung cancer and mediastinal thymoma represent a rare clinical entity. While 18F-FDG PET/CT serves as a cornerstone modality for differentiating benign from malignant thoracic neoplasms, hypermetabolic activity secondary to severe localized inflammation can frequently masquerade as highly aggressive malignancies. Such radiologic-pathologic discordance significantly elevates the risk of misdiagnosis, thereby adding substantial complexity to clinical decision-making and patient management. CASE PRESENTATION: A 73-year-old woman with prior tuberculosis and stented

Primary synovial sarcomas of the mediastinum: a clinicopathologic, immunohistochemical, and ultrastructural study of 15 cases.

A series of 15 cases of primary mediastinal neoplasms displaying histopathologic, immunohistochemical, and ultrastructural features of synovial sarcoma is presented. The patients' ages ranged from 3 to 83 years, with a male-to-female ratio of 2:1. Nine cases presented as anterior mediastinal masses with chest pain, shortness of breath, and pleural effusion, and 6 cases were in paravertebral location in the posterior mediastinum and presented with neck or back pain and pleural effusion. The tumors measured from 5 to 20 cm in greatest diameter and showed a tan white, soft to rubbery cut surface

Cytologic features and diagnostic accuracy of analysis of effusions for detection of ovarian carcinoma in dogs.

BACKGROUND: Presence of an abdominal effusion is a typical presenting sign associated with ovarian carcinoma (OC) in dogs. OBJECTIVES: The aims of this study were to describe the cytologic features of effusions associated with OC and to evaluate the diagnostic accuracy of such features for the diagnosis of OC in dogs. METHODS: Cytologic evaluations of 7 OC-associated peritoneal effusions in dogs were used to define cytomorphologic features of this neoplasm. Then, in a blinded study to evaluate the accuracy of these features in identifying OC, 2 independent board-certified clinical pathologis

Hyperthermic pleural perfusion with cisplatin: early clinical experience.

BACKGROUND: The preferred treatment for patients with pleural tumors or tumors with pleural extension has not been determined. Systemic and local modalities were studied, including intrapleural chemotherapy and hyperthermia, which mainly have been investigated separately. The objective of this study was to investigate the feasibility, toxicity and early results of a multimodality treatment consisting of surgery, chemotherapy, and hyperthermia in one session for the treatment of patients with pleural malignancies. METHODS: From November 1994 to September 1998, 26 patients (7 patients with meso

A case of advanced intrahepatic cholangiocarcinoma successfully treated with chemosensitivity test-guided systemic chemotherapy.

Intrahepatic cholangiocarcinoma (ICC) is a relatively rare and highly fatal neoplasm that arises from the biliary epithelium. Prognosis is generally poor and survival is limited to a few months. Here we present a case of advanced ICC successfully treated by chemosensitivity test-guided systemic chemotherapy combining S-1 and cisplatin (CDDP). A 65-year-old woman with a liver tumor was referred to our hospital on November 21, 2007. Abdominal ultrasonography and computed tomography (CT) showed low-density masses of 50 and 15 mm in diameter, respectively in segment VIII of the liver and in the en

"Omental Cake" in a Patient With Asbestosis Leading to the Diagnosis of Malignant Peritoneal Mesothelioma.

We report a case of a 68-year-old man with medical history of pleural asbestosis and diagnosed with malignant peritoneal mesothelioma. This neoplasm is rare, has a poor prognosis, and is associated with asbestosis in many cases. It manifests clinically insidiously and in relation to the intra-abdominal locoregional effect. Radiological findings are variable, although the finding of "omental cake" by CT scan is characteristic but not pathognomonic, as seen in our case. A biopsy is required for the diagnosis, which can be guided by radiology or surgery. Treatment options available are cytoreduct

Mesothelioma Biomarkers: Discovery in Search of Validation.

Malignant pleural mesothelioma (MPM) is an asbestos-related neoplasm that can only be treated successfully when correctly diagnosed and treated early. The asbestos-exposed population is a high-risk group that could benefit from sensitive and specific blood- or tissue-based biomarkers. We review recent work with biomarker development in MPM and literature of the last 20 years on the most promising blood- and tissue-based biomarkers. Proteomic, genomic, and epigenomic platforms are covered. SMRP is the only validated blood-based biomarker with diagnostic, monitoring and prognostic value. To stre

Cardiac cavernous hemangioma and multiple pulmonary cavernous hemangiomas.

We describe for the first time a rare coexistence of a cardiac cavernous hemangioma with multiple pulmonary cavernous hemangiomas. Computed tomography revealed bilateral pulmonary nodules, left pleural effusion, and pericardial effusion. Positron emission tomography showed a pericardial neoplasm. Pathologically, multiple large dilated vascular spaces, lined by a single layer of endothelial cells and filled with blood, were revealed in both the cardiac tumor and the pulmonary nodules. Immunohistochemical examination of the lining cells showed positivity for CD31, FLI1, FVIII, and CD34. Taken to

Askin's tumour in pregnancy with dismal prognosis.

Askin's tumour is a rare malignant neoplasm of the chest wall. A 23-year-old primigravida patient was admitted to the hospital with complaints of chest wall swelling, cough and dyspnoea. Radiological examination showed a chest wall mass in right hemithorax with erosion of ribs and moderate pleural effusion. Fine-needle aspiration cytology was suggestive of malignant small round cell tumour. Immunohistochemical analysis together with the clinical and radiological findings led to the definitive diagnosis of Askin's tumour. Despite receiving multimodality therapy, she had residual soft tissue mas

Surgical resection of esthesioneuroblastoma metastasis to the chest wall.

A 51-year-old female patient was evaluated for a painful chest wall mass causing atelectasis of the right lung, pleural effusion, and dyspnea. The patient's history was significant for esthesioneuroblastoma at the age of 24; multiple recurrences of the tumor had been treated with surgery, radiotherapy, and chemotherapy. Surgical resection of the chest wall mass relieved her symptoms and improved her quality of life. Histologic examination confirmed metastatic esthesioneuroblastoma. The patient developed generalized disease and finally died 2 years after surgery. This case demonstrates the long

Canine intrathoracic sarcoma with ultrastructural characteristics of human synovial sarcoma - case report.

BACKGROUND: Canine joint sarcomas, designated synovial sarcomas, are uncommon malignant mesenchymal neoplasms that occur in the large joints of the extremities of middle-aged, large-breed dogs. We report the diagnosis of an intrathoracic sarcoma with ultrastructural characteristics reminiscent of human synovial sarcoma in a dog. CASE PRESENTATION: A 7-year-old female spayed Tibetan terrier crossbred dog was presented for acute severe labored breathing and diagnosed with an intrathoracic neoplastic mass. The neoplasm resulted in the accumulation of substantial amounts of viscous pleural fluid
