Who's really in the loop? Rethinking oversight in AI-assisted health care.

Human-in-the-loop oversight is widely invoked as a safeguard against potential harm from artificial intelligence (AI) used in health care, yet it functions more as symbolic reassurance than substantive protection. We argue that human-in-the-loop fails for three interconnected reasons: AI used in health care can amplify existing structural inequities at unprecedented scale, intersectional harms elude detection by oversight models premised on neutral singular reviewers, and clinicians operate under constraints that preclude meaningful interrogation of algorithmic outputs. Drawing on actor-networ

Healthcare Resilience Should Include the Worker-Patient Safety Loop: A Taiwan Psychiatric Hospital Perspective.

Research before and during COVID-19 has framed health-system resilience as the capacity to absorb shocks, adapt, transform and learn while sustaining essential functions. Building on this broader literature, including Meredith et al.'s account of local public-health adaptation under political and resource constraints, we propose a worker-patient safety loop from a Taiwan psychiatric hospital perspective: workforce strain can weaken safety-critical care; near misses, adverse events and continuity failures should trigger reporting, organisational learning and corrective action; and the resulting

From exposure to embedding: how interconnected social determinants drive health inequalities-a complex adaptive system dynamics approach.

The social determinants of health (SDOH)-including socioeconomic position, education, housing, and social networks-are fundamental drivers of health inequalities worldwide. These inequalities persist because social determinants interact dynamically across time and context, with health emerging from the interplay of social, political, environmental, and biological processes in which determinants act simultaneously as causes and consequences. Early-life exposures and historical conditions produce enduring physiological vulnerability through biological embedding, encompassing stress dysregulation

Strengthening hepatitis B virus clinical research in Africa: the need for multistakeholder collaboration, funding, and political will.

Africa bears a disproportionate burden of hepatitis B virus (HBV) infection, with 68 million chronically infected individuals and 65% of global new infections. However, only 18 (1%) of 1804 global HBV clinical trials have been conducted in Africa, limiting the generalisability of therapeutic findings. The HBV arm of the Forum for Collaborative Research convened a series of multistakeholder discussions between 2023 and 2024, to identify gaps in clinical research and identify solutions. This Personal View synthesises insights from these convenings, highlighting key barriers and potential solutio

The myth of pandemic preparedness: persistent gaps in global readiness for Disease X.

Recent outbreaks of H5N1 avian influenza have exposed persistent weaknesses in global pandemic preparedness. Despite scientific advances, surveillance remains fragmented, hospital surge capacity is declining, and frontline workers continue to face inadequate protection. In low-income settings, delayed outbreak detection - exemplified by cholera in Sudan and mpox in central Africa - reflects ongoing gaps in diagnostic access and communication infrastructure. These challenges are compounded by geopolitical instability, fragile supply chains, and a lack of transparent, sustained investment in pre

Food insecurity, xenophobia, and political legitimacy: exploring the links in post-COVID-19 South Africa.

Food insecurity in South Africa was critical prior to the COVID-19 outbreak, but the problem deepened quickly during the pandemic when government controls caused job losses, a food supply collapse, and escalating hunger. The food and fuel price hikes and political instability that followed led to the July 2021 'unrest', which left more than 350 people dead. Behind this lay a crisis within the governing African National Congress. In this paper, we draw on in-depth interviews and ethnography with individuals working in food-based livelihoods to investigate how people continued to secure food, an

Trends and disparities in non-communicable diseases in the Western Pacific region.

The WHO Western Pacific region bears disproportionate deaths from non-communicable diseases (NCDs), with increased overall NCD proportional mortality over the past two decades. The disease burden of mental health increased, resulting from rapid ageing, enhanced stress, and the COVID-19 pandemic, but it was largely neglected. The highly diverse cultures, religions, political systems, socioeconomic contexts, lifestyles, and environmental factors probably have led to massive disparities across countries in NCD mortality, risk factors, and NCD management. Geographically, East Asia had the lowest N…

Clinical trials with antiviral drugs against COVID-19: some progress and many shattered hopes.

Vaccines and drugs are the cornerstones in the fight against the SARS-CoV-2 pandemic. While vaccines were a success story, the development of antiviral drugs against SARS-CoV-2 turned out to be difficult. For an accelerated use of antivirals in the clinic, most SARS-CoV-2 antivirals represented repurposed drugs. The present article summarizes the outcomes of clinical trials with antiviral drugs in COVID-19 patients. Many antiviral drugs failed to demonstrate beneficial effects or showed mixed results. One reason for the low success rate of clinical trials was shortcomings of antiviral tests in

Broad approaches to cholera control in Asia: Water, sanitation and handwashing.

Cholera has been eliminated as a public health problem in high-income countries that have implemented sanitation system separating the community's fecal waste from their drinking water and food supply. These expensive, highly-engineered systems, first developed in London over 150 years ago, have not reached low-income high-risk communities across Asia. Barriers to their implementation in communities at highest risk for cholera include the high capital and operating costs for this technological approach, limited capacity and perverse incentives of local governments, and a decreasing availabilit

Seasonal malaria chemoprevention: successes and missed opportunities.

Seasonal malaria chemoprevention (SMC) was recommended in 2012 for young children in the Sahel during the peak malaria transmission season. Children are given a single dose of sulfadoxine/pyrimethamine combined with a 3-day course of amodiaquine, once a month for up to 4 months. Roll-out and scale-up of SMC has been impressive, with 12 million children receiving the intervention in 2016. There is evidence of its overall benefit in routine implementation settings, and a meta-analysis of clinical trial data showed a 75% decrease in clinical malaria compared to placebo. SMC is not free of shortco

Review of rabies epidemiology and control in South, South East and East Asia: past, present and prospects for elimination.

Rabies is a serious public health problem in Asia. It causes substantial animal welfare, economic and human health impacts, with approximately 39,000 human deaths each year. Domestic dogs are the main reservoir and source of rabies in Asia. Common constraints for the control of rabies in the countries of Asia include inadequate resources; lack of political commitment to control programs; lack of consensus on strategy; weak intersectoral coordination and inadequate management structure; insensitive surveillance systems; limited accessibility to modern rabies vaccine and supply problems; lack of

Toxicology testing in neonates. Is it ethical, and what does it mean?

A positive toxicology screen in a newborn may provide some explanation for signs and symptoms affecting the newborn's problems in the NICU. But, simply stated, it does not prove cause; it does not prove child neglect; it does not prove inadequate parenting capabilities. The evidence of maternal illicit substance use may provide us with information that a variety of risk factors associated with drug use may be interacting that could interfere with the infant's long-term well-being. The objective of screening must be focused on medical and health care issues, not on political or legal issues, an

Enacting Social Justice Leadership: Principals' Modes of Involvement in Urban Secondary Schools in Canada.

This qualitative study examines how principals enact social justice through different modes of leadership involvement in urban Canadian secondary schools. Drawing on semi-structured interviews with principals and vice-principals across three Canadian cities and contextual analysis of school-based initiatives, the study develops an empirically grounded typology of leadership enactment. The findings identify three distinct but interrelated modes of involvement: peripheral support and legitimation, community-embedded brokerage, and structural and curricular intervention. Instead of presenting soc

Beyond Innovation-Speak: Swiss Telephone Workers and the Making of Technical Identity.

This article examines how maintenance workers in Switzerland's Post, Telephone and Telegraph company (PTT) understood and represented their own labor between 1921 and 1959. Drawing on union magazines and previously unpublished sources, it argues that maintenance was not inherently invisible or undervalued but became the object of ongoing struggles over status, expertise, and technological identity. As Switzerland's telephone network expanded, automated, and aged, workers moved from identifying themselves as builders of infrastructure to presenting maintenance as a specialized and indispensable

Anti-Colonial Recoveries: Building an Alternative Infrastructure in New Mexico.

This paper explores the emergence of Quebrar, a community-led drug recovery program, as a dynamic and evolving alternative infrastructure within New Mexico's complex historical, political, and social contexts. Established in 1968 by a coalition of activists, drug users, and individuals in recovery, Quebrar aimed to address the pervasive lack of support for drug addiction in a region marked by racial violence, land dispossession, and the criminalization of drug use. Through ethnographic research and archival sources, this paper explores the processes and strategies through which Quebrar transfo

Arab-Israeli Leaders in Israeli Higher Education During the Iron Swords War.

The study delves into the experiences of Arab-Israeli academic leaders during the Iron Swords War, revealing their emotional responses, coping mechanisms, and reflections on leadership amidst the conflict. The study highlights a range of intense negative emotions experienced by the leaders, including fear, anxiety, helplessness, and despair, reflecting a state of secondary traumatic stress. Despite facing significant challenges, some leaders demonstrated proactivity through their work duties and support from Jewish friends. However, a prevailing sense of pessimism about the future and feelings

Recognizing Humanity, Perceiving Ableism: Supporting Parents of Children with Down Syndrome.

This commentary reflects on twelve narratives authored by parents of children with Down syndrome. While the narratives represent a diverse set of experiences, the commentary identifies some repeated narrative aims, unified under the goal of recognition. For example, multiple authors explicitly or implicitly seek recognition of their children beyond stereotypes while others hope to make ableist values and practices more visible to clinicians and other readers. Through their rich descriptions of parenting, the authors also show themselves to be in need of support both in terms of material and bu

Emotion as a cross-layer mechanism in filter bubbles: a social-psychological perspective.

In increasingly personalized media environments, individuals encounter information that aligns with their existing beliefs, raising concerns about polarization, intergroup hostility, and the erosion of shared political reality. This review synthesizes recent research from a social-psychological perspective, arguing that filter bubbles are not produced by algorithms alone but emerge through the recursive interaction of motivated cognitive processing, identity-based social network structures, and algorithmic amplification of behavioral and emotional cues. We identify emotion as an underrecognize
