OBJECTIVE: To study the clinical, electrophysiological and pathological characteristics and outcome of immune-mediated neuropathy (IMN) following stem cell transplantation (SCT).
METHODS: Retrospective chart review of the Mayo Clinic Rochester SCT database between January 1997 and August 2012.
RESULTS: Of the 3305 patients who underwent SCT, 12 patients (0.36%) had IMN. The median time from SCT to IMN was 7 months. IMN typically presented as an asymmetric radiculoplexus neuropathy (7/12 patients) or acute polyradiculoneuropathy (Guillain-Barré syndrome) (4/12). Neurophysiology showed demyeli…

Reported are three children with MS who responded dramatically to interferon-beta (IFNbeta) therapy. While on immunomodulatory therapy, they developed chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) that responded to IV immunoglobulin (IVIG) administration. These cases emphasize two interesting observations: 1) IFNbeta treatment did not prevent development of CIDP; 2) CIDP in the context of MS responded to IVIG, even though IVIG had no therapeutic effect on the central demyelinating disease.

BACKGROUND: Polyradiculoneuropathy following infection with varicella zoster virus (VZV) is rare and most of the time, happens in the context of reactivation of latent VZV. We report a case of acute polyradiculoneuropathy following primary infection with VZV marked by atypical clinical features raising the hypothesis of a para-infectious disease.
CASE PRESENTATION: We describe a 43-years-old male who developed ataxia, dysphagia, dysphonia, and oculomotor disorders (vertical binocular diplopia and bilateral ptosis) followed by quadriplegia with areflexia which occurred 4 days later. The patien…

Inflammatory neuropathies encompass groups of heterogeneous disorders characterized by pathogenic immune-mediated hematogenous leukocyte infiltration of peripheral nerves, nerve roots or both, with resultant demyelination or axonal degeneration or both. Inflammatory neuropathies may be divided into three major disease categories: Guillain-Barré syndrome (particularly the acute inflammatory demyelinating polyradiculoneuropathy variant), chronic inflammatory demyelinating polyradiculoneuropathy and nonsystemic vasculitic neuropathy (or peripheral nerve vasculitis). Despite major advances in mole…

Cytomegalovirus (CMV) infection is a relatively late complication of AIDS. Like other viruses contributing to co-morbidity of HIV infection, cytomegalovirus has the propensity to cause multiorgan involvement. We report the case of a 34-year-old seropositive man who presented with bilateral lower limb weakness and symptomatic pallor. He was already on antiretroviral drugs for a month prior to presentation. Detailed clinical examination and laboratory investigations revealed cytomegalovirus polyradiculoneuropathy associated with bone marrow dysplasia. Dysplasia of haematopoeitic cell lines occur…

The statistics for this report were obtained from a current survey being conducted by the Philippine Society for Hematology and Blood Transfusion for the purpose of organizing a national apheresis registry. Preliminary data coming from 8 of 10 medical centers included 194 patients and 735 procedures from 1994 to 2004. Eighty percent of patients belonged to ASFA indication Category I with a predominance of neurologic and hematologic disorders. Five most common indications were acute inflammatory demyelinating polyradiculoneuropathy (AIDP), multiple sclerosis (MS), thrombotic thrombocytopenic pu…

This review focuses on recent neuroimmunological findings in autoimmune inflammatory neuropathies. In Guillain-Barré syndrome and paraneoplastic neuropathy most current investigations are centred on the hypothesis of molecular mimicry. In chronic inflammatory demyelinating polyradiculoneuropathy and multifocal motor neuropathy the data on immunopathology are more fragmentary. Why and how patients with autoimmune inflammatory neuropathies raise an increased anti-self-reactivity and how this leads to disease remains a major challenge for future research.

This case report involves a 47-year-old male who presented to the emergency department (ED) with a positive coronavirus disease 2019 (COVID-19) test and symptoms of Guillain-Barré syndrome (GBS). Electrodiagnostic (EDX) studies reported an acute inflammatory demyelinating polyradiculoneuropathy (AIDP). The patient underwent intravenous immune globulin (IVIG) treatment and four weeks of acute inpatient rehabilitation with some functional improvement but remained unable to ambulate independently at discharge.

HLA-DR antigens have been found on Schwann cells in peripheral neuropathies of different origins but not in normal control cases. Class II antigen reactivity was more intense in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and hereditary motor and sensory neuropathy type 1 (HMSN), but was also observed in toxic or metabolic neuropathies. The expression of HLA-DR antigen on Schwann cells does not appear to be related to the inflammatory or autoimmune origin of the disease.

A 50-year-old villager with chronic inflammatory demyelinating polyradiculoneuropathy developed pain abdomen, diarrhea, and vomiting after 8 weeks of prednisolone (40 mg/day) therapy. After 10 weeks, he developed abdominal distension, leucocytosis, thrombocytopenia, liver dysfunction, coagulopathy, and respiratory failure. Stool examination showed larvae of Strongyloides stercoralis. He died in spite of antibiotics, metronidazole, ivermectin, vasopressor, and artificial ventilation. The patients on corticosteroid therapy are at risk of fatal septicemia due to Strongyloides stercoralis hyperinf…

Different conditions that may lead to enlarged nerves or nerve roots include hereditary motor and sensory neuropathy (HMSN), neurofibromatosis (NF) type 1, chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), and intraneural perineurioma. Differential diagnosis of hypertrophic mono- and polyradiculopathies remains challenging but is important because of different treatments and prognosis. Magnetic resonance imaging (MRI) can identify the hypertrophic nerve segments and guide a fascicular biopsy. A fascicular biopsy will often be necessary for precise diagnosis.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) have become key therapies for various malignancies by restoring antitumor immunity. Howeve … was the most frequent ir-MG/myositis pattern. In ir-Neuropathies, acute sensory-motor demyelinating polyradiculoneuropathy predominated.
CONCLUSIONS: Myositis is the most common ir-MG/myositis pattern, while demyelinating polyradiculoneuropathy is typical of ir-Neuropathies. NMJ dysfunction often coexists with myositis and may be missed electrophysiologically.

Peripheral nerve disorders are common and often treatable. The 'default' presentation of a polyneuropathy is a chronic, length-dependent, sensorimotor axonopathy. Recognizing deviations from this default, informed by the clinical features and investigations, can help identify the cause of a neuropathy in most cases. For inflammatory causes, such as Guillain-Barré syndrome and chronic inflammatory demyelinating polyradiculoneuropathy, there are effective immunomodulatory treatments. For other neuropathies, management consists of supportive care and treatment of the underlying cause, to prevent …

Here we report the case of Mrs. O., a 57 years-old woman presenting with mood disorder with psychotic symptoms developing strange skin lesions, ultimately leading to the suspected diagnosis of varicella-zoster encephalitis. The later appearance of a post-infectious acute inflammatory demyelinating polyradiculoneuropathy further confirmed the suspect. This case stresses the importance for not discarding a priori neurological diagnoses when facing with psychiatric patients, especially when atypical details are present.

Guillain-Barré syndrome (GBS) is a rare immune-mediated polyradiculoneuropathy. When severe, GBS can result in near-total paralysis and respiratory failure. Psychological symptoms are common but under-recognized complications of GBS and may develop because of physical disability and social isolation. In this article, we share the patient's perspective of the psychological effects of GBS and how words of encouragement, a listening ear, communication aids, and dedicated caregiving made the difference in the patient's physical and emotional recovery.

Guillain-Barré syndrome (GBS) is an acute inflammatory demyelinating polyradiculoneuropathy of the peripheral nervous system. Involvement of the dorsal root ganglia and the medulla spinalis in GBS is rare, especially in an axonal form. Herein, we report the case of a 53-year-old woman with classic GBS and involvement of the T8-L1 dorsal root segments. Dorsal root and spinal involvement should be kept in mind in all types of GBS when patients present with segmental or dermatomal sensory impairment.

We studied clinically and electrophysiologically 8 patients affected by orthostatic tremor (OT), which is an unusual movement disorder consisting of shaking movements of the legs and trunk in the standing position. We failed to find any cause in 6 of the 8 cases. In 2 patients OT was clearly secondary to neurologic disease: hydrocephalus due to non-tumoral aqueduct stenosis and chronic relapsing polyradiculoneuropathy. The findings obtained suggest a relationship between OT and essential tremor (ET).
