Concomitant cauda equina syndrome and lumbar diskitis secondary to urinary tract infection.

Cauda equina syndrome and lumbar diskitis are relatively uncommon conditions. This case report describes a case of concomitant presentation of cauda equina syndrome and lumbar diskitis in a patient with an Escherichia coli culture-positive extruded disk fragment. To our knowledge this is the first case of a patient who presented with cauda equina syndrome secondary to lumbar diskitis from a urinary tract infection.

A hidden injury.

Transverse sacral fractures associated with cauda equina syndrome are uncommon lesions and often missed at the time of presentation. This case report highlights the benign presentation and the unpleasant outcome of such an injury.

Nitric oxide induced ectopic firing in a lumbar nerve root with cauda equina compression.

Nitric oxide is thought to be involved in the pathophysiology of radiculopathy attributable to herniated intervertebral disc, but its effect on ectopic firing, which is the main source of sciatica, has not been investigated. The authors investigated whether ectopic firing is elicited when the lumbar nerve root is exposed to nitric oxide using rats with and without cauda equina compression. A group of animals had partial L3 laminectomy, and a silicone tube was inserted in the epidural space to compress the cauda equina. A sham operation group and nontreated control group also were prepared. Sev

[Cauda equina hemisyndrome after intradural anesthesia with bupivacaine for hip surgery].

A 68-year-old man underwent hip surgery under subarachnoid anesthesia with bupivacaine and fentanyl to replace an acetabular component. Two days after surgery the patient developed unilateral cauda equina syndrome, affecting five nerve roots (L4 to S3), with no sphincter involvement. Two and a half years later, the lesion had become permanent. We discuss the possible origin of the condition, suggesting differential diagnoses such as mechanical problems (position-mobilization) and anesthetic toxicity.

Cauda equina syndrome following traction for acute sciatica.

A case report of a patient who developed cauda equina syndrome following pelvic traction for acute sciatica is presented. A good outcome was obtained with prompt surgical decompression. This case illustrates the potential dangers of traction, which is frequently employed in the management of acute back pain.

Guillain-Barré syndrome with hydrocephalus in early infancy.

Guillain-Barré syndrome (GBS) is a self-limited and uncommon acquired neurologic syndrome in childhood. It is not thought of as a problem of infancy and is not always included in the differential diagnosis of the floppy infant. This case report describes GBS ina 1-month-old infant. In addition this infant developed hydrocephalus concurrent with the appareance of high CSF protein levels. This patient is described to demonstrate that GBS can occur at all ages and that hydrocephalus may occur particularly when the CSF protein is at an unusually high level.

Generalized mastocytosis, relapsing herpes zoster and polyradiculoneuritis.

A 68-year-old woman suffering from a longstanding urticaria pigmentosa with mast cell infiltrations in bones had two attacks of herpes zoster. The second herpes zoster attack was followed by a severe polyradiculoneuritis (Guillain-Barré syndrome). The sequence of events suggests an immunological deficiency.
