We present the case of a 28-year-old man with a growing mass in his right popliteal fossa causing pain on exertion. The differential diagnosis included Baker's cyst, entrapment syndrome of the popliteal artery, as well as a benign or malignant neoplasm. An ultrasound was non-specific. Follow-up MRI of the knee demonstrated cystic adventitial disease (CAD). With only about 500 cases reported in the literature since its discovery in 1947, CAD is a rare entity. The disease is characterised by mucinous or gelatinous cysts in the arterial or venous adventitia. The disease is predominantly seen in t

Popliteal artery compression may be due to trapping of the artery, a tumor, or a subadventricial cyst. Diagnosis is suggested by the discovery of ischemia in a subject who is often young, this being an unusual finding. It is confirmed by dynamic doppler tests, ultrasonography, and vascular opacification. Eight cases of trapped popliteal, three of osteochondroma, and seven cases due to cysts were observed. Surgical treatment is regularly effective and can prevent complications of embolism and acute thrombosis.

A new type of amyloidosis, secondary to the massive deposition of beta 2-microglobulin, has been identified which is peculiar to long-term ( … iolucencies (5 and 4 cases, respectively). These findings, while documenting the high prevalence of popliteal cysts among long-term hemodialysis patients and the strong correlation with dialysis-related amyloidosis, stress the importance of diagnostic imaging in the detection and follow-up of such lesions.

PURPOSE: The purpose of this study was to determine the incidence and characteristics of Baker's cysts discovered during venous duplex examinations to rule out deep vein thrombosis (DVT).
METHODS: We reviewed the vascular laboratory charts of patients found to have Baker's cysts during venous duplex studies to rule out DVT from October 1988 through December 1995.
RESULTS: Ninety-five (3.1%) of 3072 patients who underwent venous duplex studies were found to have 111 Baker's cysts. Seven of the 95 had coexistent DVT. Ten patients had ruptured cysts, whereas six patients had cysts that compress

BACKGROUND: Baker's cysts are usually located in the posteromedial side of the knee and seldom cause neuropathy.
CASE: We describe the rare case of a 57-year-old woman with a popliteal cyst who presented with limping gait and pain in her lower leg. She was electronically diagnosed with common peroneal neuropathy and transferred to our pain clinic. On ultrasound examination, about 2.0 × 1.2 cm sized popliteal cyst was found to extend to the fibular head, compressing the common peroneal nerve. Therefore, ultrasound-guided aspiration of the cyst and a common peroneal nerve block were performed. …

A common reason for painful lesions of the popliteal fossa are baker's cysts. An important differential diagnosis is the popliteal artery aneurysm, which is rare, but is associated with severe complications. The preferred method of diagnosis is the colour-coded duplex ultrasound. By showing the case of a 58-year-old man, who experienced life-threatening complications caused by an insufficient diagnosis before the operation of a Baker's cyst, we want to underline the importance of preoperative duplex ultrasound diagnosis in this context.

OBJECTIVE: The presence of popliteal cysts is common, although often unkown to the patient. When spontaneous rupture occurs, the clinical presentation is typically dramatic, frequently prompting the patient to seek emergency care. Eventually the condition can be mistaken for other pathologies, more commonly deep vein thrombosis, leading to inappropriate treatment. The objective of this study is to describe a case series of spontaneous Baker's cyst ruptures, with a 2-year follow-up.
METHODS: Sixteen consecutive patients with symptomatic ruptured popliteal cyst were treated acutely with anti-in…

Cystic adventitial disease (CAD) is a rare vascular condition that most commonly affects the popliteal artery. Percutaneous transluminal angioplasty (PTA) is generally not considered a valid therapeutic option due to high recurrence rate. We report a case of CAD of the popliteal artery that recurred after surgical cyst enucleation that was successfully treated with PTA. To the best of our knowledge, this is the first case of successful PTA for the treatment of recurrent CAD of the popliteal artery.

Popliteal swelling is a common complaint seen in the practice of orthopaedics. Although imaging is useful to aid in the diagnosis of popliteal swelling pre-operatively, definitive diagnosis is often obtained post-operatively through histopathological report of the swelling. Baker's cyst arises medially and hence usually spares the posterolaterally located neurovascular bundle until it becomes larger in size. A thrombosed aneurysm can mimic that of Baker's cyst on computed tomography (CT) imaging in view of its location and the absence of contrast within the lesion. Diagnosis of a popliteal swe

Popliteal artery aneurysm is a potentially limb-threating lesion which is frequently difficult to diagnose by clinical examination or angiography. The risk of limb loss can best be reduced by early diagnosis. Over the past 5 years we have prospectively evaluated ultrasound examination of the popliteal space in screening patients with clinically suspected popliteal artery aneurysms. One hundred patients who were suspected clinically of having a popliteal artery aneurysm underwent bilateral ultrasound examination of their popliteal fossae. There were 21 patients who had abnormal scans. The probl

Cystic adventitial disease (CAD) is a rare cause of claudication in which a mucinous cyst forms within the adventitial layer of a blood vessel, most commonly the popliteal artery, resulting in luminal narrowing. This paper presents a case of a man in his mid-50s with unilateral lower limb popliteal artery CAD, which was not initially identified on either duplex ultrasonography or CT angiography. He was successfully treated with surgical resection of the affected segment of the popliteal artery and autologous vein grafting.

A case of adventitial cystic disease of the popliteal artery is described and the subject reviewed. Now that radiologists will often follow arteriography with angioplasty or thrombolytic therapy, it is particularly important that this condition is recognised.

Baker's cysts are fluid-filled swellings in the popliteal fossa commonly associated with degenerative and inflammatory joint diseases, including rheumatoid arthritis (RA), as well as with joint injuries. While they are usually small and asymptomatic, in rare cases, they can enlarge significantly and cause clinically relevant complications. We describe the case of a 69-year-old man with an undiagnosed RA who presented at his first Internal Medicine consultation with a five-week history of progressive swelling in the left popliteal region extending to the mid-calf, accompanied by pain on movemen

Complications of popliteal cysts may closely mimic the clinical features of a deep venous thrombosis. We assessed the sensitivity and specificity of the non-invasive procedures of radionuclide venography and popliteal space ultrasound examination compared with those of contrast venography and arthrography, respectively, and then prospectively studied 23 non-surgical patients with acutely painful, swollen calves to determine the utility of these techniques. The cause of this symptom was popliteal cyst complications in 10 patients, deep venous thrombosis in seven patients, and both conditions in…

Septic arthritis associated with extra-articular abscess is rare. We report on 2 non-rheumatic patients with septic arthritis of the knee associated with calf abscess. Magnetic resonance imaging showed a distinct leakage pathway from the knee joint in each patient. One was a ruptured popliteal cyst (posteromedial). Another was a pathologic popliteus hiatus (posterolateral). These patients underwent open drainage of the calf abscess via a small incision, followed by arthroscopic debridement of the knee. Careful palpation of the lower leg, followed by magnetic resonance imaging and needle aspira…

Cystic degeneration of the popliteal artery is a rare vascular condition. The authors report on the third case recorded in Canada and provide a follow-up report of the first recorded case published in 1969. Observations made at operation on the anatomical relationship of the cyst give more information on the etiology of this curious entity. The clinical and roentgenologic aspects, the etiologic theories and treatment are briefly reviewed. The authors conclude that there are three principles of treatment applicable to this condition: (a) incision with evacuation of the cyst may be effective, (b

Primary hydatid cyst of the mediastinum is rare. We report a patient with a primary mediastinal hydatid cyst which ruptured into the aorta, resulting in bilateral popliteal embolism. The contrast-enhanced computed tomographic features represent a pathognomonic sign of a communicating rupture of an echinococcal cyst into the aorta. Comparison is made with ultrasonography and aortography.
