We examined clinical and laboratory data and the liver pathology of 48 patients in whom porphyria cutanea tarda was related to alcohol ingestion, estrogen use and pregnancy, or was idiopathic. Biochemical test results, when abnormal, tended to be mild in most cases, with less than two-fold elevations of serum aminotransferases and alkaline phosphatase and mild hyperbilirubinemia. Fatty change, liver cell and Kupffer cell hemosiderosis and glycogenation of hepatocyte nuclei were frequent histologic findings in the 58 liver specimens studied. Numerous drugs and environmental chemicals are capable of influencing the clinical expression of human hepatic porphyria primarily by interfering with the orderly regulation of heme synthesis in the liver. Some agents trigger the disease in otherwise normal individuals whereas others exacerbate an underlying genetic abnormality leading to disease expression. Porphyria cutanea tarda (PCT) is common in Ethiopia and invariably affects the liver. Treatment by abstension from alcohol and avoidance of direct sunlight often failed to achieve lasting improvement. Phlebotomy is unacceptable to most of our patients and impractical as a routine therapy. Chloroquine phosphate 500 mg (300 mg base) given daily for 10 days to 24 patients with confirmed PCT, was found to be uniformly effective. The treatment of Porphyria Cutaneous Tarda (PCT) has greatly improved by the introduction of low-dose chloroquine. The authors report the results in the treatment of 17 patients presenting PCT. The clinical and biological controls of these patients at the 6th and 12th months of treatment showed the disappearance of clinical signs and the normalization of liver function tests and porphyrin excretion between and 6th and the 12th months of treatment in all the cases.

We report the findings in 53 biopsies from 45 patients with porphyria cutanea tarda (PCT). Red autofluorescence and birefringent acicular cytoplasmic inclusions were constant findings in all untreated cases. Autofluorescence occurs in other hepatic porphyrias, but acicular inclusions appear to be specific for PCT; we have seen them in subclinical porphyria and before development of cutaneous symptoms. Congenital porphyria was diagnosed in 4 crossbred cattle. All had pink-brown discoloration of the teeth and amber colored urine, which fluoresced bright orange with exposure to ultraviolet light. Elevated levels of uroporphyrin and coproporphyrin were found in the urine of all 4 cattle. Skin biopsies taken from 2 of the cattle revealed subepidermal blister formation and thickening of upper dermal blood vessel walls with deposition of a PAS-positive, hyaline-like material.

A 24-year-old woman developed bilateral blindness after recovery from coma secondary to acute intermittent porphyria. Gradual return of vision in the right eye with a permanent unilateral visual field defect and optic atrophy followed. We believe the pathophysiologic mechanism was spasm of the vessels supplying the optic disk leading to ischemia and infarction of the optic nerve.

Three patients with porphyria cutanea tarda and lymphoma were seen at the Mayo Clinic. In one patient, the signs and symptoms of porphyria cutanea tarda and lymphoma occurred simultaneously, whereas in the other two, the porphyria was seen before the lymphoma occurred, three years before in one and one and a half years before in the other. Involvement of liver or marrow was not related to the development of porphyria cutanea tarda in any of the 3 patients. Disturbances in the iron metabolism can quite frequently be observed in patients with porphyria cutanea tarda. Studies on 10 patients with porphyria cutanea tarda indicate that elevated iron levels are correlated with decreased latent and normal total iron binding capacity in the serum. Morphological examinations of the liver showed alterations as can be found in fatty liver up to cirrhosis, which -- in most instances -- were associated with iron deposits in the hepatocytes.
