Researchers operationalize persistent post-concussion symptoms in children and adolescents using varied definitions. Many pre-existing conditions, personal characteristics, and current health issues can affect symptom endorsement rates in the absence of, or in combination with, a recent concussion, and the use of varied definitions can lead to differences in conclusions about persistent symptoms and recovery across studies. This study examined how endorsement rates varied by 14 different operational definitions of persistent post-concussion symptoms for uninjured boys and girls with and withou

OBJECTIVE: The term "post-traumatic migraine" (PTM) has been used to describe post-traumatic headaches (PTHs) that have associated migraine features, but studies of this relationship are lacking. The objective of the present study was to determine whether PTH correlates strongly with migraine symptoms among youth with concussion.
METHODS: Twenty-three symptoms were analyzed from a retrospective cohort of 1953 pediatric patients with concussion. A principal component analysis (PCA) with oblique Promax rotation was conducted to explore underlying symptom relationships in the full cohort and in

Concussion is a pervasive health issue in the present day. Increased prevalence in recent years has indicated a need to improve the current understanding of minor traumatic brain injury (mTBI). Effort has been devoted to understanding the underlying pathophysiology of TBIs, but some mechanisms remain unknown. Potentially lethal secondary effects of concussion include second impact syndrome and chronic traumatic encephalopathy (CTE), introducing long-term considerations for the management of mTBI. Post-concussion syndrome is another long-term consequence of concussion and may be influenced by b…

Concussions represent a major public health concern due to their high prevalence and the risk of long-term consequences. In some patients, they can lead to persistent post-concussion symptoms lasting for several months or even years, significantly impacting their quality of life. To date, no curative treatment exists for this syndrome, but early and appropriate management plays a key role in preventing long-term complications. However, concussions often remain underdiagnosed, both by healthcare professionals and the patients themselves. Various assessment tools have been developed to facilitat

Traumatic brain injury (TBI) imposes a significant economic and social burden. The diagnosis and prognosis of mild TBI, also called concussion, is challenging. Concussions are common among contact sport athletes. After a blow to the head, it is often difficult to determine who has had a concussion, who should be withheld from play, if a concussed athlete is ready to return to the field, and which concussed athlete will develop a post-concussion syndrome. Biomarkers can be detected in the cerebrospinal fluid and blood after traumatic brain injury and their levels may have prognostic value. Desp…

BACKGROUND: Comparison of patient-reported outcomes in multilingual studies requires evidence of the equivalence of translated versions of the questionnaires. The present study examines the factorial validity and comparability of six language versions of the Rivermead Post-Concussion Symptoms Questionnaire (RPQ) administered to individuals following traumatic brain injury (TBI) in the Collaborative European NeuroTrauma Effectiveness Research (CENTER-TBI) study.
METHODS: Six competing RPQ models were estimated using data from Dutch (n = 597), English (n = 223), Finnish (n = 213), Italian (n =

BACKGROUND: The Postconcussion Symptom Inventory (PCSI) evaluates the severity of 21 concussion-related symptoms, yielding a total symptom severity between 0 and 126. Although widely used in concussion research and care as a key element in diagnosis and management decisions, no clinically validated cut point exists to distinguish adolescents with and without a recent concussion. Therefore, the purpose of our study was to establish a PCSI cut point with adequate sensitivity, specificity, and classification accuracy for clinical use.
METHODS: Adolescents within 21 days of concussion and uninjur

Children with concussion are injured through a variety of mechanisms, but the relationship between mechanism of injury (MOI) and recovery outcomes is unclear due to small sample sizes and varied methodological designs. Our objective was to examine the association of MOI and clinical recovery in youth with concussion using a large dataset collated from a single, multisite study. We hypothesized that sport-related concussion would be related to better clinical presentation and faster recovery trajectories compared to other mechanisms of concussion. This study was a secondary analysis of data col

A woman in her late 20s developed persistent post-concussion symptoms (PPCS) following a mild traumatic brain injury (mTBI) sustained in a motor vehicle collision. Symptoms included daily headaches, cognitive and physical fatigue and vision disturbances, which persisted despite extensive rehabilitation and medical interventions by multiple specialists. At 20 months post-mTBI, an endocrinological assessment confirmed post-traumatic growth hormone deficiency (GHD) through provocative glucagon stimulation testing (GST) revealing a peak GH level of 1.2 µg/L, and recombinant human growth hormone (r

For some, post-concussion symptoms following a mild traumatic brain injury (mTBI) are prolonged in nature, lasting for a minimum of 12 months and up to many years. There remains limited insight into the effectiveness of psychological interventions for the treatment of prolonged post-concussion symptoms (PrPCS). This systematic review aimed to evaluate the effectiveness of psychological interventions for PrPCS (>12 months post mTBI). A search was performed across five databases (PsychINFO, Medline, Web of Science, Cumulative Index to Nursing and Allied Health Literature (CINAHL), & PubMed). Two

INTRODUCTION AND OBJECTIVES: Early mild traumatic brain injury (mTBI or concussion sustained between 0 and 5 years old) can lead to post-concussive symptoms, behavioral changes, and cognitive difficulties. Although school-age children (6-17 years old) experience similar consequences, severe neuropsychological deficits are not common, and the majority have no persisting symptoms after one month. Thus, there may be value in focusing on what characterizes optimal functioning (or wellness) after mTBI, but this has not been explored in young children. This study documents the evolution and predicto

This study examined the association between past concussions and current preseason symptom reporting and cognitive performance in 9,257 youth ages 11-13. Participants completed neurocognitive testing prior to participating in a school sports between 2009 and 2019. We stratified the sample by gender and number of prior concussions and assessed group differences on the Post-Concussion Symptom Scale total score and the ImPACT cognitive composite scores. Those with≥2 prior concussions reported more symptoms than those with 0 concussions (d=0.43-0.46). Multiple regressions examining the contributio

BACKGROUND: Return to work (RTW) might be delayed in patients with complicated mild traumatic brain injury (MTBI), i.e., MTBI patients with associated traumatic intracranial lesions. However, the effect of different types of lesions on RTW has not studied before. We investigated whether traumatic intracranial lesions detected by CT and MRI are associated with return to work and post-concussion symptoms in patients with MTBI.
METHODS: We prospectively followed up 113 adult patients with MTBI that underwent a brain MRI within 3-17 days after injury. Return to work was assessed with one-day accu

BACKGROUND: Nearly 2 million youth seek acute medical care following concussion in the U.S. each year. Current standard of care recommends rest for the first 48 h after a concussion. However, research suggests that prolonged rest may lengthen recovery time especially for patients with certain risk profiles. Research indicates that physical activity and behavioral management interventions (sleep, stress management) may enhance recovery. To date, there is limited empirical evidence to inform acute (<72 h) concussion recommendations for physical activity and behavioral management in adolescents.

The long-term effects of mild TBI (mTBI) are not well understood, and there is an ongoing debate about whether there are sex differences in outcomes following mTBI. This study examined i) symptom burden and functional outcomes at 8-years post-injury in males and females following mTBI; ii) sex differences in outcomes at 8-years post-injury for those aged <45 years and ≥45 years and; iii) sex differences in outcomes for single and repetitive TBI. Adults (≥16 years at injury) identified as part of a population-based TBI incidence study (BIONIC) who experienced mTBI 8-years ago (N = 151) and a TB

CONTEXT: After concussion, a multifaceted assessment is recommended, including tests of physical exertion. The current criterion standard for exercise testing after concussion is the Buffalo Concussion Treadmill Test (BCTT); however, validated tests that use alternative exercise modalities are lacking.
OBJECTIVE: To evaluate the feasibility and concurrent validity of a universal cycling test of exertion compared with the BCTT in adults who sustained a sport-related concussion.
DESIGN: Crossover study.
SETTING: University sports medicine clinic.
PATIENTS OR OTHER PARTICIPANTS: Twenty adults

BACKGROUND.: Concussion is a common injury in an adolescent population with up to 30% experiencing persistent symptoms. Rehabilitation programs that include aerobic exercises can reduce persistent postconcussion symptoms. However, it is unclear if daily life physical activities can also reduce symptoms.
PURPOSE.: To investigate whether the level of daily life physical activity reduces postconcussion symptoms in the adolescent population.
METHODS.: Ten adolescents aged 11-18 years with a concussion self-reported their postconcussion symptoms and level of physical activity during the first, th
