The arthrogrypotic foot plan of management and results of treatment.

Fifty-one arthrogrypotic feet have been treated and followed by the Pediatric Orthopaedic Unit, Tufts New England Medical Center, (1970-1980 … problem in the simple type of posterior release including only an Achilles tendon lengthening, and posterior capsulotomy of the ankle and subtalar joint. Talectomy has been carried out in four feet and appears to be one type of reasonable salvage procedure in smaller children with recurrent varus. Treatment is difficult in these patients but a plantigrade foot should …

[Increased cataract rate and characteristics of Nd:YAG laser capsulotomy in retinitis pigmentosa].

BACKGROUND: Retinitis pigmentosa (RP) is associated with the development of a posterior subcapsular cataract (PSC). The development of posterior capsule opacification (PCO) after cataract surgery and a decrease of central visual acuity is sometimes misinterpreted by the patients as natural course of RP. Therefore, therapeutic intervention is often delayed. PATIENTS AND METHODS: In a retrospective study (part 1) the incidence of PCO was evaluated in a group of 26 RP patients who underwent cataract surgery and IOL implantation. In a prospective analyse (part 2) PCO was quantified in 13 RP patie

Rotation versus non-rotation of intraocular lens for prevention of posterior capsular opacification.

PURPOSE: To study the effect of rotation of intraocular lens (IOL) on posterior capsular opacification (PCO) in eyes with phacoemulsification. METHODS: This was a prospective, comparative, randomized case series. One eye of each patient was randomized to one of two groups. The 360-degree rotation of IOL was carried out after its placement in the capsular bag (rotation group). The control group had no rotation of IOL. PCO was analyzed by an independent observer on EPCO computer analysis system at 6, 12, 24, and 36 months. RESULTS: The study included 50 patients (100 eyes) with senile cataract

An unusual case of marble intraocular foreign body.

This report presents a case of marble intraocular foreign body that developed toxic complications during surgery. The patient is a 25 years old male who presented to the University of Benin Teaching Hospital with a history of trauma to the right eye while cutting marble. He was examined, had an ocular ultrasound scan and subsequently had an extracapsular cataract extraction. His visual acuity in the right eye was light perception. There was an entry point on the cornea, the lens was opaque, there was vitreous haemorrhage and the intraocular foreign body was localized in the posterior part of t

[Phacoemulsification for mature or hypermature cataracts].

OBJECTIVE: To discuss how to choose the methods of capsularhexis and phacoemulsification in cases with mature or hypermature cataract. METHODS: On fifty-nine mature or hypermature cataract patients, continuous circular capsularhexis or can-opener capsulotomy was performed. The nucleus was emulsified by the method of divide and conquer or phaco chop or rotatory carousel in the bag or at the plane of iris. RESULTS: Continuous circular capsularhexis was successful in 51 cases, failed or shifted to can-opener in 8 cases. There were posterior capsular rupture in 1 cases, partial dehiscence of the

Posterior capsular contracture of the shoulder.

Posterior capsular contracture is a common cause of shoulder pain in which the patient presents with restricted internal rotation and reproduction of pain. Increased anterosuperior translation of the humeral head occurs with forward flexion and can mimic the pain reported with impingement syndrome; however, the patient with impingement syndrome presents with normal range of motion. Initial management of posterior capsular contracture should be nonsurgical, emphasizing range-of-motion stretching with the goal of restoring normal motion. For patients who fail nonsurgical management, arthroscopic

Pathology of intraocular lenses in 33 eyes examined postmortem.

Thirty-three pseudophakic eyes from 29 patients were examined postmortem. Clinically, 25 of the eyes had had visual acuities of 20/50 or better. In general the intraocular lenses (IOLs) were quite well tolerated. Iris atrophy and erosion were prominent features of eyes containing an anterior chamber, iris fixation, or iridocapsular lens. Angle recession and ciliary-body erosion were found in some eyes with anterior chamber lenses. Ciliary-body erosion by haptics was seen with posterior chamber lenses. Pupillary membranes were present in eight eyes. Granulomatous reaction to IOL material was pr

Occult capsular injury in suspected traumatic and iatrogenic cataracts: a case report with imaging and pearls for surgical management.

Traumatic and iatrogenic cataracts may conceal occult capsular injury that routine slit-lamp examination and biometry fail to detect, increasing the risk of intraoperative complications. We present a mechanism-aware, imaging-guided approach to cataract surgery in an eye with suspected capsular compromise. An elderly man with a dense cataract and limited fundus view underwent preoperative imaging, which revealed a focal anterior capsular defect on ultrasound biomicroscopy, while posterior capsule integrity remained uncertain. His history included multiple intravitreal injections and prior anter

Comparison of the soft-tissue release methods in idiopathic clubfoot.

The aim of this study was to compare the clinical and radiologic results of three different surgical procedures (posterior release with lengthening of the tendo calcaneus and posterior capsulotomy, Turco's technique of posteromedial release, and Simons' technique of complete subtalar release) in idiopathic clubfoot in 77 patients who were operated on at 10 months of age or younger. The clinical examination alone is not sufficient to determine the type of the surgical intervention needed in idiopathic clubfoot; the decision must be supported by the radiologic parameters (anteroposterior and lat…

Lens exchange for management of accommodative intraocular lens tilting.

Accommodative intraocular lens (IOL) tilting, the so-called Z syndrome, is a rare complication of Crystalens (Bausch and Lomb) implantation. We report a significant IOL tilting and subsequent high lenticular astigmatism due to posterior capsular fibrosis 2 months after uncomplicated cataract surgery and Crystalens (AT50AO) implantation. The attempt to correct IOL position with neodymium-yttrium-aluminum garnet laser was unsuccessful, and Crystalens exchange with in-the-sulcus, three-piece monofocal IOL was performed. Accommodative IOL tilting could occur early after the surgery. Laser capsulot

Correction of the Pulfrich phenomenon by surgery and laser.

We describe 2 patients who presented with asymmetrical media changes and a spontaneous Pulfrich phenomenon. In the first case, the phenomenon was due to cataract formation and an interocular latency difference of 1.49 ms +/-0.07 (SD) was found. After cataract surgery, the symptoms resolved and no interocular latency difference could be measured. In the second case, the phenomenon was caused by posterior capsule opacification and an interocular latency difference of 1.16 +/- 0.08 ms was found. The symptoms were alleviated and no interocular latency difference could be measured after a neodymium

[Use of neodym-YAG-lasers in ophthalmology].

Structures within the eye bulb diminishing visual acuity can be cut by means of neodymium-YAG-laser without surgical opening of the globe. The First Eye Department has had the first instrument of this kind in Vienna at its disposal since the summer of 1984. Up to now it has been used for posterior capsulotomy, iridotomy, opening pupillary membranes and for cutting vitreous membranes in 84 operations altogether. The advantages of neodymium-YAG-laser are minimal psychological and physical stress for the patient and the possibility of out-patient surgery. A possible transient increase in the eye …

Femtosecond laser-assisted capsulotomy after penetrating injury of the cornea and lens capsule.

We present a 39-year-old patient with a penetrating injury of the cornea and anterior lens capsule containing an intralenticular foreign body. One week after the accident, the patient consulted our emergency unit. Clinical examination showed no Seidel II phenomenon. A femtosecond laser-assisted capsulotomy was performed followed by lens removal and intraocular lens implantation. The capsulotomy diameter was increased to 6.7 mm to include the foreign-body area and the break in the anterior capsule. The procedure was uneventful, and the capsule leaf, including the foreign body, was extracted saf
