Thyroid antibodies and fetal loss: an evolving story.

This article reviews the literature on thyroid antibodies and miscarriage. In 1990, in a study designed to determine the incidence and etiology of postpartum thyroiditis, a serendipitous finding emerged revealing an association between thyroid antibodies and spontaneous miscarriage. Subsequently, four other studies, performed on three different continents, have confirmed the correlation. Six studies have evaluated the relationship between thyroid antibodies and recurrent abortion, defined as three or more spontaneous miscarriages. The majority of the studies (67%) reported a statistically sign…

Thyroidopathies.

Pregnancy affects thyroid physiology in many ways: (a) The renal iodide clearance rate is increased, hence iodine requirements increase. (b) … autoimmunity usually subsides during pregnancy, but may rebound a few months after parturition, and postpartum thyroiditis may occur. Because maternal antithyroid autoantibodies cross the placenta readily, fetal and neonatal hyperthyroidism (or hypothyroidism) may develop. Pre-existing thyroid diseases are influenced. Nontoxic goiter increases in size. Iodine and/or thy …

Thyroid autoantibodies and associated complications during pregnancy.

This study was undertaken to determine the presence of thyroid autoantibodies and associated pregnancy complications from 49 pregnant women … e thyroid antibodies and pre-eclampsia (15.4%), abruptio placenta (4%), caesarean deliveries (31%), postpartum thyroiditis (19.2%) and abnormal neonatal thyroid function (15.4%). Women with positive thyroid antibodies in pregnancy need close care during and after pregnancy, as they can develop complications affecting both mother and fetus.

Post-pregnancy osteoporosis-related multiple vertebral fractures associated with post-partum thyroiditis: A CARE-compliant case report.

INTRODUCTION: Osteoporosis is a condition commonly observed in elderly and postmenopausal women. Pregnancy and lactation-induced osteoporosis are rare, and the development of severe vertebral fractures is uncommon. Postpartum thyroiditis (PPT) is a minor cause of osteoporosis. To the best of our knowledge, the development of osteoporosis associated with pregnancy has not yet been reported. PATIENT CONCERNS: Here, we report a rare case of post-pregnancy osteoporosis-related multiple vertebral fractures associated with PPT. A 25-year-old woman developed lower back pain after her first delivery.…

[Endocrine diseases at the National Hospital of Niamey (Niger). Apropos of 195 cases].

The authors report 195 cases of endocrine diseases diagnosed in a department of internal medicine at the National Hospital of Niamey (Niger) from January 1982 to December 1986. Diseases of the thyroid gland were largely predominant (70.8%), with simple (34.9%) and nodular (19%) goiters. Hypothyroidism and thyroiditis were not frequent (2 and 2.6%, respectively). The rarity of thyroid cancers (1%) was in relation to the direct hospitalization of these patients in the surgical department. They were not included in this study. In spite of the magnitude of tuberculosis endemicity in Niger, primary

A rare case of an antenatally diagnosed tumor of the neck--a hamartoma of the thyroid gland.

A case of connatal hamartoma of the left thyroid lobe is reported. During routine prenatal ultrasonographic examination, a tumor of the neck was diagnosed. The baby was born at 30 weeks' gestation, and the tumor (measuring 7.0 x 5.5 x 4.2 cm) was extirpated. Histologically, the tumor was a hamartoma of the thyroid gland. Follow-up evaluation (3 and 6 months after resection) showed that the child was doing well and had no signs of hypothyroidism or hypoparathyroidism. The case is discussed with respect to antenatal diagnosis, postpartum diagnostic measures, management, and surgical therapy.

Peripartum Management of Refractory Graves' Thyrotoxicosis.

Graves' disease (GD) is the most common cause of hyperthyroidism, characterized by autoimmune stimulation of the thyroid gland and typically managed with antithyroid drugs (ATDs), radioiodine (RAI) therapy, or surgery. While most patients respond to standard antithyroid therapy, a minority exhibit refractory thyrotoxicosis, posing significant clinical challenges. Pregnancy further complicates management due to immunological shifts, increased risk of adverse maternal and fetal outcomes, and limitations in therapeutic options. We present the case of a 39-year-old woman with severe, persistent Gr

Thyroid hormones and female reproduction.

Thyroid hormones are vital for the proper functioning of the female reproductive system, since they modulate the metabolism and development of ovarian, uterine, and placental tissues. Therefore, hypo- and hyperthyroidism may result in subfertility or infertility in both women and animals. Other well-documented sequelae of maternal thyroid dysfunctions include menstrual/estral irregularity, anovulation, abortion, preterm delivery, preeclampsia, intrauterine growth restriction, postpartum thyroiditis, and mental retardation in children. Several studies have been carried out involving prospective…

Sheehan's syndrome co-existing with Graves' disease.

Sheehan's syndrome (SS), which is an important cause of hypopituitarism, is common in developing countries. The most common presentation is the absence of lactation and amenorrhea. Hypothyroidism rather than hyperthyroidism is the usual expected phenomenon in SS. Postpartum hyperthyroidism is also common and Graves' disease (GD) is an important cause of postpartum hyperthyroidism. Here we report a case of a 22-year-old female patient in our clinic presented symptoms of amenorrhea, lack of lactation, palpitations and sweating. Her physical examination revealed goiter, moist skin and proptosis.

Pregnancy-associated thyroid disorders: the role of genetic, epigenetic, and oxidative stress factors.

Thyroid inflammation during pregnancy, particularly Hashimoto's thyroiditis (HT) and postpartum thyroiditis (PPT), has a strong genetic and epigenetic basis. Susceptibility to these conditions is associated with specific HLA haplotypes (HLA-DR3, DR4, DR5) and immune-regulatory genes, including CTLA-4, PTPN22, FOXP3, as well as thyroid-specific genes such as TSHR, TG, and TPO. CTLA-4 polymorphism (CT60) is linked to increased thyroid autoantibody production, while PTPN22 R620W variant disrupts immune tolerance, exacerbating autoreactive lymphocyte activation.Epigenetic modifications play a cruc…

Bilateral breast uptake of radioiodine in a male patient with gynecomastia: A case report.

A 56-year-old male patient with papillary thyroid carcinoma was given radioiodine for the treatment of persistent disease. Post radioiodine whole body scan revealed uptake at the thyroidal region and bilateral uptake at the upper thoracic region. Single photon emission computed tomography/computed tomography (SPECT/CT) confirmed uptake at the left thyroid lobe, and additional symmetrical mammary gland uptake was observed at both breasts. The patient had obesity-related gynecomastia, but he did not have any history of breast cancer, mastitis, hyperprolactinemia, or galactorrhea. Although breast

Perinatal Thyroid Dysfunction: Prenatal Diagnosis and Treatment.

Thyroid diseases affect up to 10% of women, but most respond well to treatment. During pregnancy, however, normal metabolic changes may obscure pathology, and improper management may harm the fetus. Tests for levels of thyroid stimulating hormone (TSH), free thyroxine, and free triiodothyronine are essential. Generally, high TSH values suggest primary hypothyroidism, while suppressed levels indicate hyperthyroidism. Hyperthyroidism is commonly manifested by goiter, ophthalmopathy, proximal muscle weakness, tachycardia, and weight loss or inability to gain weight. Among women, the most common e

Diabetes insipidus associated with craniopharyngioma in pregnancy.

A case is presented of a pregnancy complicated by a suprasellar mass diagnosed at 27 weeks' gestation. This patient developed diabetes insipidus, which was successfully treated with 1-desamino-8-D-arginine vasopressin. Thyrotropin-releasing hormone and ACTH stimulation tests were also abnormal, requiring the institution of thyroid and cortisol replacement therapy. The patient was delivered at 34 weeks' gestation secondary to worsening visual field testing. A craniotomy was performed in the postpartum period with removal of a craniopharyngioma. With successful medical treatment and careful obse

From Gut to Goiter: How Salmonella Gastroenteritis Causes Thyroid Abscess.

INTRODUCTION: The innate characteristics of the thyroid gland that resist infection render thyroid abscesses extremely uncommon. Moreover, gram-negative species, such as Salmonella spp., are a rare cause of thyroid abscess from hematogenous dissemination. CASE DESCRIPTION: A 41-year-old postpartum, breastfeeding female developed neck swelling and dysphagia following gastroenteritis. Imaging revealed a thyroid abscess. Fine needle aspiration (FNA) identified purulent fluid positive for Salmonella spp. Although the patient was initially responsive to antibiotics, recurrent swelling, dysphagia,
