PROSPECT guideline for hallux valgus repair surgery: a systematic review and procedure-specific postoperative pain management recommendations.

Hallux valgus repair is associated with moderate-to-severe postoperative pain. The aim of this systematic review was to assess the available literature and develop recommendations for optimal pain management after hallux valgus repair. A systematic review using PROcedure SPECific Postoperative Pain ManagemenT (PROSPECT) methodology was undertaken. Randomized controlled trials (RCTs) published in the English language from inception of database to December 2019 assessing postoperative pain using analgesic, anesthetic, and surgical interventions were identified from MEDLINE, EMBASE, and Cochrane

A Multimodal Protocol to Diminish Pain Following Common Orthopedic Sports Procedures: Can We Eliminate Postoperative Opioids?

PURPOSE: To determine whether postsurgical pain, measured by the visual analog scale (VAS), following common orthopaedic sports procedures could be managed effectively with a nonopioid multimodal analgesic protocol. METHODS: This prospective study evaluated a custom multimodal nonopioid pain protocol in patients undergoing common orthopaedic sports procedures by a single fellowship-trained orthopaedic sports surgeon from May 2018 to December 2018. Procedures included anterior cruciate ligament reconstruction, rotator cuff repair, arthroscopic partial meniscectomy, and labrum repair. The nonop

One Less Painful Procedure: Using Umbilical Cord Blood as Alternative Source to Admission Complete Blood Count.

Objective This study aims to evaluate the use of umbilical cord blood as an alternative to the admission complete blood count (CBC) in the well-appearing late preterm neonates admitted to the neonatal intensive care unit. Study Design Paired umbilical cord and admission blood CBC samples from well late preterm infants were compared using a two-sample t-test or analysis of variance with an unequal variance for differences in the hemoglobin, platelet counts, white blood cell, and absolute neutrophil counts. Results A total of 100 infants were enrolled in the study. The study included 46 females,

Pain management using patient controlled anaesthesia in adults post Nuss procedure: an analysis with respect to patient satisfaction.

BACKGROUND: The Nuss procedure for correction of pectus excavatum is commonly associated with severe postoperative pain. The belief that thoracic epidural offers the best pain control has recently come under scrutiny after several studies have shown patient controlled anaesthesia (PCA) to be just as effective. Nevertheless, centres who have shifted to an exclusively PCA regime exist in the minority. We have conducted a retrospective survey of all patients who underwent the Nuss procedure at the Heart Hospital, London, where all patients are managed exclusively using PCA. The study aims to asse

Introduction of a novel ultrasound-guided extrathoracic sub-paraspinal block for control of perioperative pain in Nuss procedure patients.

BACKGROUND: A safe and effective method of multilevel thoracic pain control remains an elusive goal in patients undergoing the Nuss procedure. The aim of our study was to develop a nonopioid centered approach using a novel regional technique as part of a quality improvement initiative. METHODS: The proposed ultrasound-guided technique positions multi-perforated soaker catheter deep to the paraspinal muscles from T2 to T11. The project was conducted in two phases. First, a cadaveric dissection was performed to establish the pathway of spread of local anesthetic in vivo. Second, a pilot double

Nonpharmacological techniques to reduce pain in preterm infants who receive heel-lance procedure: a randomized controlled trial.

INTRODUCTION: The heel-lance (HL) method for blood collection from the newborn is controversial for the pain it causes. This is the first randomized controlled trial on the management and reduction of pain using the music of Wolfgang Amadeus Mozart ("Sonata K. 448") in premature infants hospitalized in a neonatal intensive care unit (NICU). This study has compared nonpharmacological techniques with standard procedure for reducing pain during HL procedure. METHODS: Thirty-five premature infants were enrolled, each for 3 HL procedures, of which each was randomized to 1 of the 3 study arms. Arms

Infection risk from the use of continuous local-anesthetic infusion pain pumps in aesthetic and reconstructive abdominal procedures.

Postoperative pain control after abdominal procedures can be an area of significant concern. Continuous local-anesthetic infusion pain pumps have been clearly documented in recent literature to provide effective early postoperative pain control, in addition to other benefits. Our goal was to evaluate any increase in the risk of infection with the use of pain pumps with aesthetic and reconstructive abdominal procedures. A retrospective chart review evaluated 159 patients who underwent abdominoplasty (with or without suction-assisted lipectomy), panniculectomy, or a transverse rectus abdominis m

Persistent groin pain following a trans-obturator sling procedure for stress urinary incontinence: a diagnostic and therapeutic challenge.

Groin pain after a tension-free vaginal tape-obturator (TVT-O) procedure can occur but mostly disappears within 4 weeks. Persistent groin pain is extremely rare and there is a paucity of literature on how to diagnose and manage this adverse event. We present two cases with severe persistent groin pain after uncomplicated TVT-O, in which magnetic resonance imaging and electromyography did not reveal the cause. We concluded that the tape entrapped or cut through peripheral branches of the obturator nerve. We removed as much of the tape as possible in both cases. Removal partially relieved the pa

The efficacy of continuous fascia iliaca compartment block for pain management in burn patients undergoing skin grafting procedures.

UNLABELLED: Postoperative pain from split skin donor sites is often more intense than the pain at the grafted site. In this prospective, randomized, double-blind study we assessed the efficacy of a continuous fascia iliaca compartment block (FICB) in reducing the pain at the thigh donor site. Twenty patients, with a total burn surface area of 16% +/- 13% (mean +/- SD) were randomized 1:1 to receive either ropivacaine 0.2% or saline 0.9%. All patients received a general anesthesic followed by preincision continuous FICB with 40 mL of the randomized solution, then an infusion of 10 mL/h of eithe

[Development of a guideline for rehabilitation of patients with low back pain-- phase 2: analysis of data of the classification of therapeutic procedures].

Initiated by the Federal Insurance Institute for Salaried Employees (BfA, Bundesversicherungsanstalt für Angestellte), the project is aimed at developing an evidence-based guideline for rehabilitation of patients with low back pain (LBP). Guideline development will be based on a systematic review of the literature, an analysis of the treatment procedures currently employed in rehabilitation, inclusion of the patients' perspectives as well as consultation of experts' panels of clinically experienced physicians and therapists. Formulation of the guideline will then be carried out in a structured

Persistent foreign body sensation and pharyngeal pain due to retention of staples: an interesting sequelae of endoscopic stapling procedure.

The surgical treatment of a pharyngeal pouch with endoscopic stapling diverticulotomy is a relatively new concept. Long-term results and complications are yet to be fully studied. We describe a patient who developed persistent pharyngeal pain and foreign body sensation due to retention of a clump of staples at the cricopharyngeal sphincter. This complication has not been reported before. This case highlights the need for repeat endoscopy rather than a barium swallow X-ray if the patients are symptomatic after stapling procedures.

What are the outcomes of jaw function and pain reduction with advanced TMJ arthroscopy after failed lysis and lavage procedures?

When temporomandibular joint (TMJ) lysis and lavage procedures fail to treat intraarticular pain and dysfunction (IPD), the therapeutic effectiveness of advanced arthroscopy to treat persistent symptoms remain debatable. This is a retrospective study of 34 subjects at the University of Michigan from November 2020 to July 2023 with persistent IPD (Wilkes II-V diagnosis) after arthrocentesis or arthroscopic lysis and lavage, who then underwent advanced arthroscopy. Subjects had a mean age of 35.9±15.3 years and 33 (97%) were female with minimum 3-month follow-up. Outcomes were jaw function (meas

Virtual reality for pain and anxiety reduction during venous puncture procedures : A within-subject clinical crossover study.

BACKGROUND: Venous puncture is a routine procedure in extracorporeal photopheresis (ECP) and may be associated with pain and anxiety. Virtual reality (VR) distraction has been shown to reduce distress in pediatric and acute settings. This study investigated if immersive VR can reduce pain and anxiety during venous puncture in ECP patients. METHODS: A prospective, single-center clinical study with a within-subject crossover design was conducted at the University Hospital Innsbruck. A total of 21 adult patients underwent 2 consecutive ECP sessions, first with standard care and then with immersi

Beyond the Procedure: Non-Clinical Complaints and Gender Disparity Dominate One-Star Yelp Reviews of Pain Physicians.

BACKGROUND: Yelp.com allows patients to review their experiences. Few studies have focused on dissatisfaction in pain medicine. This study aims to characterize one-star Yelp reviews of anesthesiology-trained pain physicians to better understand the scope and nature of extreme patient dissatisfaction. RESULTS: Non-clinical issues comprised the most complaints (n = 927; 70%), while clinical concerns comprised 391 (30%). Office staff communication topped non-clinical complaints (n = 200). Unsatisfactory results (n = 90) topped clinical concerns. Eastern cities like Philadelphia, PA (53%) had the

Analysis of balance in adults with chronic axial spinal pain receiving medial branch block or radiofrequency ablation procedures.

BACKGROUND: Medial branch blocks (MBB) and radiofrequency ablations (RFA) are common diagnostic, prognostic, and interventional procedures performed for the management of cervical and lumbar spine facetogenic pain. Although these procedures have established utility as pain management strategies, it is unclear if the disruption in function of the medial branch nerve leads to patient balance impairment. OBJECTIVE: (1) To determine if there was a difference in postural balance, measured using a head mountable physiological vibration acceleration (Phybrata) sensor, between those pursuing cervical

Interventional Pain Procedures: A Narrative Review Focusing on Safety and Complications. Part 3 - Sympathetic and Ganglion-Targeted Techniques.

Interventional procedures that target the sympathetic nervous system and specific autonomic and sensory ganglia are widely used for refractory visceral, neuropathic, and cancer-related pain. They can provide substantial, often durable analgesia but carry a small yet clinically important risk of serious adverse events. This is the third part of our narrative-review series on the safety of interventional pain procedures, following Part 1 (spinal pain injections) and Part 2 (procedures for back pain), both published in the Journal of Pain Research. It is a structured but non-systematic narrative

Sham surgery as an alternative procedure for unexplained pain following transvenous implantable cardioverter defibrillator implantation: case report.

BACKGROUND: Persistent pain after implantable cardioverter-defibrillator (ICD) implantation is uncommon and often unexplained after ruling out device-related or musculoskeletal causes. Psychological factors can contribute to symptom persistence, complicating treatment. In rare cases, sham surgery has been considered to address psychogenic pain. This report explores its role in managing atypical post-ICD implantation pain. CASE SUMMARY: A 76-year-old female with hypertrophic cardiomyopathy, atrial fibrillation, and a history of ICD implantation presented with persistent left parasternal pain r
