Evaluation of the impact of motivational interviewing by a physiotherapist on the older patient's participation in the rehabilitation program and the care relationship: the MOTIV-Age qualitative study.

OBJECTIVES: Although motivational interviewing (MI) has shown benefits in younger patients with long-term conditions or older patients in hospital settings, we investigated the experienced impact of MI delivered by a physiotherapist (PT) on community-dwelling older patients' engagement in rehabilitation. DESIGN: Qualitative study following the descriptive-interpretive concept. SETTING AND PARTICIPANTS: The study was conducted in a suburban area of Paris. Participants were community-dwelling older adults aged over 75 years who consulted the same PT for musculoskeletal disorders and/or functio

Therapy discharge becomes part of the life story.

Despite great strides in the life-participation approach to aphasia therapy and the arduous work of several therapists to include patients in the decision-making and goal-setting process of therapy, the patient's direct perspective on the experience of treatment is seldom portrayed in academic journals. In the current article, the voices and different agendas of a stroke survivor with aphasia, her speech-language therapists, and an aspiring qualitative researcher are discussed by means of intertwined narratives, dialogues, and discourses. During an in-depth interview process, the researcher he

The community intensive therapy team: development and philosophy of a new service.

The Community Intensive Therapy Team (CITT) has been operating since 1998. It was developed to cater for the needs of patients with complex difficulties referred to a specialist Child and Adolescent Health Service (CAMHS) in South Wales, UK. The patients served by the CITT are comparable with patients who might be referred for admission to an inpatient unit and include patients with eating disorders, psychosis, affective disorders, adjustment disorders or repetitive self-harm. The theoretical model used is based on a biopsychosocial model which aims to empower and support family members. The p

Dental treatment of a child with congenital central hypoventilation syndrome.

BACKGROUND: Congenital Central Hypoventilation Syndrome (CCHS) is a rare condition combining respiratory hypoventilation with symptoms of autonomic dysregulation. Management of patients requires both medical and dental expertise to achieve a successful outcome. The aim of this paper is to present the dental management of a child diagnosed with CCHS, without pharmacological measures and in cooperation with medical expertise. CASE REPORT: A 7-year-old girl was referred to a private dental practice with the chief complaints being pain and poor aesthetics. The child had been diagnosed with CCHS s

"Who's Caring for Us?": Understanding and Addressing the Effects of Emotional Labor on Home Health Aides' Well-being.

BACKGROUND AND OBJECTIVES: Interventions to strengthen the home care workforce focus on workers' economic and physical well-being, without acknowledging the caring labor affecting emotional well-being. Our study examined workers' perceptions of the emotional effects of caring work, coping mechanisms, and desired support. RESEARCH DESIGN AND METHODS: We conducted 4 worker focus groups (n = 27). Moderators cross-checked codes and themes, and aides provided input through report-backs. RESULTS: Building close, trusting relationships with clients was central to aides' emotional well-being. Specialist hospital treatment and care as reported by children with intellectual disabilities and a cleft lip and/or palate, their parents and healthcare professionals.

BACKGROUND: Research into hospital treatment and care of children with intellectual disabilities is extremely limited, but available literature points to difficulties. Some children have a co-occurring condition alongside an intellectual disability which requires ongoing treatment, such as a cleft lip/palate. To date, their experiences remain untapped. METHOD: Semi-structured interviews with 23 participants; five children with intellectual disabilities (aged 11-16), their parents (n = 9) and nine healthcare professionals working in cleft care. Thematic analysis determined patterns across the

"Those eyes that look at you:" somatic modes of care in professional encounters with amyotrophic lateral sclerosis patients.

In the advanced stages of amyotrophic lateral sclerosis (ALS), individuals experience a gradual and irreversible loss of speech and voluntary movement, while cognitive and emotional capacities often remain largely preserved. ALS frequently culminates in the locked-in state (LIS), where subjectivity endures despite an almost complete breakdown of expressive capacity. This article examines how professional caregivers sustain relational engagement and recognition under such conditions. The analysis draws on eleven qualitative interviews with social workers, psychologists, occupational therapists,

Van Gogh, Cowper, and Unfinished Suffering: Narrative Hope in Clinical Chaplaincy.

This clinical pastoral reflection explores how historical and artistic narratives may support meaning-making in chaplaincy encounters. During a pastoral visit with a patient experiencing advanced cancer and depression, the lives of Vincent van Gogh and William Cowper became interpretive resources through which suffering was reframed as spiritually continuous rather than spiritually disqualifying. The encounter illustrates how narrative engagement can foster renewed hope while suffering remains unresolved within clinical pastoral care.

Lessons Learned: A Straight Therapist's Perspective on Treating a Gay Woman's Romantic Breakup.

This article presents the retrospective insights of a straight female psychologist working with a gay female patient through the emotional aftermath of a romantic breakup. Central to this case study is the therapist's journey toward understanding the culturally specific dynamics that shape the breakup experiences of non-heterosexual individuals differently from those of heterosexual individuals. In addition to deepening her understanding of important therapeutic considerations unique to a non-heterosexual breakup, the author explores how certain cultural norms within the queer community, inclu

"I'm Not Here to Teach You How to Be Gay:" LGBTQ+ Client Experiences of Othering in Emotionally Focused Couple Therapy.

Despite the increasing emphasis on addressing the intersections of client and therapist identity to improve therapeutic outcomes, therapy clients holding marginalized identities continue to experience othering. Othering is a process that engenders marginalization and inequality based on preconceived group identity, involves the hegemonic construction of belonging and not belonging, and often entails both stereotyping and racialization. This paper reports on the experience of lesbian, gay, bisexual, transgender, queer/questioning plus (LGBTQ+) emotionally focused couple therapy (EFCT) clients'

Presence before alliance? Longitudinal evidence from psychotherapy trainees in China.

This study examined the longitudinal associations and temporal precedence between therapeutic presence and working alliance in psychotherapy. The data set comprised 14,498 sessions conducted by 208 master's-level trainee therapists with 1,587 clients in China. After each session, both therapists and clients completed standardized measures of therapeutic presence and working alliance. Results from random-intercept cross-lagged panel modeling indicated that, at the between-person level, therapist-client dyads that reported higher average therapeutic presence also tended to report higher average

Communicative practices used by health and social care professionals, older adults and informal carers during advance care planning conversations: systematic review of qualitative evidence and narrative synthesis.

BACKGROUND: While advance care planning (ACP) is linked with clinical and psychosocial benefits for older adults and their informal carers, uptake is low. Inadequate communication skills amongst clinicians has been identified as a key barrier to effective ACP. While existing literature reviews have focussed on hypothetical preferences, barriers and facilitators, none have examined direct experience of ACP communication with this population. The aim of this review was to identify and synthesise qualitative evidence regarding the communicative practices used to support older adults, informal car

Prevalence of Violence Perpetrated by Healthcare Workers in Long-Term Care: A Systematic Review and Meta-Analysis.

This systematic review and meta-analysis aimed to determine the prevalence of violence perpetrated by healthcare workers (HCWs) against patients in long-term care (LTC). For this purpose, five relevant databases were searched. Two reviewers extracted data from the included articles independently and assessed their quality. Overall and subgroup random-effects pooled prevalence meta-analyses were performed. A series of meta-analyses stratified by study quality were also performed due to high heterogeneity. Nineteen articles were included, physical restraint (22%; CI: 15-29), verbal abuse (22%; C

The evolution of patients' concept of the alliance and its relation to outcome: A dynamic latent-class structural equation modeling approach.

The working alliance (WA) has been widely identified as the key concept for psychotherapy and allied health care services. The WA, measured at different phases of diverse kinds of therapies, has been shown to robustly predict posttreatment outcomes. But the way the clients' conceptualization of the alliance evolves overtime, and the relation between this kind of conceptual change and subsequent symptom improvements, has not been investigated. Dynamic Latent Class Structural Equation Models (DLC-SEM) were applied to data drawn from two randomized clinical trials of cognitive-behavioral therapy

The alliance-outcome association in the treatment of adolescent depression.

A growing body of research has consistently demonstrated a relationship between alliance and treatment outcomes in youth psychotherapy. However, previous research often suffered methodological issues that prevented detailed investigation of temporal relationships between alliance and symptomatology. The current study explored the directions of effect between alliance and outcome by examining the associations between early alliance and subsequent outcome while controlling for patients' baseline severity and prior symptom change. It also examined potential moderators of this association. Data we

Central Sensitization Symptom Severity and Patient-Provider Relationships in a Community Setting.

INTRODUCTION: Central sensitization syndromes (CSS) comprise an overlapping group of clinical conditions with the core feature of "pain arising as a direct consequence of a lesion or disease affecting the somatosensory system." Patients with CSS are known to have challenging interactions with healthcare providers contributing to psychological distress and increased healthcare utilization. CSS symptom severity has been associated with psychologic comorbidities, but little is known about how symptom severity relates to provider interactions. METHODS: We performed a cross-sectional survey among

Experience and knowledge of Lyme disease: A scoping review of patient-provider communication.

There has been limited research on patient-provider communication dynamics regarding Lyme disease (LD) diagnosis and treatment. Evidence suggests communication in the clinical encounter improves when both patient and healthcare provider (HCP) have concordant orientations (or beliefs) on discussed topics, resulting in higher patient satisfaction and care outcomes. The purpose of this scoping review was to characterize and summarize current research findings on patient and provider knowledge and experiences regarding LD - two factors that may influence the orientation of both patients and provid

The role of therapeutic alliance in therapy for adults with problematic aggression and associated disorders.

The nature and quality of the relationship between therapist and client in psychotherapy, known as therapeutic alliance, have been proposed as one of the most important factors for successful treatment outcome, which has been has robustly supported across many types of treatment, populations, raters of alliance (i.e., client, therapist, or observer), and alliance measures. However, most research on alliance and treatment outcome has been conducted in children and adults with internalizing problems (e.g., mood and anxiety disorders) or children with externalizing problems (e.g., conduct disorde

Collaboration in cognitive-behavioral therapy.

Collaborative empiricism, which involves a systemic process of therapist and patient working together to establish common goals in treatment, has been found to be one of the primary change agents in cognitive-behavioral therapy (CBT). This article focuses on the development of a therapeutic relationship and implementation of collaborative empiricism along with the elements that lead to success in treatment. This method is used to uncover patients' automatic thoughts and underlying beliefs in treating an array of emotional and behavioral disorders. The role of the therapist is discussed in deve
