Psychiatrists frequently see patients in their practices who struggle with issues of chronic physical pain. This can present diagnostic and therapeutic dilemmas. These patients require an approach that allows them to talk about their pain and feel supported while simultaneously being nudged to develop a meaningful life alongside their pain. This article addresses an approach to accomplish this difficult balancing act over the course of time and includes case examples.

OBJECTIVES: This study has been designed to investigate the effectivity of psychodynamic therapies.
METHODS: In a naturalistic prospective longitudinal study, 117 patients are compared with 116 untreated probands.
RESULTS: The improvement of the general psychosocial impairment of patients of clinical relevance and significance. The effective magnitude of change is high. Untreated patients show on the average no remission of impaired well-being and symptoms six months later.

OBJECTIVE: The authors tested the efficacy of individual psychotherapy in the rehabilitation counseling of psychiatrically symptomatic opiate-dependent patients during methadone maintenance treatment in community programs.

The authors conducted a controlled, prospective 4-month study of 10 male inpatients with chronic schizophrenia and polydipsia. The five men who were treated with group psychotherapy drank significantly less fluid than the five men not given this therapy. The theoretical basis, the therapeutical approach and the methods of individual psychotherapy and of family therapy are presented. Possibilities and limitations, advantages and disadvantages of the two forms of therapy are discussed and illustrated by means of case histories. Careful clinical formulation describing the dimensions of core neurotic conflicts is integral to the practice of psychodynamic psychotherapy. Therefore, a method for reliable and valid formulation is a necessary precursor for the empirical investigation of the impact of such therapy.
