Public health in the twentieth century: advances and challenges.

Substantial advances in public health methods, practice, and the health of the public have occurred in the twentieth century. Some of the co … estments promise greater efficiency and effectiveness for public health. An increasingly fragmented public health infrastructure must confront unprecedented challenges including dramatic global population growth, increased aging of the population associated with enhanced longevity, and possible irreversible changes in key environmental health determinants.

Intersecting risks: A longitudinal analysis of climate exposure, social vulnerability, and mental health in the Texas Gulf Coast.

This study investigates the evolving intersection of social vulnerability, climate exposure, and mental health distress in Harris County, Texas, from 2014 to 2022, with projections through 2030. Using census tract-level data and integrating indicators from the CDC Social Vulnerability Index, climate exposure metrics, and mental health outcomes, we developed predictive models to forecast future trends. Results indicate a significant rise in overall social vulnerability-from a score of 8.16 in 2022 to 8.87 by 2030, driven by increasing poverty, extreme heat, flooding, and deteriorating mental he

Design of health care technologies for the developing world.

Approximately 20 years ago, the international community embarked on a project to bring health care to everyone by the year 2000 featuring, among other things, technologies that were known to be effective and economical. It was largely a failure. In fact, health care deteriorated in many of the target nations. Problems such as public mistrust, lack of spare parts, lack of required consumables, lack of reliable power and water, lack of public infrastructure such as roads, lack of technical expertise, and other problems plague health care technology in the developing world. Biomedical engineers a

The people's role in U.S. national health security: past, present, and future.

Over the past decade, assumptions have been made and unmade about what officials can expect of average people confronting a bioterrorist attack or other major health incident. The reframing of the public in national discourse and doctrine from a panic-stricken mob to a band of hearty survivors is a positive development and more realistic in terms of the empirical record. So, too, is the realization that citizen contributions to national health security encompass not only individual preparedness and volunteerism but also mutual aid and collective deliberation of the tough choices posed by healt

Can trust in healthcare be restored after armed violence? Evidence from a randomized field experiment in north-eastern Nigeria.

Armed violence undermines public health not only by destroying infrastructure but also by eroding trust in government institutions, including the health system. Prior research shows that victimized individuals often withdraw from public health services, with particularly adverse consequences for child health. Whether such distrust can be restored, however, remains an open question. We address this question in northeastern Nigeria, a region heavily affected by armed violence, through a randomized field experiment embedded in a representative household survey of caregivers of young children (n =

Advancing Health Policy and Outcomes for People With Intellectual or Developmental Disabilities: A Community-Led Agenda.

IMPORTANCE: At least 10 million people in the United States have an intellectual and/or developmental disability (IDD). People with IDD experience considerably higher rates of poor overall health, chronic conditions including diabetes, mental health challenges, maternal mortality, and preventable deaths. This Special Communication proposes national goals based on a community-led consensus model that advances priority health outcomes for people with IDD and their caregivers/partners and identifies critical policy opportunities and challenges in achieving these goals. A community-led consensus a

Global lessons from Nigeria's ebolavirus control strategy.

The current Ebola virus disease outbreak challenged medical and public health systems in West Africa. In Nigeria, the existing infrastructure of the polio surveillance system was leveraged rapidly to contain the spread of Ebola virus. We highlight important lessons learnt from the successful implementation of Ebola virus disease surveillance strategies, which should be amplified further to prepare the ground for successful vaccination programs. Close collaboration between national and international stakeholders as well as public/private partnerships will be instrumental in future Ebola virus i

Epidemiology and Management of the 2013-16 West African Ebola Outbreak.

The 2013-16 West African Ebola outbreak is the largest, most geographically dispersed, and deadliest on record, with 28,616 suspected cases and 11,310 deaths recorded to date in Guinea, Liberia, and Sierra Leone. We provide a review of the epidemiology and management of the 2013-16 Ebola outbreak in West Africa aimed at stimulating reflection on lessons learned that may improve the response to the next international health crisis caused by a pathogen that emerges in a region of the world with a severely limited health care infrastructure. Surveillance efforts employing rapid and effective poin

Public transit infrastructure and heat perceptions in hot and dry climates.

Many cities aim to progress toward their sustainability and public health goals by increasing use of their public transit systems. However, without adequate protective infrastructure that provides thermally comfortable conditions for public transit riders, it can be challenging to reach these goals in hot climates. We took micrometeorological measurements and surveyed riders about their perceptions of heat and heat-coping behaviors at bus stops with a variety of design attributes in Phoenix, AZ, USA, during the summer of 2018. We identified the design attributes and coping behaviors that made

Cholera resurges in Zambia: Challenges and future directions.

Cholera remains a major global health challenge, disproportionately affecting low-income countries with inadequate water, sanitation, and hygiene (WASH) infrastructure. Zambia, historically prone to outbreaks, experienced a severe resurgence in 2023-2024, with over 10,887 cases and 432 deaths. The contributing factors include poor WASH access, systemic health care weaknesses, stigma, and logistical barriers. Only 32% of households have basic water services, and health care worker shortages persist, especially in densely populated areas. Misinformation and cultural practices hinder public healt

It's Your Game…Keep It Real: can innovative public health prevention research thrive within a comparative effectiveness research framework?

The federal comparative effectiveness research (CER) initiative is designed to evaluate best practices in health care settings where they can be disseminated for immediate benefit to patients. The CER strategic framework comprises four categories (research, human and scientific capital, data infrastructure, and dissemination) with three crosscutting themes (conditions, patient populations, and types of intervention). The challenge for the field of public health has been accommodating the CER framework within prevention research. Applying a medicine-based, research-to-practice CER approach to p

India's Stray Dogs as a Public Health Risk: An Autopsy Case Report and Comprehensive Review.

A major public health issue is stray dogs, particularly in urban and peri-urban areas. Particularly for small children, who are more vulnerable to serious injury and deadly assaults, stray dogs can present serious risks. While playing outside his house, a 5-year-old kid was fatally bitten by a stray dog. Severe head, neck, and back injuries, including deep cuts, exposed occipital bone, abrasions, and bruises, were discovered during an autopsy. The internal findings were a jugular vein tear, laryngeal perforations, vertebral avulsion, and neck muscle lacerations. The growing number of stray dog

Viral Hepatitis and a Hospital Infrastructure in Ruins in Cameroon.

Ethnographic material dealing with the contemporary viral hepatitis B and C epidemics in Cameroon provide a window onto the acute constraints and shortcomings of hospital care for patients, families, and health care workers. Although viral hepatitis has long been an invisible epidemic in international and global public health regimes, in Cameroon, it is diagnosed, made visible, and felt as a financially daunting and feared disease. Building on Ann Stoler's framework of imperial ruins, I consider hepatitis as an iatrogenic disease, emerging from scarce and unsound hospital infrastructures, such

Measures for the prevention and control of Taenia solium taeniosis and cysticercosis.

Taeniosis and cysticercosis due to Taenia solium are public health problems in many developing countries. Many studies of this parasitic zoonosis have focused on clinical features, diagnosis, treatment, surveillance, epidemiology and risk factors analysis. More recently projects on community and mass intervention strategies had been conducted in several rural areas worldwide focused on pig vaccination, pig cysticercosis treatment, human mass treatment, infrastructure development, as well as health education campaigns. Their advantages, disadvantages and public health impact have been published

Ebola virus disease in the modern era: pathogenesis, therapeutic advances, vaccine strategies, and global health preparedness.

Ebola virus disease (EVD) remains one of the most severe and complex viral hemorrhagic fevers confronting global public health systems. Since its first recognized emergence in 1976, recurrent outbreaks caused primarily by Zaire ebolavirus have demonstrated the extraordinary epidemic potential of filoviruses, particularly in regions with fragile healthcare infrastructure. The unprecedented West African epidemic of 2013-2016 fundamentally altered scientific understanding of Ebola virus transmission, pathogenesis, clinical progression, and outbreak management. Subsequent outbreaks in the Democrat

Challenges for programmatic implementation of killed whole cell oral cholera vaccines for prevention and control of cholera: a meta-opinion.

INTRODUCTION: Cholera remains a public health threat. The development of safe, effective, easy-to-administer, heat-stable, and cheap killed whole cell oral cholera vaccines (OCVs) has provided an additional tool to counter cholera. In this meta-opinion, we review the challenges of delivering OCVs through the existing public health infrastructure in vulnerable areas. AREAS COVERED: We provide an overview of the available vaccines against cholera, the existing evidence about the effectiveness of a two-dose as well as a single-dose OCV strategy. We also highlight the experience from the public h…
