Congenital double pylorus is extremely rare; only 2 cases have been found in the English and French literature. Acquired double pylorus occurs more often; up to 1982, 66 cases had been reported. During 6 810 consecutive barium meal examinations over a period of 2 years, we diagnosed the condition in 5 patients. Two came to operation, at which the diagnosis was confirmed. The effects of solid non-nutritive diet on pylorus-ligation ulcers were tested. An experimental group of 16 rats received a bulky mixture of silica and methylcellulose during 48 hr prior to ligation. Control rats were food deprived for the same period of time. A multivariate analysis of variance was applied to nine measures of the study. It was found that rumenal ulceration and total acidity were lower in experimental animals than in controls. This finding confirms a previous observation indicating that the physical property of diet has antiulcerogenic effects.

A 67-year-old patient was admitted to our hospital with lack of appetite and upper gastrointestinal bleeding. The immediately performed emergency gastroscopy showed a giant subepithelial tumour with ulceration located in the antrum. Emerging from the tumourous ulcer was a peg-shaped outgrowth reaching through the pylorus down to the duodenum causing gastric outlet obstruction. Based on this unusual presentation of a gastric giant lipoma we have reviewed the literature concerning differential diagnosis and treatment options.

There has been a considerable debate about whether pylorus preservation significantly detracts from the radicality in a palliative procedure where a conventional Whipple operation would have been curative. We know now, that the extending radicality of the Whipple operation does not improve the long-term survival rates.
