Interventional radiology and fluoroscopy utilisation trends in Ireland: A multi-centre analysis (2018-2023).

INTRODUCTION: This study addressed a significant knowledge gap regarding contemporary utilisation trends of fluoroscopy (FL) and interventional radiology (IR) in Ireland. Specifically, it aimed to (1) evaluate annual trends in adult FL and IR usage between 2018 and 2023, (2) analyse age-stratified utilisation patterns, and (3) identify the most frequently performed procedures. METHODS: A retrospective, multi-centre analysis used data from the National Integrated Medical Imaging System, incorporating ten public hospitals across five Health Service Executive groups in Ireland. The dataset compr

[Interventional Radiology for Intra-Abdominal Abscess after Gastrectomy].

Approximately 20% of patients develop some complications after gastrectomy. These complications should be treated appropriately to achieve a positive outcome. The records of 6 patients with postoperative intra-abdominal abscesses treated with interventional radiology (IVR) were analyzed. The cause of abscess was anastomotic leakage in 4 patients and contaminated surgery after gastric perforation in 2 patients. Intra-abdominal abscesses were detected on postoperative day 12 (median), and an IVR-guided drainage tube was inserted with a median interval of 1 day. The drainage tube was kept in plac

Diagnostic strategies for mediastinal tumors and cysts.

Many histologically different tumors and cysts that affect people of all ages arise from the multiple anatomic structures present in the mediastinum. The number of diagnostic possibilities can be narrowed by considering the patient's age, tumor location, the presence or absence of symptoms and signs, the association of a specific systemic disease, radiographic findings, and biochemical markers. Pathologic diagnosis is often required to confirm a presumed diagnosis and to select the optimal treatment modality. A variety of biopsy techniques for obtaining tissue from the mediastinum have been de

[Splenic abscesses. Their percutaneous treatment and the role of the interventional radiologist].

Percutaneous abscess drainage is not as common in the spleen as in other anatomical sites, probably because of an uncontrollable fear of bleeding. Five cases of intrasplenic abscess drainage are presented. A double-way 12/14-F vanSonnenberg catheter was percutaneously inserted under CT guidance in four patients and under US guidance in one patient. Orthogonal scout CT views were useful to check the correct positioning of the drainage. In three patients the maneuver was successful, with no recurrence at follow-up at 12, 16 and 24 months, respectively. In one patient with a splenic abscess due t

Femoral and Brachial Artery Cut-Down for Vascular Access in Interventional Procedures.

Femoral and brachial artery cut-downs are classic surgical methods for establishing vascular access in interventional procedures. Although largely replaced by less invasive percutaneous techniques and vascular closure devices, these open approaches remain essential when anatomy is challenging or large devices are needed. This paper outlines the indications, preoperative assessment, and step-by-step surgical techniques for safely exposing the femoral and brachial arteries. Direct visualization during cut-down facilitates reliable hemostasis and mitigates access-related complications, such as ps

Laser positioning versus conventional CT-Guided lung biopsy: A systematic review and meta-analysis of clinical outcomes.

INTRODUCTION: CT-guided percutaneous lung biopsy remains essential for diagnosing pulmonary nodules but is frequently complicated by pneumothorax, hemorrhage, and hemoptysis. Laser-assisted CT-guided biopsy (LACT) has been proposed to enhance procedural precision and safety. This meta-analysis compared the efficacy and safety of LACT versus conventional CT-guided (CCT) lung biopsies. METHODS: A comprehensive search of six electronic databases was performed to identify studies comparing LACT and CCT lung biopsies. Statistical analyses were performed using RevMan 5.4.1, and P < 0.05 was conside

Safety and efficacy of dynamic catheter-directed cerebral digital subtraction angiography for diagnosis of bowhunter syndrome spectrum disorders: A systematic review of the literature.

IntroductionDynamic catheter-directed cerebral digital subtraction angiography (dcDSA) is the gold standard for diagnosing dynamic vascular occlusion syndromes such as bowhunter syndrome (BHS). Nonetheless, concerns about its safety exist and no standardized protocols have been published to date.MethodsWe describe our methodology and insights regarding the use of dcDSA in patients with BHS. We also perform a systematic literature review to identify cases of typical and atypical presentations of BHS wherein dcDSA was utilized and report on any procedural complications related to dcDSA.ResultsOu

Interventional cardiology and X-ray exposure of the head: overview of clinical evidence and practical implications.

Interventional cardiologists are significantly exposed to X- rays and no dose of radiation may be considered well tolerated or harmless. Leaded aprons protect the trunk and the thyroid gland, leaded glasses protect the eyes. The operator's legs, arms, neck and head are, instead, not fully protected. In fact, the operator's brain remains the closest part to the primary X-ray beam and scatter in most interventional procedures and specifically the physician's front head is the most exposed region during device implantation performed at the patient's side. After the initial description of cases of

Uterine Artery Embolization for Uterine Fibroids: A Closer Look at Misperceptions and Challenges.

Uterine artery embolization (UAE) has been shown to be a safe and effective treatment for symptomatic uterine fibroids, with over 25 years of supporting data. Although UAE is a well-established treatment option, several misperceptions exist that may limit the number of patients who are considered candidates for UAE. There are also challenges that may affect the ability of interventional radiologists to effectively treat some patients and offer the best possible experience. This article will discuss these misperceptions and challenges, which represent opportunities for further growth and innova

Novel Therapeutic Strategy Using Interventional Radiology (IVR) for Hepatitis C Virus (HCV)-Related Decompensated Liver Cirrhosis: A Case Report.

BACKGROUND The appearance of direct acting antivirals (DAAs) has produced a major paradigm shift in hepatitis C virus (HCV) infection treatment, and virus elimination has become possible in most patients. Improvement of the model for end-stage liver disease (MELD) score by elimination of HCV has been reported, but for decompensated liver cirrhosis, it is also important to overcome various complications before antiviral treatment. CASE REPORT A 72-year-old male, who had been treated for HCV-related liver cirrhosis was referred to our hospital for treatment of refractory hepatic encephalopathy.

Spontaneous internal hemorrhage of a giant hepatic hemangioma: A case report.

RATIONALE: Hepatic hemangioma, a benign liver tumor, can rarely spontaneously rupture and hemorrhage, which is then associated with significant mortality. The diagnosis of internal hemorrhage is challenging and the management is disputed. PATIENT CONCERNS: We describe the case of a 52-year-old female patient with chief complaints of fever and anemia, with no history of recent trauma. DIAGNOSES: Ultrasound suggested the possibility of a liver abscess and computed tomography (CT) examination revealed a giant space occupying lesion (SOL) (approximately 16 cm) in the right hepatic lobe, indicati
