A proposed surgical strategy for complex articular distal radius fractures: The Marseille approach for radius salvage (MARS).

INTRODUCTION: Complex intra-articular distal radius fractures with severe comminution or fragment rotation may be difficult to reduce using conventional fixation. We describe a dorsal salvage strategy, the Marseille Approach for Radius Salvage (MARS), enabling direct articular visualization in selected cases. HYPOTHESIS: The MARS strategy allows reliable reduction and acceptable short-term functional outcomes in fractures not amenable to standard fixation. MATERIALS AND METHODS: Single-center prospective case series (May 2021-October 2025). CT assessed reduction quality (step-off ≤ 2 mm). Fu…

Conservative treatment of distal ulna metaphyseal fractures associated with distal radius fractures in elderly people.

INTRODUCTION: Although volar locking plates (VLPs) have been exclusively used for unstable distal radius fractures (DRFs), the optimal management of distal ulna fractures (DUFs) remains controversial. Internal fixation using a plate for DUFs might be challenging because of the presence of osteoporotic bone and fragile bone fragments in elderly patients. HYPOTHESIS: We hypothesized that a strategy including VLP fixation for DRFs and non-intervention for DUFs with early wrist mobilization would provide encouraging results in elderly patients. MATERIALS AND METHODS: Eighteen patients (17 women …

Functional outcomes of distal radius fractures with and without ulnar styloid fractures: a meta-analysis.

UNLABELLED: The aim of this meta-analysis was to compare the functional outcomes of patients with a distal radius fracture with and without a concomitant fracture of the ulnar styloid process. A systematic literature search was performed to identify all studies comparing patients with a distal radius fracture with and without an ulnar styloid process fracture. The initial search revealed 511 articles, of which 12 articles with a total of 2243 patients were included; 1196 patients with and 1047 patients without an ulnar styloid process fracture. A statistically significant mean difference of 3.

Incidence of subsequent osteoporotic fractures after distal radius fractures and mortality of the subsequent distal radius fractures: a retrospective analysis of claims data of the Korea National Health Insurance Service.

UNLABELLED: A better understanding of the features of subsequent fractures after distal radius fracture (DRF) is important for the prevention of further osteoporotic fractures. This study found that the cumulative incidence of subsequent osteoporotic fractures in South Korea increased over time and that the mortality rates of subsequent DRFs were lower than those of first-time DRFs. INTRODUCTION: We examined the incidence of osteoporotic fractures following distal radius fractures (DRFs) and the mortality rate after subsequent DRFs using claims data from the Korea National Health Insurance (K…

Single-handed volar locking plate fixation for distal radius fractures.

INTRODUCTION: Dorsally displaced distal radius fractures are among the most common orthopedic injuries, requiring surgical techniques that ensure optimal functional recovery while minimizing complications. The traditional method of volar locking plate osteosynthesis, although considered the gold standard, often requires a multidisciplinary surgical team. In the context of staff shortages and the need to optimize resources, this work presents a technique that can be performed by a single surgeon, particularly suited for young practitioners. The objective of this study is to demonstrate that dis…

Scaffold Fixation of Multi-Fragmented Intra-Articular Distal Radius Fractures.

OBJECTIVE: Volar locking plates (VLPs) are commonly used to treat displaced intra-articular distal radius fractures (DRFs), but multi-fragmented fractures may require additional dorsal fixation. This study assessed the outcomes of a technique combining VLP and a dorsal bone clamp, described here as "scaffold fixation," and evaluated whether dorsal ulnar corner (DUC) fixation is essential for radiocarpal alignment. METHODS: In this retrospective cross-sectional study of the ICUC database, 87 patients with DRF with preoperative CT-identified DUC fragments were included. The range of motion was …

Bariatric surgery and distal radius fractures, a population-based study.

INTRODUCTION: Obesity is a growing global concern. Bariatric surgery (BS) is the only intervention which leads to significant and long-lasting weight reduction. However, it has been associated with an increased risk of fracture. We aimed to investigate the association between BS and distal radius fractures (DRF). METHODS: A retrospective cohort study of patients with obesity who underwent BS was conducted. The incidence of DRF and the non-osteoporotic scaphoid fracture in the six-years pre-and post-BS was compared. Sub-analysis for surgical type, weight loss, nutritional supplements, and trea…

Cartilage reconstruction using Chondrofiller in intra-articular distal radius fractures.

INTRODUCTION: Intra-articular distal radius fractures (DRF) frequently result in post-traumatic cartilage defects, which can lead to early osteoarthritis, particularly when residual step-offs or gaps ≥ 2 mm remain after fracture reduction. While cartilage repair techniques are well established in large joints, options for small joints like the wrist remain limited. Chondrofiller liquid, a cell-free collagen type I hydrogel, has demonstrated regenerative potential in larger joints; however, its application in the wrist has not yet been studied. METHODS: In this prospective clinical study, 59 p…

Arthroscopic Scaffold Fixation Technique for Intra-articular Distal Radius Fractures.

Arthroscopic-assisted reduction and internal fixation of displaced intra-articular fractures of the distal radius is a useful adjunct in the management of these complex injuries. This paper describes an arthroscopic technique to reduce and internally fix the anterior endplate articular surface with subchondral wires to create a two-part fracture. The wire configuration resembles a scaffold so that the distal fracture component can then be indirectly reduced using a volar locking plate to correct the anteroposterior and radioulnar inclination of the articular surface of the distal fragment.

Single versus double percutaneous pinning of pediatric distal radius fractures.

The aim of this study was to compare early clinical and radiographic results of single- versus double-pin fixation of unstable pediatric distal radius fractures. A total of 103 consecutive closed distal radius fractures treated with either single or double percutaneous pinning at a tertiary level I pediatric hospital were analyzed. All patients had open physes and had fractures that failed initial closed reduction and casting. Postoperative fracture displacement was assessed by measuring the difference in angulation of the radius in the anteroposterior and lateral views from intraoperative flu…

Outcomes of Distal Radius Fractures in Solid Organ Transplant Recipients.

PURPOSE: Distal radius fractures (DRFs) are one of the most common conditions that musculoskeletal providers treat. As the frequency of solid organ transplants (SOT) increases, these providers are often called upon to manage DRFs in these patients. These patients are at increased risk for osteopenia and osteoporosis, given the altered bone metabolism after SOT and frequent use of glucocorticoid and immunosuppressive medications. This study aimed to examine both surgical and nonsurgical treatment outcomes of DRFs in the SOT population and the prevalence of decreased bone mineral density. METHO…

The Pitfalls of Difficult Distal Radius Fractures and Provisional Reduction.

Distal radius fractures are some of the most common injuries encountered in orthopedics and require careful consideration when determining the appropriate treatment options. These fractures can be difficult injuries to treat surgically based on a large variability of fracture patterns, bone quality, and anatomy. It is important to understand the potential pitfalls associated with the treatment of difficult distal radius fractures to prevent avoidable complications. Some of these pitfalls include but are not limited to appropriate surgical exposure and soft tissue handling, provisional reductio…

Management of Comminuted Distal Radius Fractures: A Critical Analysis Review.

Distal radius fractures occur in a bimodal age and sex distribution. Their incidence continues to rise with an increasingly active aging population. The current American Academy of Orthopaedic Surgeons guidelines for operative intervention are radial shortening of >3 mm, dorsal tilt of >10[degrees], or intra-articular displacement or step-off of >2 mm. The method of reduction and fixation should be selected on a case-by-case basis with a focus on patient factors, fracture behavior, surgical proficiency, and functional outcomes.

Minimal invasive plating of distal radius fractures. A safe procedure?

BACKGROUND: Minimal invasive plate osteosynthesis (MIPO) with preservation of the pronator quadratus (PQ) muscle represents a new technique for stabilization of distal radius fractures. However, the complex anatomy of the distal radius metaphysis requires implants with features that address all morphologic specifics of this area to avoid complications, which are still reported with this technique. It was the aim of our anatomic investigation to evaluate the feasibility of plate insertion via a minimal transverse approach as well as the risk of soft- tissues compromise with the use of an implan…
