Obstructive sleep apnea and other sleep-related syndromes.

Obstructive sleep apnea syndrome (OSAS) is a common disorder characterized by repetitive episodes of breathing cessation due to complete or partial collapse of the upper airway therefore affecting ventilation. It is quite common, with a prevalence of about 2-4%, has a strong genetic component, and creates a proinflammatory state with elevated TNFα and other cytokines. If untreated, OSA can lead to significant neurological problems that include stroke, cognitive decline, depression, headaches, peripheral neuropathy, and nonarteritic ischemic optic neuropathy (NAION). Treatment reverses some of

[New knowledge on iron metabolism and its disorders].

Iron is an essential element to all living organisms. It is a component of many proteins with important functions in physiological processes such as oxygen transport, respiration, DNA synthesis, cell cycle regulation and many others. Free iron is highly reactive and its excess can lead to tissue and organ damage. Intestinal absorption of iron is precisely regulated because there is no excretory mechanism for excessive iron. Improved methodology led to the identification of many genes and proteins involved in the iron metabolism and to the understanding of basic processes of iron intake, transp

[Effect of the respiratory phases on the right jugular vein pulsation curve and pulmonary hemodynamics in mitral valve stenosis].

In patients with mitral stenosis the main and early changes in the pulsation curve of the right jugular vein consist in an elevation of the y-collapse. The height of the y-collapse correlates with the degree of venous congestion in the lungs, i.e. a parallelism exists between the signs of venous congestion in the pulmonary and general circulation systems. The degree of venous congestion in the lungs of patients with mitral stenosis varies with the phases of respiration. An elevation of the v-collapse in mitral stenosis cases is observed at early stages of the disease, simultaneously with the d

GPAM mediates mitochondrial dysfunction and the progression of alcoholic liver disease through lipid remodeling.

Lipid metabolic disorders and mitochondrial dysfunction are key and core pathological processes that contribute to the progression of alcoholic liver disease (ALD). However, the effects of lipid metabolism disorders on mitochondrial dysfunction in patients with ALD remain unknown. Here, we demonstrated that glycerol-3-phosphateacyltransferase (GPAM) expression was down-regulated in patients with ALD and associated with ALD progression. Dysregulation of GPAM, a triglyceride synthetase, reshaped lipid metabolism by increasing the levels of toxic lipids such as ceramide and lysophosphatidylcholin

Autonomic dysfunction in movement disorders.

Dysfunction of the autonomic nervous system is an under-recognised but important aspect of the aetiological and clinical manifestation of primary degenerative dysautonomias such as multiple system atrophy (MSA) and Parkinson's disease (PD). Although the clinical presentation of dysautonomia in these two disorders may overlap, yet pathological and in vivo imaging studies suggest considerable differences. Functional imaging studies suggest that selective cardiac sympathetic denervation may occur early in PD but not in other parkinsonian syndromes. The clinical implication of this apparently dise

[Heart failure].

It seems that the causes of the insomnia are dyspnea and an orthopnea in the heart failure patient. But, only such a fit is not the cause of the insomnia because it complains about the insomnia even if heart failure is slight. An obstructive sleep apnea (OSA) is the risk of the heart failure. A heart failure patient often complicates a central sleep apnea (CSA) and a Cheyne-Stokes respiration (CSR), and has much sleep fragmentation and difficulty maintaining sleep. And sleep disorders are sometimes started by the medications such as the cardiovascular system agent thing; beta blocker and the s

[Neonatal apnea].

OBJECTIVE: To review medical literature related to apnea of prematurity. SOURCES: Extensive literature search and clinical practice-oriented concepts. SUMMARY OF THE FINDINGS: Apnea is one of the most common respiratory disorders in the neonatal period. Immaturity of the central nervous system is associated with instability of respiration. Therefore, apnea manifests itself in other systems, causing problems such as hypoglycemia, hypothermia, infection, or patent ductus arteriosus. Apnea may be central, obstructive or mixed depending on the presence of air flow through the upper airways. Diagno

Congenital Anomalies of the Nose: From Embryologic to Surgical Treatment.

The nose, a facial organ, not only plays a crucial role in olfaction and respiration but also has a major impact on the overall anatomy of the face. However, congenital anomalies affecting the nose can be challenging as they require both functional impairments and cosmetic concerns. Facial maldevelopments during the early embryonic period induce a wide-ranging nasal deformity. Understanding the etiology of these anomalies is essential for healthcare professionals involved in diagnosis, treatment, and management. Based on current literature, this review aims to present the spectrum of congenita

Thoracic insufficiency syndrome: Approaches to assessment and management.

Thoracic insufficiency syndrome (TIS) was described in 2003 as the inability of the thorax to support normal respiration or lung growth. TIS includes a broad and disparate group of typically degenerative thoracospinal conditions. Although TIS arises due to a heterogeneous group of disorders and thus its incidence is not well quantified, general approaches to management and treatment exist. Evolving imaging techniques and measurements of health-related quality of life augment tests of pulmonary function to quantify disease burden, longitudinally and pre- and post-intervention. Intervention is p

[Sleep apnea syndrome and heart failure].

More than half of all patients with stable heart failure are affected by sleep apnea syndrome. The mechanism of apnea may be either obstructive, by collapse of the upper airways, or central with nocturnal Cheyne-Stokes respiration. Changes in intrathoracic pressure, awakening and hypoxemia all lead to an increase in left ventricular afterload and to sympathetic activation, which can aggravate heart failure and prognosis. Treatment with positive pressure ventilation improves symptoms as well as cardiac function and prognosis. Sleep respiratory disorders are underdiagnosed and undertreated in pa

[Anesthesia in a patient with relapsing polychondritis].

A 46-year-old female with relapsing polychondritis (RP) was scheduled for surgical repair of saddle-nose and peri-chondritic ear. RP is a rare systemic, inflammatory, and destructive disease of the cartilaginous structures leading to multiple functional disorders in the affected organs. Preoperatively, her chest radiogram of the trachea showed narrowing in diameters which altered about 2 mm with expiration and inspiration. Propofol administered intravenously was given as induction and anesthesia was maintained with nitrous oxide-oxygen-sevoflurane with spontaneous ventilation using laryngeal m

Anesthesia in a patient with acute respiratory insufficiency due to relapsing polychondritis.

We report on a male patient with advanced relapsing polychondritis (RP), who was hospitalized for an ophthalmologic intervention. He had to undergo a tracheostomy with general anesthesia due to respiratory decompensation. RP is a rare systemic, inflammatory, and destructive disease of the cartilaginous structures leading to multiple functional disorders in the affected organs. Involvement of the tracheobronchial tract may cause severe respiratory problems and even life-threatening respiratory insufficiency. Treatment consists of corticoids, chemotherapy, antirheumatics, and surgical interventi

Respiratory patterns during sleep in mitochondrial myopathies with ophthalmoplegia.

Nocturnal polygraphic recordings (electroencephalography, electro-oculography, submental and intercostal muscle electromyography, electrocardiography, respiration by thoracic strain gauges and oronasal thermistors) with continuous monitoring of arterial oxyhemoglobin saturation by pulse oximeter were performed in 8 patients with ophthalmoplegia plus. All patients except 1 had normal blood gas values and normal lung volumes associated with a diminished ventilatory response to inhaled CO2 during wakefulness. Four patients showed pathological sleep-related breathing patterns consisting of sleep a

Rediscovering the importance of nasal breathing in sleep or, shut your mouth and save your sleep.

Recent research, stimulated by the growing awareness of the sleep apnea syndrome, has shown that nasal breathing plays a major role in the regulation of respiration in sleep. These observations are not new; they confirm century-old clinical findings on the importance of nasal breathing in sleep. The earliest account of the deleterious effects of mouth breathing in sleep was made by Lemnious Levinus towards the end of the sixteenth century. Two hundred years later, Catlin dedicated an entire book to the superiority of nasal breathing over mouth breathing in sleep; and in the late 1800's, Cline,

Phrenic and hypoglossal nerve activity during respiratory response to hypoxia in 6-OHDA unilateral model of Parkinson's disease.

AIMS: Parkinson's disease (PD) patients apart from motor dysfunctions exhibit respiratory disturbances. Their mechanism is still unknown and requires investigation. Our research was designed to examine the activity of phrenic (PHR) and hypoglossal (HG) nerves activity during a hypoxic respiratory response in the 6-hydroxydopamine (6-OHDA) model of PD. MAIN METHODS: Male adult Wistar rats were injected unilaterally with 6-OHDA (20μg) or the vehicle into the right medial forebrain bundle (MFB). Two weeks after the surgery the activity of the phrenic and hypoglossal nerve was registered in anest

Associations of Breathing Pattern Disorder and Nijmegen Score With Clinical Outcomes in Difficult-to-Treat Asthma.

BACKGROUND: Breathing pattern disorder (BPD) reflects altered biomechanical patterns of breathing that drive breathing difficulty and commonly accompanies difficult-to-treat asthma. Diagnosis of BPD has no gold standard, but Nijmegen Questionnaire (NQ) >23 is commonly used. OBJECTIVES: We sought to advance clinical characterization of BPD and better understand the clinical utility of NQ in difficult asthma in patients from the Wessex AsThma CoHort of difficult asthma (WATCH) study. METHODS: Associations between demographic and clinical factors in difficult asthma and BPD, ascertained by clin

Respiratory function in Parkinson's disease.

This article reviews the spectrum of respiratory dysfunction in Parkinson's disease (PD). It includes the primary effects of PD on the ventilation, response to medications, and pulmonary complications of antiparkinson therapy. Primary pulmonary abnormalities include a restrictive change mainly secondary to chest wall rigidity and upper airway obstruction; both are responsive to dopaminergic modulation. Respiratory dyskinesia, a side effect of levodopa therapy, may produce both restrictive and dyskinetic ventilation. Therapy with ergot derivatives may result in pleuropulmonary fibrosis. Lastly,

Breathing pattern abnormalities in full term asphyxiated newborn infants.

Perinatal asphyxia is a cause of significant morbidity among full term infants, but breathing abnormalities after an asphyxic insult have not been studied. This report details breathing patterns of 16 full term asphyxiated infants, during the first week of life who were studied by transthoracic impedance pneumocardiograms. Pneumocardiograms were abnormal in 69% of infants in the asphyxiated group and 13% of infants in the control group. Significant differences were noted in the incidence of prolonged apnoea, the percentage of periodic breathing, and in apnoea density. These results indicate th
