Teflon paste injection into the paralytic vocal fold. A simple procedure to improve disturbed cough function.

Teflon paste injection into the paralytic vocal fold is a simple method for the relief of difficulties with insufficient expectoration and for the prevention of aspiration caused by laryngeal paralysis. The laryngeal valve and voice functions in three patients with unilateral laryngeal paralysis were studied before and after teflon paste injection. Aerodynamic registrations comprised subglottal and supraglottal air pressure and oral air flow during cough and phonation. The aerodynamic records, as well as the patients' own reports, all showed improved laryngeal valve function as a result of the

Role of medialization in the improvement of breath control in unilateral vocal fold paralysis.

A prospective study was performed on 18 adults to analyze disturbances of voice and respiratory function in unilateral vocal fold paralysis and the role of medialization in the improvement of breath control. In addition to physical examination, an acoustic and aerodynamic analysis, as well as patient self-assessment, were performed both before and after medialization (14 fat injections, 4 thyroplasties) to evaluate voice and both phonatory breath control and respiratory function outcome. Preoperative and postoperative results, at short, middle, and long term, were compared to data from 14 cont

Diagnosis and treatment of botulism in lions.

Six circus lions (Panthera leo) showed neurological and gastrointestinal signs after consuming casualty broiler chickens. Signs included ata … ls made slow recoveries over varying periods; one lion was recumbent for 41 days. No lion developed respiratory paralysis; other animals which had consumed the chickens remained healthy. Aspects of the treatment of botulism in animals are discussed.

Phrenic Nerve Reconstruction with Short-Term Diaphragm Pacing Corrects Diaphragm Paralysis Due to Intrathoracic Nerve Injury.

BACKGROUND: Symptomatic diaphragm paralysis and subsequent pulmonary debilitation has been shown to occur following phrenic nerve injury within the thoracic cavity. In this study, the authors investigated the effectiveness of intrathoracic reconstruction of the phrenic nerve combined with short-term diaphragm pacing in restoring diaphragm function. METHODS: A retrospective review of 34 consecutive patients who underwent intrathoracic phrenic nerve reconstruction and diaphragm pacing for symptomatic diaphragm paralysis was performed. Inclusion criteria included confirmation of diaphragm paraly

Multivesicular mediastinal hydatid cyst presenting with left recurrent and phrenic nerve palsy.

Hydatid disease caused by Echinococcus granulosus is endemic in Indian subcontinent and a common surgical problem. Although hydatid cyst is common in the liver and lung, it can be encountered in almost every part of the body. Mediastinal hydatid disease is a rare occurrence and only a few cases have been reported in the literature. The present article reports a case of a multivesicular mediastinal hydatid cyst associated with nerve palsies. The patient presented with left recurrent nerve and phrenic nerve palsy. Ultrasonography (US) and computed tomography (CT) of the thorax revealed a multivesi

[Intravenous regional anesthesia in patients with hypokalemic periodic paralysis].

Hypokalemic periodic paralysis is a familial autosomal dominant trait. The paralytic attacks are precipitated by large carbohydrate-rich meals, cold, mental or surgical stress, infections, exercise, drugs, electrolytes and endocrine abnormalities. Death may occur from respiratory arrest, infections, aspiration or cardiac arrhythmias. Anesthesia and surgical procedures may induce an attack and complicate the perioperative patient condition. Guidelines for anesthetic management should include preventive measures i.e. reduce mental stress and carbohydrate intake and correction of electrolytes and

Laser cordotomy for the treatment of bilateral vocal cord paralysis in infants.

OBJECTIVE: Vocal cord paralysis is the second cause of neonatal stridor. Several surgical treatments are proposed in order to avoid tracheotomy or to decanulate patients. Laser posterior partial cordotomy is supposed to be a minimal invasive procedure. The purpose of the study is to share our experience in management of laryngeal paralysis with this technique in infants and appreciate its role in avoiding tracheotomy in infants. DESIGN: Retrospective study. PATIENTS: The charts of 0-2-year-old patients treated for vocal cord paralysis from 1996 to 2007 are reviewed. Eleven infants with bilat

Impact of laryngeal paralysis and its treatment on the glottic aperture and upper airway flow characteristics during exercise.

Patients with unilateral vocal fold paralysis occasionally report shortness of breath during exercise. This symptom may persist in some patients after medialization thyroplasty. A review of the literature revealed no study that objectively evaluated laryngeal dynamics or airway flow characteristics during exercise after medialization thyroplasty for unilateral laryngeal paralysis. This study evaluates glottic aperture size and configuration as well as upper airway flow characteristics during exercise in 16 subjects. Six patients who underwent medialization thyroplasty for unilateral vocal fold

Fluorographic findings of diaphragmatic paralysis with spontaneous recovery.

BACKGROUND: Postoperative diaphragmatic paralysis is an unavoidable complication of cardiovascular surgery. Although diaphragmatic plication, as a surgical treatment, can be performed, spontaneous recovery is possible. We aimed to identify differences in fluorographic findings of diaphragmatic paralysis between pediatric patients with and without spontaneous recovery within 1 year of intrathoracic surgery. METHODS: Ten children, who had been followed-up for at least 1 year post-surgery and who had not received diaphragmatic plication were included and classified into those with or without spo

Idiopathic diaphragmatic paralysis and asthma bronchiale.

A 38-year-old man suffered from chronic asthma. He also had paralysis of the right diaphragm which had been present for some years. During an acute asthmatic attack following administration of acetyl salicylic acid, a chest roentgenogram revealed hyperinflation of the lungs, especially of the right side. The paralytic diaphragm was pushed down considerably. After treatment of the acute attack of asthma, the diaphragm ascended to its 'normal' place. This case illustrates 'air trapping' in acute asthma and sheds some light on the mechanism behind it, which is discussed.

Rapid Onset of Neuromuscular Paralysis or Weakness.

The focus of this narrative review is the differential diagnosis of disease involving the peripheral or lower motor neuron component of the neurology of breathing. The clinical context is limited to those conditions leading to admission to the intensive care unit with a time course often described as acute or of rapid onset, meaning within days to weeks. However, the article also reviews those underlying inherited or congenital conditions that may have gone unnoticed until fulminant deterioration with respiratory failure.

[DIAPHRAGMATIC PARALYSIS].

One-sided diaphragmatic paralysis is a common phenomenon which is usually a-symptomatic. In case of acute onset or if there is an underlying lung disease, the phenomenon may be symptomatic and even limiting. In this article, we present a patient who arrived with subacute shortness of breath when lying down. She underwent thorough investigations but, as happens in most cases, the cause of the paralysis was not identified and it remains idiopathic. The authors present an overview of the etiology, differential diagnosis and treatment of diaphragmatic paralysis.

Acute Neuromuscular Respiratory Failure.

PURPOSE OF REVIEW: Neurologists working in the hospital are often called to evaluate patients with severe muscle weakness. Some of these patients can develop ventilatory compromise and require admission to the intensive care unit (ICU). This article reviews the general evaluation of neuromuscular respiratory failure, discusses its differential diagnosis, and provides practical advice on the management of its most common causes. RECENT FINDINGS: Determining the cause of acute neuromuscular respiratory failure is crucial because functional prognosis is poor in patients for whom the cause cannot

Spinal muscular atrophy type 1: is long-term mechanical ventilation ethical?

We present a baby with spinal muscular atrophy type 1, an inherited disorder causing progressive weakness, leading to complete paralysis of respiratory, facial and limb muscles. Without intervention, death occurs in infancy due to respiratory failure. Mechanical ventilatory support can prolong life, but the child's quality of life is highly debatable. We discuss the appropriateness of initiating and continuing intensive care for this child and others in a similar position.

[Change in the histamine and serotonin levels in the external respiratory apparatus in botulinic poisoning in cats].

General botulinic intoxication in cats, accompanied by severe respiratory insufficiency, was characterized by distinct impairments in histamine and serotonin metabolism in external respiratory system. Pronounced increase in content of histamine was observed in impaired muscles and their corresponding regional centers of medulla spinalis, when botulinic intoxication had the local type with unilateral paralysis of diaphragm and intercostal muscles. Content of serotonin was increased in the tissues studied only at the later steps of local botulinic intoxication.
