Bilateral central retinal artery occlusion in occult temporal arteritis.

A rare case of bilateral central retinal artery occlusion due to occult temporal arteritis is reported. We stress temporal artery biopsy in every patient with central retinal artery occlusion in old age.

Branch retinal artery occlusion in the Churg-Strauss syndrome.

Amaurosis fugax followed by retinal infarction occurred as an early manifestation of exacerbation in a 46-year-old woman with allergic angiitis and granulomatosis (Churg-Strauss syndrome). There was evidence of widespread intraluminal branch retinal artery occlusions without accompanying retinal vasculitis. Low molecular weight dextran and heparin, in addition to systemic steroid therapy, appeared to be beneficial. A review of the ophthalmic manifestations of the Churg-Strauss syndrome and of other hypereosinophilic conditions suggests that thromboembolism resulting from a hypercoagulable stat…

Traumatic central retinal artery occlusion with sickle cell trait.

An 8-year-old black boy with sickle cell trait struck his left brow and globe on the edge of a table and had immediate blurring of vision. Six days later visual acuity was light projection, and traumatic iritis with secondary glaucoma and perimacular edema were present. Fluorescein angiography indicated obstructed peripheral and perimacular arterioles and dye leakage from the disk.

A review of the management of central retinal artery occlusion.

Central retinal artery occlusion (CRAO), the ocular analog of a cerebral stroke, is an ophthalmic emergency. The visual prognosis for overall spontaneous visual recovery in CRAO is low. Furthermore, the risk of future ischemic heart disease and cerebral stroke is increased due to the underlying atherosclerotic risk factors. There is currently no guideline-endorsed treatment for CRAO. This review will describe the anatomy, pathophysiology, epidemiology, and clinical features of CRAO, and investigate the current and future management strategies.

Spontaneous involution of a large retinal arterial macroaneurysm.

BACKGROUND: Acquired retinal macroaneurysms are round dilatations of retinal arterioles that occur in the posterior fundus. They tend to rupture before they grow large and cause a visual decrease. The average diameter of macroaneurysms in the previous reports is 281 micrometer. METHODS: A 63-year-old man complained of a sudden decrease of visual acuity in the left eye. Fundus examination of his left eye revealed a large protruded mass, 2 disc diameters in size, at the first bifurcation of the inferotemporal retinal artery. Preretinal vitreous bleeding and serous retinal detachment were observ…

[Thrombolysis of arterial retinal occlusion using urokinase].

The authors present an account on possible aimed thrombolytic treatment of occlusion of the retinal artery by urokinase. Aimed thrombolysis can be performed at ophthalmological departments with an available radiodiagnostic department, which performs angiographies of carotid arteries. The authors discuss the necessary dose of 200,000 to 300,000 i.u. of urokinase. One-hour infusion with an initial booster dose is best. Early administration of the preparation is considered most important by the authors. The authors present the case-history of a 34-year-old patient with complete occlusion of the c…

Vanishing retinal arterial aneurysms: a case report.

An 18-year-old woman with retinal vasculitis developed multiple retinal arterial aneurysms over a period of 3 years. Subsequently she developed macular oedema, peripheral neovascularisation, and vitreous haemorrhage and was treated with systemic steroids, laser photocoagulation, and vitreoretinal surgery. No systemic cause for vasculitis was found. Serial fluorescein angiography demonstrated gradual resolution of the arterial aneurysms over the subsequent 7 years.

Idiopathic recurrent branch retinal arterial occlusion.

Recurrent multiple branch retinal arterial occlusions of unknown cause occurred in nine apparently healthy individuals. With the exception of three patients who developed associated auditory symptoms, none developed other signs or symptoms of systemic vascular disease. Ophthalmoscopic and fluorescein angiographic findings suggested focal arteritis and arteriolitis as the cause of the obstructions. Most patients retained good visual acuity.

Segmental retinal periarteritis with branch arterial occlusion.

An 18-year-old woman complained of acute visual field loss in the right eye due to branch arterial occlusion. Similar disease occurred in the left eye two years later. Extensive laboratory examination revealed no factor of etiologic significance, and no sign of uveitis was observed. The retinal arterioles around the optic disc were segmentally inflamed angiographically, which led to their obstruction. Corticosteroid therapy (betamethazone, 3.0 mg/day) improved the arteriolitis within two weeks.

Retinal arterial obstruction in children and young adults.

The records of 27 patients who developed retinal arterial obstruction (RAO) prior to the age of 30 years were studied to ascertain associated systemic and ocular findings as possible etiologic factors. A history of migraine was found in approximately one third of the patients, and coagulation abnormalities wer also common. Trauma, sickle cell hemoglobinopathies, cardiac disorders, use of oral contraceptives, pregnancy, systemic lupus erythematosus and intravenous drug abuse were less frequently encountered. Ocular abnormalities included increased intraocular pressure, subtle buried drusen of t

Cardiovascular disease in patients with retinal arterial occlusion.

103 patients with retinal-artery occlusion causing permanent visual-field defect were examined clinically and angiographically to assess the extent of cardiovascular disease. The 68 patients with branch-artery occlusion had a higher prevalence of previous transient episodes, ischaemic and valvular heart-disease, and more often had an operable lesion in the carotid artery than had the 35 patients with central-artery occlusion who were more often hypertensive, had fewer warning attacks, a greater prevalence of complete carotid occlusion, and fewer operable carotid lesions. The two types of retin
