Gastrointestinal neuroprosthesis for motility and metabolic neuromodulation.

Gastrointestinal (GI) dysmotility and associated conditions affect over 20% of population, yet pharmacological, behavioural, and surgical interventions offer limited therapeutic efficacy. Targeted electrical stimulation addressing underlying neuromuscular pathology stands to transform our ability to treat dysmotility. Here, we developed a closed-loop GI neuroprosthesis which activates or relaxes GI tract musculature through electrochemical stimulation in response to sensed food stimuli. We additionally describe a tool supporting minimally invasive endoscopically guided implantation that can pe

Food, Dietary Patterns, or Is Eating Behavior to Blame? Analyzing the Nutritional Aspects of Functional Dyspepsia.

Functional dyspepsia is a gastrointestinal disorder characterized by postprandial fullness, early satiation, epigastric pain, and epigastric burning. The pathophysiology of the disease is not fully elucidated and there is no permanent cure, although some therapies (drugs or herbal remedies) try to reduce the symptoms. Diet plays a critical role in either the reduction or the exacerbation of functional dyspepsia symptoms; therefore dietary management is considered to be of high importance. Several foods have been suggested to be associated with worsening functional dyspepsia, such as fatty and …

Gut peptides in the control of food intake.

Multiple gut peptides are involved in the overall control of food intake. Plasma levels of gut peptides are differentially affected by food intake, and the different patterns of release around meals provides an indication of a peptide's specific role in feeding control. Ghrelin is a gastric peptide whose plasma levels are high before meals and are suppressed in response to food intake. Consistent with this pattern, ghrelin has been shown to stimulate food intake by hastening meal initiations. Cholecystokinin (CCK) is released from upper intestinal sites in response to food intake. CCK inhibits

Effect of occluding the pylorus on intraoral intake: a test of the gastric hypothesis of meal termination.

Meal size does not change in response to food being restricted to the stomach by occlusion of the pylorus. This result has been used as evidence for a gastric model of meal termination where feedback arising solely from the stomach is taken to underlie satiation. Such data provide support for the gastric model, however, only if the rate of gastric emptying during ingestion in the unoccluded condition is slow, such that comparable amounts of food would be found in the stomach at the end of the meal in both the pylorus-occluded and unoccluded conditions. To evaluate this tissue directly, rats we…

[Control of ingestive behavior and rumination by neuropeptides].

In sheep as in other species, intracerebroventricular (ICV) administration of picomolar doses of several peptide hormones affects food intake and rumination, suggesting that these hormones play a role in the physiological control of feeding behavior. Their effects depend on their duration (short vs long-term) and/or their nature (orexigenic vs anorexigenic). Among the short-term satiety factors, CCK8, CRF and calcitonin, administered ICV, decrease food intake by reducing the rate (CCK8) or the duration (CRF, calcitonin) of ingestion; in contrast, the gastrin group of peptides (gastrin 17, pent

Imbalanced insulin action in chronic over nutrition: Clinical harm, molecular mechanisms, and a way forward.

The growing worldwide prevalence of overnutrition and underexertion threatens the gains that we have made against atherosclerotic cardiovascular disease and other maladies. Chronic overnutrition causes the atherometabolic syndrome, which is a cluster of seemingly unrelated health problems characterized by increased abdominal girth and body-mass index, high fasting and postprandial concentrations of cholesterol- and triglyceride-rich apoB-lipoproteins (C-TRLs), low plasma HDL levels, impaired regulation of plasma glucose concentrations, hypertension, and a significant risk of developing overt t

Central amylin expression and its induction in rat dams.

Amylin, or islet amyloid polypeptide, is known as a satiating signal expressed in pancreatic beta-cells but not in the brain. In this study, regulations of postpartum mRNA expressions were investigated in the preoptic area of the hypothalamus. mRNA levels of lactating dams and mothers whose pups were removed immediately after delivery were compared in microarray experiments. There were 20 genes identified with significantly increased and 14 with decreased expression 9 days postpartum. Amylin mRNA level demonstrated the largest change, a 25.7 times increase. Quantitative RT-PCR measurements val

Circulating metabolites associated with objectively measured sleep duration and sleep variability in overweight/obese participants: a metabolomics approach within the SATIN study.

STUDY OBJECTIVES: To investigate the associations of circulating metabolites with sleep duration and sleep variability. We also assessed the ability of metabolites to discriminate between sleep duration and sleep variability categories. METHODS: Cross-sectional analyses were performed on baseline data from 205 participants with overweight/obesity in the "Satiety Innovation" (SATIN) study. A targeted metabolite profiling (n = 159 metabolites) approach was applied using three different platforms (nuclear magnetic resonance, liquid chromatography coupled to mass spectrometry, and gas chromatogra

Partial replacement of meat and fat with hydrated wheat fiber in beef burgers decreases caloric value without reducing the feeling of satiety after consumption.

The aim of this study was to evaluate the effects of different levels of hydrated wheat fiber replacing meat and fat in beef burgers on technological characteristics, sensory acceptance and hunger satisfaction. The different levels of hydrated wheat fiber (1 g fiber: 6 g water) were 0, 1.25, 2.5, 3.75 and 5.0 g of fiber/80 g burger portion. Results showed that the greater the addition of hydrated wheat fiber, the lower the protein (P < .0001) and lipid (P = .0006) content and consequently the greater the reduction in caloric value. Burgers with up to 3.75 g fiber/80 g portion showed the same (

Role of endogenous opioids in the control of gastric sensorimotor function.

Endogenous opioids have been implicated not only in the process of feeding but also in the control of gastric sensitivity and gastric motor responses, and impairment of antinociceptive opioid pathways has been hypothesized to contribute to the pathogenesis of functional dyspepsia. Our aim was to study the effect of suppression of endogenous opioid action by naloxone on gastric sensorimotor function in healthy volunteers. During intravenous administration of saline or naloxone (0.4 mg intravenous bolus followed by continuous infusion 20 microg kg(-1) h(-1)), sensitivity to gastric distension, g

Eating behavior in bulimia nervosa: multiple meal analyses.

Ten bulimic individuals were admitted to an inpatient unit and for 7 consecutive days eating behavior was observed and recorded. Age, sex, and weight-matched control subjects (n = 10) were admitted to the same unit for 4 d. All food and fluid intake, frequency of binge eating and purging, and ratings of appetite and mood before and after eating were recorded every 24 h. Bulimic patients demonstrated chaotic eating patterns that varied within as well as between individuals. Total daily energy intake was significantly higher for bulimic patients (41982 +/- 113 kJ; 10,034 +/- 2701 kcal) than for

Serotonin activity in anorexia and bulimia nervosa: relationship to the modulation of feeding and mood.

Brain serotonin activity contributes to satiety. Theoretically, binging behavior is consistent with reduced serotonin function, whereas anorexia nervosa is consistent with increased serotonin activity. Brain serotonin abnormalities could also contribute to psychopathologic behaviors such as dysphoric mood, disturbance of impulse control, and obsessionality, as well as neuroendocrine disturbances. In fact, disturbances of serotonin have been found in acutely ill anorexia nervosa patients. Tryptophan, an essential amino acid found in the diet, is the precursor of serotonin. Thus serotonin distur

Role of pylorus in mediating cholecystokinin-stimulated satiety in the Zucker rat.

Obese Zucker rats are less responsive than their lean littermates to the effects of cholecystokinin-octapeptide on satiety and pancreatic growth and exocrine function. We hypothesized that the hyperphagia observed in obese Zucker rats may be caused by a decreased pyloric contractile response to cholecystokinin, resulting in an increased rate of gastric emptying, decreased postprandial gastric distention, and thus decreased satiety. Pyloric muscle strips from six obese Zucker rats and six lean littermates were mounted in separate tissue baths and isometric contraction was measured in response t

Opiate receptor blockade in man reduces 2-deoxy-D-glucose-induced food intake but not hunger, thirst, and hypothermia.

Opioid peptides may act as neuromodulators in the central nervous system to conserve energy stores and water in mammals. To examine this hypothesis in man, the effect of opiate receptor blockade with naloxone on the hunger, thirst, and hypothermic response to 2-deoxy-D-glucose-induced glucoprivic stress was assessed. Opiate receptor blockade decreased stress-induced food intake but did not reduce marked increases in hunger produced by glucoprivation. Naloxone infusions did not change the hypercortisolemic, polydipsic, hypothermic, and thermogenic response to 2-deoxy-D-glucose. While these resu

Listening to children with obesity: lived experiences of hunger, cravings, and satiety.

BACKGROUND: Appetite regulation is central to childhood obesity, yet little is known about how children themselves experience and interpret sensations of hunger, cravings, and satiety. This study aimed to explore how children living with overweight or obesity describe these sensations and how they respond to them in everyday life. METHODS: This qualitative study included 32 children (17 girls, 15 boys) aged 8-10 years who were interviewed four years after enrolling in a randomized controlled obesity treatment trial in Stockholm, Sweden. At follow-up (mean age 9.4 years, SD 0.8), the majority

Association between constipation and insufficient dietary intake to achieve satiety in preschool age.

Background: Few studies have explored the relationship between constipation and not eating enough to feel satiety. Aim: The study aims to examine the association between constipation and not eating enough to feel satiety. Methods: This cross-sectional study included 189 children 3 to 6 years of age. Constipation was defined as two or fewer defecations per week. To assess the satiety of children, mothers were asked, "Does your child eat until he/she is full every day?" The response options were as follows: "eats every day," "eats occasionally," "neither eats every day nor eats occasionally," "d

Hedonic Responses to Meal Ingestion in Patients With Functional Dyspepsia and Postprandial Symptoms.

INTRODUCTION: It is not clear whether patients with postprandial symptoms enjoy the meal. METHODS: Prospective trial in patients with postprandial distress syndrome and healthy controls, comparing their responses with stepwise ingestion of a comfort meal. RESULTS: Healthy individuals (N = 18) enjoyed the meal and experienced digestive well-being. Despite their symptoms, some patients enjoyed the meal as much as healthy individuals (n = 9), while others did not (n = 15), and both groups experienced the same degree of early satiety, fullness, and discomfort. DISCUSSION: The hedonic response t

Systemic and Central Amylin, Amylin Receptor Signaling, and Their Physiological and Pathophysiological Roles in Metabolism.

This article in the Neural and Endocrine Section of Comprehensive Physiology discusses the physiology and pathophysiology of the pancreatic hormone amylin. Shortly after its discovery in 1986, amylin has been shown to reduce food intake as a satiation signal to limit meal size. Amylin also affects food reward, sensitizes the brain to the catabolic actions of leptin, and may also play a prominent role in the development of certain brain areas that are involved in metabolic control. Amylin may act at different sites in the brain in addition to the area postrema (AP) in the caudal hindbrain. In p…

Postprandial distress syndrome: stratification and management.

Functional dyspepsia (FD), defined by the Rome consensus as the presence of functional symptoms originating from the gastroduodenum, is one of the most common functional gastrointestinal disorders. FD is subdivided into postprandial distress syndrome (PDS), with meal-related symptoms such as postprandial fullness and early satiation, and epigastric pain syndrome (EPS), with meal-unrelated symptoms such as epigastric pain or burning. We used a literature search for a narrative review on the current state of knowledge regarding PDS. Areas covered: Epidemiological studies support PDS as a separat…
