Incidence and prevalence of episcleritis and scleritis in Northern California.

PURPOSE: To evaluate the incidence and prevalence of episcleritis and scleritis in a large well-defined population in Northern California. METHODS: Secondary analysis was performed on data from the Northern California Epidemiology of Uveitis Study. The patient database of a large regional health maintenance organization was searched for all patients who potentially experienced ocular inflammatory disease during the 12-month study period. Medical records were reviewed for all potential patients to confirm ocular inflammatory disease and specific diagnosis, establish the time of onset, and coll

[Scleritis: when might a systemic disease be involved?].

Scleritis is typically a severe painful inflammatory disorder of the eye. This process may involve the sclera but also the cornea, adjacent episclera, and underlying uvea and therefore threaten vision. Careful clinical history taking, detailed ocular examination, appropriate investigation for systemic disease, and timely intervention with the use of immunosuppressive drugs when necessary has improved the long-term outcome of patients with scleritis.

Posterior scleritis in patients with systemic lupus erythematosus.

PURPOSE: To describe three cases of posterior scleritis associated with systemic lupus erythematosus. METHODS: Retrospective chart review including ophthalmic examination, fundus photography, optical coherence tomography, angiography, ultrasonography, and/or magnetic resonance imaging. RESULTS: Three patients presented with blurred vision, red eye, and/or eye pain in one or both eyes. The diagnosis of posterior scleritis was confirmed in each patient using B-scan ultrasonography. One patient presented with giant nodular scleritis. In two patients, posterior scleritis was the presenting sign

Necrotic choroidal melanoma with scleritis and choroidal effusion.

PURPOSE: To report a case of a choroidal melanoma that presented with painful scleritis and choroidal effusion. METHODS: Interventional case report with cytopathologic correlation. RESULTS: A 78-year-old otherwise healthy woman presented with sudden, severe painful ocular inflammation and marked injection of the right eye over a 3-day period, consistent with scleritis. Upon referral, 360 degrees of scleral and conjunctival injection and peripheral choroidal effusion were found. In addition, a solid hemorrhagic, choroidal mass was evident, suspicious for melanoma. Fine needle aspiration biops

Evaluation of subconjunctival triamcinolone for nonnecrotizing anterior scleritis.

PURPOSE: Subconjunctival corticosteroid injection (SCI) for nonnecrotizing anterior scleritis remains controversial, partly because long-term follow-up is not available. This study documents the efficacy and adverse effects of SCI. DESIGN: Retrospective, noncomparative, interventional case series. PARTICIPANTS: Thirty-eight eyes of 35 patients with nonnecrotizing, noninfectious anterior scleritis resistant to prior systemic or local therapy. INTERVENTION: Subconjunctival triamcinolone acetonide injection. MAIN OUTCOME MEASURE: Persistence or resolution of signs and symptoms of anterior scl

Post-surgical scleritis associated with the ganciclovir implant.

Although the ganciclovir implant is an effective and well-tolerated treatment for cytomegalovirus retinitis in patients with human immunodeficiency virus infection, complications that may occur include retinal detachment, implant extrusion, and endophthalmitis. A 22-year-old woman with human immunodeficiency virus infection presented with a painful left eye with scleritis overlying previous ganciclovir implant sclerotomy sites. The inflammation progressed 360 degrees around the pars plana with progressive thinning at the implant sites. Post-surgical necrotizing scleritis is another complicatio

Posterior scleritis--an unusual presentation of facial pain.

Defining the aetiology of facial pain can be difficult. While acknowledging the principle of 'common things are common', it is prudent to consider all possible causes. We describe a patient with posterior scleritis that presented as facial pain and was cured by oral prednisolone.

Episcleritis and scleritis: clinical features and treatment results.

PURPOSE: To evaluate the clinical experience with episcleritis and scleritis at a tertiary care eye center. METHODS: Retrospective chart review. RESULTS: One hundred thirty-four patients with scleral inflammation were seen over a 12-year period. Thirty-seven patients had episcleritis, and 97 patients had scleritis. Ocular complications occurred in only 13.5% of patients with episcleritis but in 58.8% of patients with scleritis (P <.0001). No patient with episcleritis had a decrease in visual acuity, whereas 15.9% of patients with scleritis did. Only 16.7% of patients with episcleritis requir

Scleral patch graft in the management of necrotizing scleritis.

NS is a rare but devastating consequence of certain connective tissue disorders, most commonly RA. The presence of NS mandates that patients be treated with at least one immunosuppressive chemotherapeutic drug. The cure rate is high in such instances after 1 year of freedom from any evidence of recurrent inflammation. Surgical treatment through tectonic scleral and peripheral corneal grafting is rarely (although sometimes) indicated, except in instances of NS that has advanced to the point of perforation of the globe. Various graft materials can be used, including fascia lata, periosteum, spli

Posterior scleritis--an unusual manifestation of Cogan's syndrome.

Cogan's syndrome is characterized by a non-luetic interstitial keratitis associated with vertigo, tinnitus and profound deafness. Evidence of a systemic vasculitis is found in up to 50% of patients. Atypical forms of Cogan's syndrome have been described in which the ocular inflammatory disease may be more severe. We describe a case of atypical Cogan's syndrome in association with bilateral posterior scleritis. Serial B-scan ultrasound measurements of posterior scleral thickness were found to be useful in assessing disease activity, in combination with clinical findings. Combination therapy wit

Scleritis as an initial manifestation of choroidal malignant melanoma.

PURPOSE: The purpose of this article is to present the unusual circumstances in which malignant melanoma of the choroid can masquerade as scleritis, thus confounding its diagnosis. METHODS: Three cases of plaque-like malignant melanomas of the choroid are reported that, on initial examination, had features of scleritis. The events leading to their eventual correct diagnosis also are presented. RESULTS: In each case, there was ocular pain, blurred vision, anterior chamber and/or vitreous cavity cellular reaction, and an exudative retinal detachment associated with an ill-defined, relatively f

Scleritis as the presenting manifestation of procainamide-induced lupus.

Scleritis developed in a patient using procainamide as part of a drug-induced lupus syndrome. Systemic findings, which developed after the onset of ocular signs and symptoms, included arthralgias, myalgias, weight loss, and markedly elevated antinuclear antibody (ANA) titers and antihistone titers. The clinical picture and laboratory abnormalities improved after discontinuation of the drug. Although systemic findings secondary to drug-induced lupus have been well described in the medical literature, this case represents the first detailed ophthalmologic documentation of scleritis as the presen
