BACKGROUND: Urethral metastatic adenocarcinoma is extremely rare. Moreover, only 9 previous cases with metastases from colorectal cancer have been reported to date, and not much information on urethral metastases from colorectum is available so far.

The rectosigmoid junction (RSJ) is the limit between the sigmoid colon and the rectum. The limits of this transition zone have been a source of controversy. There are multiple definitions that aim to establish the beginning of the rectum. Some organizations propose the use of more than one definition. The frequency of liver metastasis was analyzed in 1,558 patients with primary colorectal carcinomas. The highest frequency of liver metastasis occurred in carcinomas of the transverse colon (17%) and the sigmoid colon (15%), when compared to the other segments, such as the cecum (10%), ascending colon (9%), descending colon (6%), rectum (9%) and anal canal (0%). It would appear that carcinoma located in those segments of the human colorectum containing any portion of the mesocolon are more prone to metastasize into the liver.

A 72-year-old women was referred to our hospital because of lower left abdominal pain. Computed tomography showed prominent sigmoid colon dilation and double tumors on both the oral and anal sides. Surgical resection revealed an expanded sigmoid colon involved in double cancer that showed strong adhesion to the surrounding tissues. The pathological findings revealed obstructive colitis and minor perforation in the dilated colon. The minor perforation was considered to have been caused by fecal impaction in the closed cavity between the two tumors, resulting in an increase in colon pressure.
