[Endoscopic treatment of sphenoid aspergilloma].

In recent years there has been an increase in fungal infections of the nose and paranasal sinuses, even though it is difficult to ascertain the incidence and prevalence of such disorders. The only information which can be drawn from the literature is that such infections are diagnosed in 6% to 13% of surgically treated chronic sinusitis. Aspergillus is the etiologic agent in approximately 80% of cases. Fungal infections are classified into two groups: non-invasive and invasive. The former includes mycetoma and allergic fungal sinusitis. The latter comprises the chronic indolent form, which pre

[Orbital and intracranial complications of acute sinusitis in childhood--status of endoscopic paranasal sinus surgery based on case examples].

BACKGROUND: Orbital or endocranial complications of sinusitis are rare clinical syndromes in infancy. METHODS: We retrospectively examined the complications that occurred and the role of functional endoscopic sinus surgery (FES). RESULTS: Most of the complications were caused by acute inflammation of the ethmoid or sphenoid cells with direct extension through dehiscent areas (flat bone) or neurovascular foramina. MRT had the highest sensitivity in detecting acute inflammatory foci. Early elimination of the cause of inflammation by FESS prevents spread of the complication. Advantages of FESS

Bacterial Flora in the Sphenoid Sinus Changes with Perioperative Prophylactic Antibiotic Administration.

The complications of endonasal transsphenoidal surgery (ETSS) include meningitis and sinusitis, and these complications are troublesome. Some reports have investigated the type of bacteria and the susceptibility of sphenoid sinus mucosal flora to drugs. However, most specimens can be collected after perioperative antibiotic administration. In this study, 95 and 103 sphenoid sinus mucosal samples collected during ETSS from September 2013 to February 2015 and from June 2017 to January 2019, respectively, were examined for bacterial culture. Sphenoid sinus mucosal samples were collected after ant

Pneumosinus dilatans after prolonged cerebrospinal fluid shunting in young adults with cerebral hemiatrophy. A report of two cases and review of the literature.

Pneumosinus dilatans is a generalized or partial enlargement of the paranasal sinuses containing only air. Pneumosinus dilatans occurs as an idiopathic disorder as well as in association with other disorders, including cerebral hemiatrophy. We report two cases of patients with congenital cerebral hemiatrophy who developed juvenile pneumosinus dilatans of the frontal, ethmoidal, and sphenoidal sinus. The hydrocephalus of both patients was treated by prolonged cerebrospinal fluid shunting. The development of hyperpneumatization of the paranasal sinuses was proved by plain radiographs and CT. Pre

[The diagnostic reliability of the occipitomental radiograph of the paranasal sinuses. Correlation with magnetic resonance tomography].

100 conventional occipitomental radiographs of the paranasal sinuses were studied independently by five radiologists. Predictive values were determined, based on MRI as the gold standard. Sensitivity and specificity for detection of mucosal thickening in the sinuses were: maxillary sinus: 0.73/0.76, frontal sinus: 0.20/0.85, ethmoid sinus: 0.38/0.87, sphenoid sinus: 0.14/0.96. In the maxillary sinus polypoid thickening was demonstrated with a significantly higher sensitivity than that of diffuse mucosal thickening (0.82 versus 0.65, Chi-square-test, p less than 0.01). Because of the low predic

[Maxillary sinus by waters' view-superimposed structures].

The image of the maxillary sinus obtained by Waters' view contains such superimposed structures as lateral extensions of the sphenoidal sinus and posterior ethmoidal cells. Planimetric evaluations were done for the maxillary sinus itself and the two superimposed sinuses. The medial portion of maxillary sinus is occupied by these two sinuses and the areas covered by these amount to one third of the maxillary sinus' area. To properly interpret the radiolucency of the maxillary sinus, the areas covered by these two sinuses and the areas corresponding to the zygomatic recess should be avoided. The

A Comparison of Sphenoid Sinus Osteoneogenesis in Aspirin-Exacerbated Respiratory Disease.

BACKGROUND: Aspirin-exacerbated respiratory disease (AERD) is characterized by excessive leukotriene production, diffuse polyp burden and osteitic bone changes. These bony changes have not been previously characterized. OBJECTIVE: The aim of this radiographic study is to characterize the bony changes noted on computed tomography (CT) scans of the sphenoid sinus in patients with AERD compared to other diseased sinonasal inflammatory states and non-diseased controls. METHODS: A retrospective review of 43 patients with clinically confirmed AERD were included and compared to 22 non-diseased, 9 a

Frontal sinus pneumatisation: an isolated finding or a sign of concomitant anatomical variation?

OBJECTIVE: To determine the association between frontal sinus pneumatisation and concomitant anatomical variations in paranasal sinuses as seen on computed tomography. METHODS: A total of 403 patients who underwent paranasal sinus computed tomography were allocated to three groups according to the degree of frontal sinus pneumatisation (type 1 - hypoplasia; type 2 - medium size; and type 3 - hyperplasia). In this unique model, the prevalence of ipsilateral variations on paranasal sinuses on each side of the head were analysed separately within each pneumatisation group. RESULTS: The type 3 f

A modified surgical procedure for endoscopic optic nerve decompression for the treatment of traumatic optic neuropathy.

BACKGROUND: Although the endoscopic anterior-to-posterior technique offers many advantages, the long-term effects of the iatrogenic trauma (removal of the uncinate process and anterior ethmoidal sinus) resulting from the complete ethmoidectomy procedure used to gain full access to the optic nerve canal is unknown, and sequelae such as nasal synechia and sinusitis should not be ignored. AIMS: The aim of our study is to develop a less invasive procedure for endoscopic optic nerve decompression. MATERIALS AND METHODS: We proposed a modified trans-sphenoidal surgical procedure for endoscopic opt

Sphenoid and optic nerve sheath meningioma revealed by recurrent brain infarctions.

Meningioma, though benign, may invade adjacent structures such as bone, soft tissues, dural sinuses and arteries. However brain infarctions secondary to meningioma involving the cavernous sinus and encasing and narrowing the intracranial carotid artery are rare. We report the case of a young man with recurrent left carotid artery infarctions due to a left sphenoid meningioma infiltrating the posterior optic nerve sheath through the optic canal and circumscribing the intracranial carotid artery. The patient had a gradually progressive occlusion of the middle cerebral artery, the distal internal

Mucoceles of the sphenoid sinus.

Mucoceles of the sphenoid sinuses are uncommon; 60 cases have been reported in the world literature to date. Signs and symptoms are caused by local expansion of the mucocele and include headache (the most common symptom), ocular field deficits, external ophthalmoplegia (particularly the sixth cranial nerve), and proptosis. Radiologic correlation in the form of plain films and polytomorgraphy is the most reliable guide in the diagnosis of sphenoid sinus disease. Therapy of these lesions is surgical, and drainage can be achieved via the external ethmoid or sublabial transseptal route. Three case

Changes in paranasal sinus abnormalities found incidentally on MRI.

Abnormal opacification of paranasal sinuses on brain MRI was studied in 178 consecutive adult patients who underwent two examinations separated by at least 4 months because of suspected neurological disease. Sinus abnormality was present in 79 (44%) patients; in 60% the abnormality involved two or more sinuses and it was bilateral in 51%. The most frequently abnormal sinus was the maxillary (46%), followed by the ethmoid (32%), sphenoid (16%), and frontal sinuses (6%). Interval occurrence or complete resolution of abnormalities was observed in 52% of patients (66% of the sinuses). In the remai

MRI of the paranasal sinuses: incidental abnormalities and their relationship to symptoms.

Magnetic resonance imaging is able to demonstrate a wide range of sinus abnormalities. Incidental findings in patients referred for neuroradiology because of suspected intracranial pathology are surprisingly common and were present in 37.5 per cent of 483 images examined. The maxillary and ethmoid sinuses were most commonly affected with 27 and 26 per cent of images abnormal whereas the frontal and sphenoid sinuses were less commonly affected with 5 per cent of each abnormal. If minor changes were excluded then 17 per cent of patients had either fluid, a polyp or marked mucosal thickening in a

How to manage patients with hard-to-recognize postnasal drip?

Postnasal drip (PND), commonly regarded as a phenomenon wherein nasal fluid drips into the pharynx, is one of the main symptoms of chronic sinusitis and other nasal lesions. This definition is controversial, however, because some patients have PND with no evidence of fluid either in the oropharynx or around the choanae. Among 220 patients in whom PND was diagnosed at the University of Tokyo Hospital between January 1996 and December 1997, 19 (8.6%) had hard-to-recognize PND on routine examination. Through careful observation, responsible lesions could be identified in all patients with hard-to

Ventilation of the paranasal sinuses studied with dynamic emission computer tomography.

This investigation's aim was to study a noninvasive method for determining the ventilation of the nasal sinuses. For this purpose, different insufflation techniques using xenon 133 were tried, along with different scintillation camera techniques. Xenon 133 gas was used for insufflation in the nose and sinuses, and the half-time of the washout curves was determined with the aid of digital scintillation cameras, one of which was capable of dynamic tomography. The tomographic procedure was superior because it made it possible to study the washout from the ethmoidal/sphenoidal sinuses as well. The
