Detection of the genial spinal canal in atrophic mandibles with a CBCT: a cadaver study.

OBJECTIVES: The use of a single midline implant to retain a complete mandibular denture when more implants cannot be used is an incipient treatment modality. However, in the mandibular symphysis, the genial spinal canal (GSC) is an anatomical structure with neurovascular content that can be harmed during dental implant surgery. The purpose of the present study was to use CBCT of edentulous atrophic cadaver mandibles and evaluate how often the simulated placement of a single midline implant would contact the GSC if present. METHODS: CBCT scans of 47 edentulous cadaver mandibles were performed.

A glass foreign body migrating into the lumbar spinal canal: a case report.

We report on a 12-year-old boy in whom a glass fragment migrated into the lumbar spinal canal causing low back pain and radiculopathy 6 years after injury. Computed tomography showed a foreign body in the anterior spinal canal of L5/S1. The patient underwent recapping T-saw laminoplasty to remove the foreign body. Scar formation was noted at the anterior dura. The glass fragment seemed to have penetrated into the intradural space from an anterior site. Postoperatively, the pain resolved immediately. At the 9-year follow-up, the alignment of the lumbar spine and healing of L5 was good, with no

Spinal canal capacity in simulated displacements of the atlantoaxial segment: a skeletal study.

In normal, physiological circumstances there is ample room in the spinal canal to accommodate the spinal cord. Our study aimed to identify the degree of compromise of the spinal canal which could be anticipated in various atlantoaxial pathological states. We examined paired atlas and axis vertebrae using high-definition radiography and simultaneous photography in both normal and simulated pathological orientations in order to measure the resultant dimension of the spinal canal and its percentage occlusion. At the extreme of physiological axial rotation (47 degrees) the spinal canal is reduced

Dynamic changes in the dimensions of the lumbar spinal canal: an experimental study in vitro.

The variation in the dimensions of the lumbar spinal canal under both flexion-extension and axial compression-distraction was studied using computerized tomography (CT) scans in human cadaver lumbar spine specimens. In 3-mm-thick CT slices through the disk at L3-L4, the cross-sectional area of the spinal canal was reduced by around 40 mm2, corresponding to a 16% reduction of the initial area when the lumbar spines were moved both from flexion to extension and from distraction to compression. A corresponding reduction in the midsagittal diameter of the canal of 2 mm was found. During these moti

[An autopsy case of malignant schwannoma in the brachial plexus extending into the spinal canal].

A 40-year-old woman had pain and numbness in the left upper extremity, on Dec. 1982. These symptoms progressed gradually. She was admitted to our department on Jan. 26, 1984 because of neck pains and progressive paraplegia. Neurological examination revealed monoplegia of the left arm with absent DTRs, spastic paraplegia, sensory disturbances below Th 11 level and severe sphincter disturbance. A metrizamide myelogram showed intradural extramedullary mass at C 4-7. Laminectomy was immediately performed from C 3 to C 7. A soft and brown tumor was attached at the ventral aspect of the cord at C 3

Single-Level Versus Two-Level Decompression for Adjacent Segment Spinal Stenosis : A Spinal Canal Area-Matched Comparative Study.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To investigate clinical outcomes between lumbar decompression without fusion by matching canal area between the adjacent level in one-level decompression and the upper level in two-level decompression procedures. SUMMARY OF BACKGROUND DATA: In patients with multilevel lumbar stenosis, surgeons face the dilemma of whether to decompress only the symptomatic level or to prophylactically include adjacent stenotic segments. Current literature shows little consensus on this decision, with comparative studies indicating that single-level and two-

Lumbar spinal surgery improves locomotive syndrome in elderly patients with lumbar spinal canal stenosis: A multicenter prospective study.

BACKGROUND: Owing to musculoskeletal dysfunction, locomotive syndrome elevates the risk of requiring nursing care. Among degenerative musculoskeletal disorders, lumbar spinal canal stenosis (LSS) associates with locomotive syndrome; however, whether lumbar spinal surgery for LSS improves locomotive syndrome remains unclear. Hence, this study aimed to identify the efficacy of lumbar spinal surgery on locomotive syndrome among elderly patients with LSS. METHODS: We prospectively collected the clinical data from multiple institutions of patients (age >65 years) who underwent lumbar spinal surger

Factors associated with improvement in sagittal spinal alignment after microendoscopic laminotomy in patients with lumbar spinal canal stenosis.

OBJECTIVE Little is known about the relationship between sagittal spinal alignment in patients with lumbar spinal canal stenosis (LSS) and objective findings such as spinopelvic parameters, lumbar back muscle degeneration, and clinical data. The purpose of this study was to identify the preoperative clinical and radiological factors that predict improvement in sagittal spinal alignment after decompressive surgery in patients with LSS. METHODS The records of 61 patients with LSS who underwent microendoscopic laminotomy and had pre- and postoperative clinical data collected were retrospectively

Morphological analysis of the cervical spinal canal, dural tube and spinal cord in normal individuals using CT myelography.

To verify the conventional concept of "developmental stenosis of the cervical spinal canal", we performed a morphological analysis of the relations of the cervical spinal canal, dural tube and spinal cord in normal individuals. The sagittal diameter, area and circularity of the three structures, and the dispersion of each parameter, were examined on axial sections of CT myelograms of 36 normal subjects. The spinal canal was narrowest at C4, followed by C5, while the spinal cord was largest at C4/5. The area and circularity of the cervical spinal cord were not significantly correlated with any

Spinal canal restoration by posterior distraction or anterior decompression in thoracolumbar spinal fractures and its influence on neurological outcome.

Thirty consecutive patients who had suffered unstable fractures and dislocations of the thoracolumbar spine mostly associated with neurologic impairment and bony encroachment on the spinal canal were treated either with Harrington distraction rods combined with sublaminar wires or with the Zielke-VDS device. These patients were subsequently assessed for neurologic outcome, spinal canal clearance, sagittal and coronal spinal deformity correction preoperatively and postoperatively with a minimum follow-up of 26 months. In the follow-up evaluation, the patients who underwent surgery with Harringt

The relationship of developmental narrowing of the cervical spinal canal to reversible and irreversible injury of the cervical spinal cord in football players.

An evaluation of forty-five athletes who had had an episode of transient neurapraxia of the cervical spinal cord revealed a consistent finding of developmental narrowing of the cervical spinal canal. The purpose of the present epidemiological study was to determine the relationship, if any, between a developmentally narrowed cervical canal and reversible and irreversible injury of the cervical cord with use of various cohorts of football players as well as a large control group. Cohort I comprised college football players who were asymptomatic and had no known history of transient neurapraxia

Disc bulging predicts poor outcomes of decompression surgery for lumbar spinal canal stenosis.

PURPOSE: Lumbar spinal canal stenosis (LSCS) is a prevalent degenerative musculoskeletal disorder in aging populations. While decompression surgery is a standard treatment, some patients require revision surgery. Disc bulging, a distinct component of the degenerative process separate from disc herniation, contributes to spinal stenosis. However, its impact on surgical outcomes for LSCS has never been examined. This study aimed to elucidate the influence of disc bulging on outcomes of decompressive surgery for LSCS, addressing a critical knowledge gap. METHODS: This retrospective study analyze
