Sexuality in Pediatric Spinal Cord Injury.

Sexual development and sexuality in youth with pediatric spinal cord injury (SCI) are critical areas clinicians must be aware of and discuss when working with youth and their families. In addition to the general sexuality issues and challenges of adolescence and adult development, youth with SCI face unique physical and psychosocial issues. The goal of this article is to provide a developmentally based discussion of sexuality in individuals with SCI from infancy through emerging adulthood. An overview of psychosocial issues related to sexual development and sexuality are presented for each sta…

Osteochondroma causing cervical spinal cord compression.

Osteochondromas are the most common benign tumors of the bone. They occur in young adolescent patients and are frequently located in the metaphyses of the long bones; they do not grow after skeletal maturity. The incidence of osteochondroma in the spine is reported to be rare. Moreover, patients with spinal osteochondroma who develop symptoms of myelopathy are extremely rare. We report the case of an 8-year-old girl who experienced myelopathy due to spinal compression of the cervical osteochondroma. This case suggests that if a cartilage cap is observed on the spinal canal with magnetic resona

Inflammogenesis of Secondary Spinal Cord Injury.

Spinal cord injury (SCI) and spinal infarction lead to neurological complications and eventually to paraplegia or quadriplegia. These extremely debilitating conditions are major contributors to morbidity. Our understanding of SCI has certainly increased during the last decade, but remains far from clear. SCI consists of two defined phases: the initial impact causes primary injury, which is followed by a prolonged secondary injury consisting of evolving sub-phases that may last for years. The underlying pathophysiological mechanisms driving this condition are complex. Derangement of the vascula…

Acute complications of spinal cord injuries.

The aim of this paper is to give an overview of acute complications of spinal cord injury (SCI). Along with motor and sensory deficits, instabilities of the cardiovascular, thermoregulatory and broncho-pulmonary system are common after a SCI. Disturbances of the urinary and gastrointestinal systems are typical as well as sexual dysfunction. Frequent complications of cervical and high thoracic SCI are neurogenic shock, bradyarrhythmias, hypotension, ectopic beats, abnormal temperature control and disturbance of sweating, vasodilatation and autonomic dysreflexia. Autonomic dysreflexia is an abru…

Autonomic dysreflexia in spinal cord injury.

Autonomic dysreflexia is a medical emergency occurring after spinal cord injury caused by disruption of the normal autonomic responses to a stimulus below the level of spinal cord lesion. Although it can lead to stroke, convulsions, cardiac arrest and death, health professionals are largely ignorant of the condition and it is frequently misdiagnosed. This article gives an overview of autonomic dysreflexia, along with how it can be diagnosed and treated.

Spinal cord injury medicine and rehabilitation.

The rehabilitation of spinal cord injury (SCI) is a complicated process, but one in which new research is developing novel and increasingly promising methods of restorative neurology. Spinal cord injury medicine addresses not only the neurologic injury, but all the secondary complications in other organ systems whose regulation is disrupted after SCI. To some degree, the rehabilitation of SCI is focused on return to the community and functional goals are paramount, regardless of whether they can be achieved through some mechanism of compensation or due to a growing effort at engendering neurol…

Neurophysiology of complex spinal cord untethering.

Surgery of complex spinal dysraphisms can be challenging. A number of surgical maneuvers can place the conus and the cauda equina at risk for neurological injury during cord untethering, and the identification of functional neural structures within the lumbosacral region is often not possible solely on the basis of anatomy. Therefore, the assistance of intraoperative neurophysiological monitoring can be invaluable during these procedures. We describe the intraoperative neurophysiological monitoring strategy developed at our institution over the past 12 years when dealing with tethered cord sur

Diaphragmatic pacing in spinal cord injury.

After cervical spinal cord injuries, many patients are unable to sustain independent ventilation because of a disruption of diaphragm innervation and respiratory functioning. If phrenic nerve function is preserved, the patient may be able to tolerate exogenous pacing of the diaphragm via electrical stimulation. Previously this was accomplished by stimulation directly to the phrenic nerves, but may be accomplished less invasively by percutaneously stimulating the diaphragm itself. The benefits, when compared with mechanical ventilation, include a lower rate of pulmonary complications, improved …

Spinal cord ischemia after aortic surgery.

This article discusses the etiology of spinal cord ischemia (SCI) mechanisms that may lead to paraplegia during open and endovascular repair from an anatomical and physiological perspective as well as the role of various protective measures used in prevention of this dreadful complication of aortic surgery. There are many adjuncts that must be considered to reduce the risk of spinal cord injury, such as revascularisation of intercostal arteries, maintenance of high mean blood pressure, spinal cord drainage and a few new promising models like NIRS and MISACE which usefulness is yet to be determ…

Autonomic consequences of spinal cord injury.

Spinal cord injury (SCI) results not only in motor and sensory deficits but also in autonomic dysfunctions. The disruption of connections between higher brain centers and the spinal cord, or the impaired autonomic nervous system itself, manifests a broad range of autonomic abnormalities. This includes compromised cardiovascular, respiratory, urinary, gastrointestinal, thermoregulatory, and sexual activities. These disabilities evoke potentially life-threatening symptoms that severely interfere with the daily living of those with SCI. In particular, high thoracic or cervical SCI often causes di…
