Nasopharyngeal colonization of infants in southern India with Streptococcus pneumoniae.

To investigate the dynamics of nasopharyngeal colonization with Streptococcus pneumoniae, and to determine the prevalent serogroups/types (SGT) and their antimicrobial susceptibility, we studied 100 infants attending our well-baby clinic. Nasopharyngeal swab specimens were obtained at 6, 10, 14, 18 and 22 weeks and at 9 and 18 months of age and submitted for culture, serotyping and antimicrobial susceptibility testing of S. pneumoniae. Colonization with pneumococcus was seen on at least one occasion in 81 infants. The median age of acquisition was 11 weeks and the median duration of carriage w…

A surveillance study on penicillin-resistant streptococcus pneumoniae in China.

OBJECTIVES: To find out whether there are also penicillin-resistant streptococcus pneumoniae and the resistant rate in China. METHODS: A surveillance study which is a part of the international surveillance on pneumococci resistance to penicillin and other antimicrobial agents was conducted in Beijing, China. More than 900 pediatric patients with respiratory tract infections aged from six months to three years selected from two pediatric units were enrolled in the study. Perthroat swabs were immediately streaked onto blood agar plates. Isolates were identified as pneumococci by their typical a…

Streptococcus pneumoniae: Activity of Newer Agents Against Penicillin-Resistant Strains.

Strains of pneumococci resistant to antimicrobial agents have been reported on all continents. In 1997, more than 50% of strains in the United States were not susceptible to penicillin, and 30% were resistant to macrolides. In addition, many strains are resistant to multiple agents, including beta-lactams, macrolides, clindamycin, chloramphenicol, tetracyclines, and trimethoprim-sulfamethoxazole. Although resistance to beta-lactams in nonmeningeal infections can usually be overcome by parenteral administration, clinically significant resistance is an important limitation in meningitis and with

Characterization of IS1515, a functional insertion sequence in Streptococcus pneumoniae.

We describe the characterization of a new insertion sequence, IS1515, identified in the genome of Streptococcus pneumoniae I41R, an unencapsulated mutant isolated many years ago (R. Austrian, H. P. Bernheimer, E. E. B. Smith, and G. T. Mills, J. Exp. Med. 110:585-602, 1959). A copy of this element located in the cap1EI41R gene was sequenced. The 871-bp-long IS1515 element possesses 12-bp perfect inverted repeats and generates a 3-bp target duplication upon insertion. The IS encodes a protein of 271 amino acid residues similar to the putative transposases of other insertion sequences, namely IS…

Bilateral vocal cord paralysis after meningitis due to Streptococcus pneumoniae.

We report the clinical course of a 15-month-old boy who had fever, decreased activity, and weakness, with severe respiratory distress during transport to the hospital. Laboratory evaluation confirmed the diagnosis of meningitis due to Streptococcus pneumoniae. He was intubated on arrival and required 4 days of ventilatory support. Soon after extubation, he had marked stridor and dyspnea that were unresponsive to standard therapy with nebulized racemic epinephrine and intravenous dexamethasone. Magnetic resonance imaging of the brain revealed nonspecific findings, and airway endoscopy showed bi…

Surveillance for penicillin-nonsusceptible Streptococcus pneumoniae--New York City, 1995.

Streptococcus pneumoniae has become a leading cause of bacteremia, pneumonia, meningitis, and otitis media in the United States. Persons at increased risk include young children, immunocompromised persons, and the elderly. Until 1987, S. pneumoniae was uniformly susceptible to penicillin; since then, in the United States, there has been increased identification of penicillin-nonsusceptible S. pneumoniae (PNSP) (defined as minimum inhibitory concentration [MIC] to penicillin > or = 0.1 microgram/mL), especially penicillin-resistant S. pneumoniae (PRSP) (defined as MIC to penicillin > or = 2.0 m…

Limited diversity of Streptococcus pneumoniae psaA among pneumococcal vaccine serotypes.

The pneumococcal surface adhesin A (PsaA) is a surface-exposed protein of the gram-positive bacterium Streptococcus pneumoniae. It belongs to a group of proteins designated the lipoprotein receptor I antigen family. The gene encoding PsaA from an encapsulated strain of pneumococcal serotype 6B was cloned and sequenced. The peptide sequence was compared to that of homologs found in S. pneumoniae serotype 2, viridans streptococci, and Enterococcus faecalis. Identity values among the deduced peptides ranged from 57 to 98%. The polymorphism of psaA was examined among the 23 encapsulated vaccine se…

Emergence of multiple-antibiotic-resistant Streptococcus pneumoniae in Hong Kong.

Two hundred four strains of Streptococcus pneumoniae isolated in Hong Kong from January 1993 to May 1995 were analyzed for their antibiotic susceptibilities and epidemiological patterns. The ages of the patients from whom the strains were isolated from 1 month to 93 years (median, 53 years); the male-to-female ratio was 2.8, with a predominance of males in the pediatric group. Fifty-nine (28.9%) strains showed reduced penicillin susceptibility, including 40 (19.6%) with frank penicillin resistance (MIC > 1 microgram/ml). Tetracycline resistance alone was found in 28.4% of strains. Isolates wit…

Increasing resistance to penicillin in Streptococcus pneumoniae in southern Sweden.

The susceptibility to penicillin of 6 prevalent pneumococcal types isolated from nasopharynx in 1992 was compared with that of corresponding types from 1980-82. The 6 types or groups, 6, 9, 14, 15, 19 and 23, constituted 78% of consecutive isolates. 19/204 isolates in 1992 were intermediately resistant (MIC 0.12-1.0 mg/l) in comparison with 1/194 from 1980-82 (p < 0.001). Resistant strains (MIC > or = 2 mg/l) were not found. Of group 15, no fewer than 10/31 isolates were intermediately resistant, which may support the clonal origin and spread of penicillin-resistant pneumococci. At least 5.0%

[Resistance of Streptococcus pneumoniae to penicillin. "Bacteriological" resistance/"clinical" resistance].

From January 1988 to December 1992, 143 Streptococcus pneumoniae strains have been isolated at a significant level from 136 patients hospitalized in the pulmonary care unit of CHI Créteil. The clinical presentations were: 83 pneumonia (P), 48 acute bronchitis (B) and 12 colonizations (C). Fourteen strains were found to be penicillin resistant (MIC > 0.125 mg. l-1): 11 P, 2 B, 1 C. Two patients were lost of view after the beginning of the treatment: a pneumonia (Serotype 23F, MIC = 2 mg. l-1) and a bronchitis (Serotype 23F, MIC = 2 mg. l-1). Treatment was pristinamycine for one and amoxicilline…

[Vancomycin in meningitis caused by penicillin G resistant Streptococcus pneumoniae].

We report two cases of penicillin G-resistant pneumococcal meningitis in adults, with clinical and bacteriological failure of amoxicillin and negative or incomplete response to third generation cephalosporins. Meningitis occurred in a man treated for myeloma and in an elderly woman under prolonged intermittent amoxicillin therapy for chronic otitis. Such situations are known as exposing to pneumococcal meningitis and to resistance of the strain involved to penicillin G. Both patients were cured by vancomycin in continuous infusion associated with rifampicin or fosfomycin. Contrary to third gen

Identifying non-capsulate strains of Streptococcus pneumoniae isolated from eyes.

Over six months 10 biochemically and physiologically atypical strains of Streptococcus pneumoniae were isolated from eye swabs. Conventional methodology showed that these strains possessed characteristics of both S pneumoniae and other alpha haemolytic streptococci. The use of sodium dodecyl sulphate - polyacrylamide gel electrophoresis (SDS-PAGE) enabled us to characterise these strains as S pneumoniae. Loss of the capsule seemed to be associated with atypical biochemical properties.

Facilitation of plasmid transfer in Streptococcus pneumoniae by chromosomal homology.

The frequency of plasmid establishment in the transformation of Streptococcus pneumoniae by plasmid DNA was increased more than 10-fold when the plasmid carried DNA homologous to the host chromosome. Perfect homology was not necessary for such facilitation; small additions or deletions were tolerated, but extensive deletions in the homologous segment of either plasmid or chromosome reduced or eliminated facilitation. The facilitated plasmid transfer showed a linear dependence on monomeric plasmid concentration rather than the quadratic dependence found in the absence of homology, which indicat…

Lack of S100A8 Impairs Lung-Protective Immunity Against Streptococcus pneumoniae.

BACKGROUND: We recently showed that S100A9 is indispensable for lung antibacterial immunity, making it crucial for the survival of pneumococcal pneumonia. However, the role of S100A8 in lung antibacterial immunity is ill-defined. METHODS: S100A8 levels in the bronchoalveolar lavage fluid (BALF) of patients with pneumonia were quantified by enzyme-linked immunosorbent assay. Wild type and S100A8 knockout mice were orotracheally infected with Streptococcus pneumoniae, and bacterial clearance, disease progression, lung histopathology, and leukocyte recruitment were analyzed at defined time point…

Structural and Functional Interrogation of Active Streptococcus pneumoniae Sortase A.

Sortase A (SrtA) enzymes covalently anchor surface proteins to Gram-positive bacterial cell walls, promoting colonization and virulence. In Streptococcus pneumoniae, previous studies identified both a domain-swapped dimer and an active refolded monomer, but the active enzyme has not been characterized at the structural and residue-specific level. Here, we performed quantitative proteomic comparisons of wild-type and SrtA knockout strains that confirmed the loss of multiple LPxTG-containing virulence factors, including ZmpB, NanA, and IgA1 protease, consistent with an essential role for SrtA in…

Cellular immunity to Streptococcus pneumoniae: implications for vaccine-mediated prevention.

INTRODUCTION: Streptococcus pneumoniae asymptomatically colonizes the nasopharynx but can also cause mucosal infections or invasive pneumococcal disease. Despite the significant positive impact of pneumococcal conjugate vaccines, the burden of pneumococcal disease persists, reflecting the inherent limitations of a strategy that elicits serotype-dependent humoral immunity against a pathogen with > 100 known serotypes. AREAS COVERED: Animal model data suggest that in addition to anti-capsular antibodies, CD4+ T-cell responses (including Th1 and Th17) and CD8+ T cells may also contribute to prot…

Cyclic Di-AMP Affects Cell Membrane Integrity of Streptococcus pneumoniae.

Competence is an important bioprocess for Streptococcus pneumoniae. Previously, we demonstrated that the bacterial second messenger cyclic di-adenosine monophosphate (c-di-AMP) modulates pneumococcal competence. Surprisingly, cdaA*, a strain producing less c-di-AMP due to a point mutation in the diadenylate cyclase CdaA, is susceptible to competence-stimulating peptide (CSP). In this study, we screened cdaA* suppressor mutants resistant to CSP to explore the underlying mechanism. Of 14 clones sequenced, nine clones possessed mutations in the c-di-AMP phosphodiesterase Pde1, indicating that the…

Detection of pathogens and vaccine design strategies for Streptococcus pneumoniae.

Streptococcus pneumoniae, often referred to as S. pneumoniae, plays a critical role in numerous infectious conditions, notably targeting the respiratory system with diseases such as lobar pneumonia, bronchitis, and pleurisy. This bacterium is the predominant cause of bacterial pneumonia in children and a major agent of bacterial meningitis in those over three months of age. Moreover, S. pneumoniae accounts for significant childhood mortality, with approximately one million deaths each year among children younger than five globally, as reported by the World Health Organization. These infections…
