Stuttering treatment and brain research in adults: A still unfolding relationship.

PURPOSE: Brain imaging and brain stimulation procedures have now been used for more than two decades to investigate the neural systems that contribute to the occurrence of stuttering in adults, and to identify processes that might enhance recovery from stuttering. The purpose of this paper is to review the extent to which these dual lines of research with adults who stutter have intersected and whether they are contributing towards the alleviation of this impairment. METHOD: Several areas of research are reviewed in order to determine whether research on the neurology of stuttering is showing…

Brief report: treating stuttering in an adult with autism spectrum disorder.

Stuttering and autism can co-occur and when they do it presents a significant communication challenge. This study examined the effectiveness of a modified version of the fluency rules program (FRP; Runyan and Runyan, Stuttering and related disorders of fluency, in 2007) to reduce stuttering frequency in a man with autism spectrum disorder (ASD). The participant's percentage of stuttered words (%SW) was calculated during conversational interactions with multiple conversation partners both within and outside of the clinic treatment sessions. Visual inspection methods revealed a reduction in %SW …

Stuttering inhibition via visual feedback at normal and fast speech rates.

BACKGROUND: Immediate and drastic reductions in stuttering are found when speech is produced in conjunction with a variety of second signals (for example, auditory choral speech and its permutations, and delayed auditory feedback). Initially, researchers suggested a decreased speech rate as a plausible explanation for the reduction in stuttering as people who stutter produced speech under second signals. However, this explanation was refuted by research findings that demonstrated reductions in stuttering at both normal and fast speech rates under second signals. Recent studies have also demons…

Past speech therapy experiences of individuals exploring a new stuttering treatment.

Perceptions of benefits of speech therapy, success of therapy across clinical settings, reasons for returning to therapy, client-clinician relationships, and clinicians' competency were assessed in 57 participants (47 men, 10 women; M age = 34 yr.) trying a new therapy. A majority of respondents had cumulatively five or more years in therapy and at least two stuttering therapies. Respondents rated university or hospital settings as more successful than services in public schools. 70% of the respondents noted difficulty communicating basic needs; they blamed themselves for inability to maintain…

Developmental and persistent developmental stuttering: an overview for primary care physicians.

Stuttering is a speech disorder characterized by a disruption in the fluency, timing, and rhythm of normal speech. It affects approximately 5% of children at some point in their lives. Although dysfluency often resolves before adulthood, it may cause periods of extreme anxiety for patients, especially those who continue to stutter in adolescence and adulthood. Although these patients are unlikely to stop stuttering, treatment options are available to reduce anxiety and therefore the severity of symptoms. In the present review article, the authors discuss the pathophysiology, diagnosis, and man…

A dialogue among various cultures and its manifestation in stuttering therapy.

UNLABELLED: This paper presents a historical retrospective of the evolution of the clinical aspects of stuttering, and refers to social, cultural, political, and economic variables that might have exerted an influence on this evolution, particularly in relation to the authors' experience in Israel. The retrospective commences in the early decades of the 20th century, when speech and language pathology did not exist as a profession, and presents the major approaches to therapy in children and adults employed by clinicians throughout the century. We focused on the impact of the change that occur…

Prevalence of anxiety disorders among adults seeking speech therapy for stuttering.

The present study explored the prevalence of anxiety disorders among adults seeking speech therapy for stuttering. Employing a matched case-control design, participants included 92 adults seeking treatment for stuttering, and 920 age- and gender-matched controls from the Australian National Survey of Mental Health and Well-being. A conditional logistic regression model was used to estimate odds ratios for Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) and International Classification of Diseases (ICD-10) anxiety disorders. Compared with matched controls, the stu…

Screening for personality disorders among adults seeking speech treatment for stuttering.

UNLABELLED: Stuttering is frequently associated with negative consequences which typically begin in early childhood. Despite this, no previous studies have investigated the presence of personality disorders among adults who stutter. Therefore, the aims of the present study were to screen for personality disorders among adults who stutter, and to compare these screening estimates with matched controls from a national population sample. Using a matched case-control design, participants were 94 adults seeking treatment for stuttering, 92 of whom completed the International Personality Disorders E…

[Gray matter abnormalities in developmental stuttering determined with voxel-based morphometry].

OBJECTIVE: To investigate the differences of regional grey matter volume between adults with persistent developmental stuttering and fluent speaking adults, and to determine whether stutterers have anomalous anatomy of speech-relevant brain areas that possibly affect speech fluency. METHODS: High-resolution magnetic resonance imaging (MRI) scanning was performed on 10 adults with developmental stuttering, aged 26 (21 - 35) with the onset age of 4 (3 - 7) and 12 age, sex, hand preference, and education-matched controls. The customized brain templates were created in order to improve spatial no…

Dissecting choral speech: properties of the accompanist critical to stuttering reduction.

UNLABELLED: The effects of choral speech and altered auditory feedback (AAF) on stuttering frequency were compared to identify those properties of choral speech that make it a more effective condition for stuttering reduction. Seventeen adults who stutter (AWS) participated in an experiment consisting of special choral speech conditions that were manipulated to selectively eliminate specific differences between choral speech and AAF. Consistent with previous findings, results showed that both choral speech and AAF reduced stuttering compared to solo reading. Although reductions under AAF were …

Acute transient cerebellar dysfunction and stuttering following mild closed head injury.

INTRODUCTION: A wide range of pathologies can cause cerebellar dysfunction but there have been few reports of transient dysfunction after mild head injury. There are none describing stuttering as an acute symptom after such injuries. CASE REPORT: We report a 12-year-old boy who presented with headache, stuttering speech and truncal and bilateral peripheral cerebellar dysfunction after falling prone from his skateboard. Computed tomographic brain imaging showed normal results. All clinical symptoms and signs had resolved within 24 h obviating further investigation. A follow up clinical intervi…

The modification of speech naturalness during rhythmic stimulation treatment of stuttering.

This study investigated the modification of speech naturalness during stuttering treatment. It systematically replicated an earlier study (Ingham & Onslow, 1985) that demonstrated that unnatural-sounding stutter-free speech could be shaped into more natural-sounding stutter-free speech by using regular feedback of speech-naturalness ratings during speaking tasks. In the present study, the some procedure was used with three persons who stutter-2 adolescent girls and 1 adult man-during rhythmic stimulation conditions. The two adolescent participants spoke only English, but Spanish was the first …

Effect of monaural and binaural altered auditory feedback on stuttering frequency.

The effect of monaural and binaural alterations in auditory feedback on stuttering frequency was investigated. Eleven participants who stutter read aloud under nonaltered auditory feedback (NAF) and monaural and binaural conditions of frequency altered feedback [(FAF), on-quarter octave shift upward] and delayed auditory feedback [(DAF), 50-ms delay] at a normal speech rate. Relative to the NAF condition, reductions in stuttering frequency of approximately 60%-75% were found with the altered auditory feedback conditions. Post hoc single-df comparisons revealed a reduction in stuttering frequen…

Parent verbal interactions and speech rate: a case study in stuttering.

This single-case study of indirect stuttering treatment is intended to identify variables for further research. The study used post hoc analyses of changes in parent speech variables and changes in the child's stuttering. The analysis was conducted in two parts. Study I examined the relationships between the child's percent syllables stuttered and the parents' speech rates and percentages of nonaccepting statements, interruptions, questions, nonaccepting questions, and talk time. The only parent variable significantly correlated with the child's stuttering was the mother's speech rate. In Stud…

Choosing a treatment procedure for early stuttering: issues and future directions.

The new responsibility of speech-language pathologists to provide direct, early intervention for stuttering creates a further responsibility for them to choose justifiable treatment procedures for that intervention. This paper has two purposes. The first is to encourage clinicians to evaluate the conceptual and practical aspects of the treatments they use for early stuttering. The second purpose of the paper is to overview available early intervention procedures and consider the advantages, disadvantages, and prominent issues associated with each. The procedures considered are environment mani…
