LeFort partial colpocleisis: tips and technique.

INTRODUCTION AND HYPOTHESIS: We present a video describing the technical considerations for performing a LeFort colpocleisis. METHODS: A 79-year-old woman presented with a symptomatic vaginal bulge. She was not sexually active, and had no desire to maintain the vaginal canal. Her history was significant for aortic valve replacement, chronic anticoagulation, and a cardiac pacemaker. She had uterine procidentia, with occult stress urinary incontinence. After discussing options, she elected to undergo LeFort colpocleisis. Following sharp endometrial curettage, hydro-dissection was performed with

Obturator nerve injury: a rare complication of retropubic tension-free vaginal sling.

The tension-free vaginal tape (TVT) retropubic sling is a very effective treatment for stress urinary incontinence. High cure rates are typically achieved, and the reported complication rate is relatively low. Obturator neuralgia secondary to insertion of a midurethral sling is a rare and specific type of chronic pain that is more commonly associated with transobturator tape slings. The purpose of this video case report was to demonstrate that obturator nerve injury is a possible complication of TVT retropubic slings placement. A discussion of symptoms and signs of obturator nerve injury and h

Female stress urinary incontinence and the mid-urethral sling: is obstruction necessary to achieve dryness?

BACKGROUND: Recently, the American Urogynecologic Society and Society of Urodynamics, Female Pelvic Medicine and Urogenital Reconstruction released position statements on the use of mid-urethral slings. The statement offers that the polypropylene mesh mid-urethral sling (retropubic and transobturator) is now the recognized worldwide standard of care for the surgical treatment of stress urinary incontinence. The purpose of the current manuscript is to examine whether the polypropylene mesh mid-urethral sling should be the standard of care. METHODS: Data for this review were acquired by a syste

Female urinary stress incontinence.

Urinary incontinence (UI) is more common than any other chronic disease, such as hypertension, depression or diabetes, with the prevalence estimated between 9 and 74%. Among the various forms of urinary incontinence, stress incontinence (SUI) is the most prevalent (50%), with urgency incontinence (UUI) representing 11% and mixed type (MUI) 36% (3% not classified). Nowadays, the restoration of urinary continence is one of the greatest challenges for the well-being and quality of life of women. The introduction of minimally invasive surgical procedures changed the anti-incontinence surgery, lead

Minimally invasive techniques for female stress urinary incontinence, how, why, when.

AIM: The main purpose of this paper is to summarize the recent experience that has been obtained till now in the surgical treatment of urinary stress incontinence with less invasive techniques. MATERIALS AND METHODS: We used current literature and papers published in MEDLINE and Cochrane library. The keywords used for this review were Stress urinary incontinence, Tension-free vaginal tape (TVT), Transobturator tape (TOT, TVT-O), and Single-incision mini-slings (SIMS). RESULTS: Tension-free vaginal tape and the transobturator urethral suspension are the most commonly used surgical approach fo

A life-threatening hematoma after the single-incision sling MiniArc procedure: a case report.

The newer generation of single-incision slings such as the MiniArc may minimize these risks by avoiding the passage of needles through the obturator foramen and muscles. However, they may still lead to severe vascular injuries. We report a case of a life-threatening obturator hematoma forming after a MiniArc procedure. We determined the exact location using a computed tomographic scan of the pelvis, and it was surgically managed. The patient's symptoms subsided, and her vital signs stabilized after the hematoma was removed using a transvaginal approach. Single-incision slings may be associated

Obturator approach in the surgical treatment of stress urinary incontinence using tension-free transvaginal tape.

The study reviews the surgical treatment results of urinary stress incontinence in the group of 51 female patients, in whom the tension-free transvaginal tape was placed beneath the middle part of urethra using obturator approach during the period from 2005 to 2009. The method of surgery applied in all patients was obturator approach ("inside-out" method sec. de Leval), using a synthetic tension-free transvaginal prolen tape. After the sub-urethral tunnel had been created by scissors, the obturator membrane was perforated, then the placement of wing guides followed, through which helical needl

Repeat synthetic mid urethral sling procedure for women with recurrent stress urinary incontinence.

PURPOSE: We reported and compared the outcomes of repeat mid urethral sling with primary mid urethral sling in women with stress urinary incontinence. MATERIALS AND METHODS: A total of 1,225 consecutive women with urodynamic stress incontinence underwent a synthetic mid urethral sling procedure (955 retropubic, 270 transobturator) at our institution between 1999 and 2007. Of the patients 91% (1,112) were interviewed via telephone call with a structured questionnaire and were included in the analysis. Mean +/- SD followup was 50 +/- 24 months (range 12 to 114). A comparison between repeat (77,

Symptoms of combined prolapse and urinary incontinence in large surgical cohorts.

OBJECTIVE: To estimate whether prolapse severity is a major contributor to urinary incontinence severity, as measured by validated incontinence questionnaires. METHODS: We analyzed data from two large female stress urinary incontinence (SUI) surgical cohorts: the Stress Incontinence Surgical Treatment Efficacy Trial (SISTEr) study (N=655) and the subsequent Trial of Mid-Urethral Slings (TOMUS) study (N=597). All participants completed a standardized baseline assessment including validated measures of symptom severity, quality of life, objective measures of urine loss (Urogenital Distress Inve

Two cases illustrating a potential difference between transobturator and retropubic slings.

The transobturator sling procedure is a relatively new technique as compared to the retropubic version. The functional differences between these two procedures are largely unknown. Two cases of failed transobturator slings are reported. In both cases, the procedure was done under local anesthesia and a cough stress test was performed to adjust the tape. The transobturator slings were unable to stop the leakage and we replaced them with the retropubic versions, which were successful. Even when pulled very tightly, some transobturator slings will fail to stop stress incontinence that is amenable

[Implants for genital prolapse : Contra mesh surgery].

Alloplastic transvaginal meshes have become very popular in the surgery of pelvic organ prolapse (POP) as did alloplastic suburethral slings in female stress incontinence surgery, but without adequate supporting data. The simplicity of the mesh procedure facilitates its propagation with acceptance of higher revision and complication rates. Since attending physicians do more and more prolapse surgeries without practicing or teaching alternative techniques, expertise in these alternatives, which might be very useful in cases of recurrence, persistence or complications, is permanently lost. It is…

Impact of Single-Incision Sling Placement on Female Sexual Function in Women with Stress Urinary Incontinence.

INTRODUCTION AND HYPOTHESIS: Stress urinary incontinence (SUI) is a common condition among women of childbearing age, often requiring surgical intervention. The midurethral sling (MUS), including first-generation tension-free vaginal tapes (TVT) and second-generation transobturator tapes (TOT, TVT-O), has long been the standard treatment. However, both approaches have associated risks, prompting the development of single-incision slings (SIS) as a third option. SIS have proven their efficacy in SUI treatment, but the impact of these slings on female sexual function specifically remains underex

Safety and Efficacy of Single Incision Sling Versus Midurethral Sling in the Treatment of Stress Urinary Incontinence: A Randomized Controlled Trial.

IMPORTANCE: This study compares the long-term efficacy and safety of an innovative single-incision sling (SIS) with the inside-out transobturator tape (TOT) sling in the treatment of female stress urinary incontinence. OBJECTIVES: Women with urodynamic stress urinary incontinence were randomized to either SIS or TOT groups and followed up for 4 years. The primary outcome was objective cure defined with a negative cough stress test result. Secondary outcomes involved subjective cure reported via patient's satisfaction scale, surgery complications, postoperative de novo urgency, and patient's l
