The cause of death in a 45-day-old male infant who was found apneic at home and died 21 hours later was disseminated intravascular coagulation (DIC). The patient was admitted to a hospital in a state of cardio-respiratory arrest. The initial diagnosis was interrupted sudden infant death syndrome. The patient remained apneic, and recurrent discharge of bloody stool was the dominant clinical findings. He died without spontaneous respiration being restored. The autopsy revealed hemorrhages in the lungs and the ventricular septum and the free wall of the left ventricle of the heart. Microscopicall…

BACKGROUND: Despite post-mortem examination and autopsy, many cases of sudden infant death (SID) remain unexplained. The aim of this study was to assess usefulness of CT-scan in the Sudden Infant Death Syndrome (SIDS).
POPULATION: Twenty-three cases of SIDS had a post-mortem CT-scan evaluation of skull and brain. The pictures were retrospectively reviewed by several independent radiologists who were unaware of the circumstances of death and results of autopsy.
RESULTS: Aspects of pneumatocele, probably due to lumbar puncture were found in 6 cases. The subarachnoid spaces appeared inexplicall…

BACKGROUND: The purpose of this study was to estimate the annual morbidity and mortality among fetuses and infants that can be attributed to … timated 1900 to 4800 infant deaths resulting from perinatal disorders, and 1200 to 2200 deaths from sudden infant death syndrome (SIDS).
CONCLUSIONS: Tobacco use is an important preventable cause of abortions, low birthweight, and deaths from perinatal disorders and SIDS. All pregnant women should be advised that smoking places their unborn children in danger. The l …

We studied 114 neonates by pneumocardiogram recordings in order to examine the effects of cocaine with and without opiate exposure on neonatal respiration, heart rate, apparent life threatening events (ALTE), and sudden infant death syndrome (SIDS). In full-term infants exposed to cocaine without opiates we found increased longest apnea duration and more episodes of bradycardia, but decreased periodic breathing and average heart rate than in control full-term infants. Term infants prenatally exposed to cocaine with opiates also had less periodic breathing. Preterm infants exposed to cocaine wi…

Risk factors for sudden infant death syndrome (SIDS) were studied among infants born to the nearly 56,000 women enrolled in the US Collaborative Perinatal Project from 1959 through 1966. The 193 SIDS cases identified in the cohort were compared with 1930 controls randomly selected from infants who survived the first year of life. The previously documented excess risk associated with black race disappeared after adjusting for maternal education and family income. Maternal smoking, maternal anaemia during pregnancy, and lack of early prenatal care were all positively associated with SIDS. After …

Milk from breast or baby formula is the exclusive source of nutrition for newborn infants. Short chain opioid peptides such as beta-casomorphins have been isolated from breast milk as well as baby formula. These biologically active peptides are absorbed from the gastrointestinal tract. In infants predisposed to respiratory apnea because of abnormal autonomic nervous system development and respiratory control mechanisms, opioid peptides derived from milk might be one of the etiological factors for sudden infant death syndrome and near miss sudden infant death syndrome.

Since the value of home apnea monitoring for subsequent siblings (subsibs) of an infant who died of sudden infant death syndrome is uncertain, we describe an evaluation and monitoring program for subsibs. Eighty subsibs were screened in hospital at an average age of 4.6 weeks. The most valuable investigations included history, physical examination, blood gas tests, and four days on an apnea monitor in hospital. Sleep recordings added no decision-making data. Only 23 infants met one of the following criteria for home apnea monitoring: (1) sleep apnea for more than 15 s (either on sleep recordin…

During the last decade evidence has been accumulated that there might be a continuum from normally occurring short spells of apnea during sleep to life threatening long lasting respiratory pauses with marked bradycardia or even cardiac arrest from which, however, the infant could be resuscitated - the so-called Near Miss for Sudden Infant Death Event - and finally, to the Sudden Infant Death Syndrome (SIDS). There are still many missing links between these three phenomena and it seems likely that the connection between all three is not a straight matter in degree of immature dysfunction of car…

We have examined the behavior of several variables which are related to respiratory control in 114 infants (up to 6 months of age) in order to assess the risk for the sudden infant death syndrome (SIDS), 23 of the infants had already had demonstratable serious or life threatening apneas or respiratory problems during surgical anesthesia. These infants were assigned as a risk group, and the rest of the investigated babies was taken as a control group. We found that practically all infants of the risk group had apneas during sleep, which lasted longer than 8 s each. Only 22% of the infants of th…

BACKGROUND: Sudden Infant Death Syndrome (SIDS) is a leading cause of postneonatal mortality. The absence of specific biomarkers of SIDS diagnosis and risk leaves a significant gap in understanding SIDS pathophysiology. Metabolomics offers an avenue to better understand SIDS biology and identifying potential biomarkers.
METHODS: Using Metabolon Inc., global discovery panel, we analysed 828 metabolites from post-mortem serum samples of infants from the Chicago Infant Mortality Study (CIMS) and the NIH NeuroBioBank (NBB). In total, 300 infants (195 SIDS; 105 non-SIDS) across multiple race/ethni…

Coordination between physiological measures (i.e., the tendency for measures to co-vary with each other) develops with maturation in the infant. We hypothesized that correlations between cardiorespiratory measures would increase with maturation in normal infants and that infants destined to die of sudden infant death syndrome (SIDS) would show lower correlations than those of age-matched controls. Twenty-two recordings of electrocardiogram and respiratory movements were obtained from infants who subsequently succumbed to SIDS and compared with 66 recordings from control infants. Each 1-min epo…

6 infants (index cases) from five different families had a near-miss sudden infant death syndrome event between 3 and 12 weeks of age and had polygraphically documented apnoeas during sleep. 4 of their siblings had died of SIDS. The index cases, their 4 living siblings, 10 parents, and 8 grandparents underwent respiratory studies during sleep. All the adults and 3 index cases had cephalometric X-rays. 2 index cases underwent volume computerised tomographic scans when awake and during sleep. Index cases had mixed and obstructive sleep apnoea. Several family members had obstructive sleep apnoea;…

It is a controversial issue whether non-hereditary prolonged cardiac repolarization (increased QT-interval in ECG) is a cause of sudden infant death syndrome (SIDS). The QT-interval has, however, been found to be normal at rest. The present experiments on infant rats and infant and adult rabbits demonstrate a transient QT-prolongation during the innate fear paralysis reflex elicited by a variety of frightening stimuli. This reflex has previously been proposed as a triggering mechanism for SIDS. It may represent an extrinsic cause of QT-prolongation which adds to an intrinsic defect in repolari…

The outcome for three siblings of SIDS (SSIDS) infants was predicted, using as a risk model the sleep and respiratory characteristics of a SIDS victim studied extensively during the neonatal period. The SIDS infant had shown unstable state organization and deviant respiration patterns, including a deficit of brief apneic pauses. Like the SIDS infant, the SSIDS infants and a group of 16 normal infants were observed in the home for 7-hr periods when they were 2, 3, 4, and 5 weeks old. Two of the infants showed normal sleep and respiratory characteristics, and they were predicted to develop witho

INTRODUCTION: Sudden Infant Death Syndrome (SIDS) and Sudden Unexpected Fetal Death (SUFD) raise complex clinical, ethical, legal, and forensic issues. Determining the cause of death is essential both for family bereavement support and for prevention strategies.
MATERIALS AND METHODS: A comparative analysis was conducted on national regulations and standard practices in five selected countries (Italy, France, Spain, Germany and United Kingdom). Sources included national legislation, implementing regulations, and peer-reviewed literature. Comparison focused on: (1) performance of post-mortem e…

Cases of sudden unexpected infant death (SUID) leave many families devastated, especially in those without an identified cause of death. Here, we describe the case of an apparently healthy 15-day-old infant who died suddenly and unexpectedly. Through whole-exome sequencing, the infant was posthumously found to have 2 mutations in the CLCNKB gene, leading to a molecular diagnosis of Bartter syndrome type III, the likely cause of death. This case illustrates the potential utility of exome sequencing in cases of SUID to suggest a diagnosis, with important implications for families, allowing them
