[Knee synovitis in a rheumatoid patient with separated joint cavity].

A 40-year-old female patient with seropositive rheumatoid arthritis of 10 years duration. She had performed a total knee replacement of her left knee. At surgery the suprapatellar pouch was found to be separate from the rest of the knee joint, where direct access to the articular collagenous tissue remained. The suprapatellar pouch, presumable separate from the knee cavity since birth, was free of disease. The rest of the knee joint had gross and histopathological evidence of severe rheumatoid synovitis. The sparing of this suprapatellar pouch by rheumatoid disease is not easily explained, but…

[The neoplastiform aspect of a case of pigmented villonodular synovitis of the knee].

A case is presented of pigmented villonodular synovitis of the knee, macroscopic neoplastiform in appearance. The histologic examination leads to believe in a granulomatous rather than neoplastic nature of the process. The cause certainly is neither traumatic nor bacteriologic, but most probably dismetabolic lipidic. The neoformed mass, singular for its remarkable dimensions (12 cm. in length), nodular in appearance, pedunculated, had not yet provoked, at the time of operation, secondary osteocartilaginous and capsular-ligamentous lesions. At follow-up, more than 11 years later, no signs of re…

Essential cryoglobulinemia associated with intense and persistent synovitis of the knee.

Two patients with essential cryoglobulinemia, an immune complex disease, developed an intense, chronic, monoarticular knee synovitis. In each patient, synovial fluid findings (high white count and left shift) suggested a pyrogenic or crystalline etiology. A synovectomy was eventually performed in both patients to control their synovitis, which was refractory to medical management. The histologic and immunofluorescent synovial changes in the arthropathy of essential cryoglobulinemia, which was not previously been characterized, are marked by diffuse eosinophilic infiltration and edema. In both …

Traumatic synovitis in a classical guitarist: a study of joint laxity.

A classical guitarist performing for at least 5 hours each day developed a traumatic synovitis at the left wrist joint that was first erroneously considered to be rheumatoid arthritis. Comparison with members of the same guitar class suggested that unusual joint laxity of the fingers and wrist, probably inherited from the patient's father, was of more importance in the aetiology of the synovitis than a wide range of movement acquired by regular practice. Hyperextension of the metacarpophalangeal joint of the left index finger, quantified by the hyperextensometer, was less marked in the guitari…

Identification of CD64 as a marker for the destructive potential of synovitis in osteoarthritis.

OBJECTIVES: OA is characterized by cartilage degeneration and persistent pain. The majority of OA patients present with synovitis, which is associated with increased cartilage damage. Activated synovial macrophages are key contributors to joint destruction. Therefore, a marker that reflects the activation of these cells could be a valuable tool to characterize the destructive potential of synovitis and benefit monitoring of OA. Here, we aimed to investigate the use of CD64 (FcγRI) as a marker to characterize the damaging potential of synovitis in OA. METHODS: Synovial biopsies were obtained f…

Accuracy of Wrist MRI in Detecting Synovitis and Correlation with Arthroscopy.

Hypothesis  Wrist magnetic resonance imaging (MRI) has shown excellent diagnostic accuracy in evaluating soft-tissue pathology of the wrist including ganglion cysts, ligament tears, and triangular fibrocartilage complex (TFCC) pathology. However, it is unclear how often MRI detects synovitis that is subsequently encountered during wrist arthroscopy and may be a source of pain for patients with symptoms unresponsive to conservative treatment. The aim of this study is to assess the diagnostic accuracy of conventional MRI in the detection of wrist synovitis observed intraoperatively with wrist ar…

Remitting Seronegative Symmetrical Synovitis With Pitting Edema in an Adolescent.

Remitting seronegative symmetrical synovitis with pitting edema (RS3PE) is a rare clinical entity characterized by symmetrical tenosynovitis of both hands and ankles with pitting edema, negative rheumatoid factor (RF), absence of radiographic erosions, and excellent response to low-dose steroids. It is classically associated with elderly patients but may occur in younger patients, with only one case reported in the pediatric age. We report a case of RS3PE diagnosed in a pediatric patient.

Radiosynovectomy in haemophilic synovitis and arthropathy of the knee: A scoping review.

INTRODUCTION: Radiosynovectomy is an established treatment for chronic synovitis in patients with haemophilia. Although its role in rheumatological diseases has diminished, it remains an accepted therapy for haemophilic synovitis. AIM: The aim of this scoping review was to map and summarise the evidence surrounding radiosynovectomy in haemophilic knees, identify gaps in the literature and inform future research. RESULTS: Forty-three manuscripts and abstracts were identified for this review. Evidence was limited to observational studies and Yttrium-90 was the most studied licensed radioisotop…

Arthroscopic Excision of Pigmented Villonodular Synovitis of the Trochanteric Bursa.

Trochanteric bursitis is a common disorder affecting middle-aged adults and usually presents with lateral-based hip pain and swelling. It us … as excised arthroscopically and biopsy of the tissue revealed a diagnosis of pigmented villonodular synovitis (PVNS). The patient had an uneventful recovery and had a full resolution of symptoms with no recurrence noted at 3-year follow-up. This is the first reported case of arthroscopic excision of PVNS of the trochanteric bursa. Given that it may mimic tr …

Cannabinoid receptors are expressed in equine synovium and upregulated with synovitis.

BACKGROUND: Osteoarthritis (OA) is a major cause of equine lameness. Cannabinoid (CB) receptors are now considered to be promising therapeutic targets in human rheumatology for pain and inflammation, however, little is known about the equine endocannabinoid system. OBJECTIVES: The primary goal was to assess the presence and expression pattern of CB1 and CB2 in the synovium of healthy joints. A secondary goal was to explore the relationship between the CB expression, degree of synovitis and OA pathology. STUDY DESIGN: Ex vivo experimental study. METHODS: Metacarpophalangeal joints (n = 25) f…

Transient synovitis of the hip: Current practice and risk of misdiagnosis.

INTRODUCTION: Transient synovitis (TS) is a common and benign cause of hip pain in children, but must be distinguished from more serious entities such as septic arthritis, osteomyelitis, and pyomyositis. Our objectives were to determine the risk of missed bacterial musculoskeletal infection and rates of diagnostic testing in children diagnosed with TS. METHODS: We performed a cohort study using the Pediatric Heath Information System of children 1-10 years diagnosed with TS in the ED. We determined rates of missed bacterial musculoskeletal infection (defined as a new diagnosis of septic arthri…

Novel inflammatory markers in the blood of patients with knee synovitis.

OBJECTIVE: Synovitis is a joint disease that seriously affects patient quality of life, but there are currently no diagnostic markers. The albumin to fibrinogen ratio (AFR) and monocyte to lymphocyte ratio (MLR) are non-invasive and cost-effective markers for various systemic inflammatory diseases. However, these markers have not yet been investigated for synovitis. This cross-sectional study evaluated the predictive ability of AFR and MLR in patients with non-specific knee synovitis. METHODS: One hundred fifty-five patients with knee synovitis and 108 healthy control patients were enrolled. …

Hemorrhagic Synovitis of the First Metatarsophalangeal Joint: A Case Report.

CASE: A 69-year-old woman presented with a painful mass at her first metatarsophalangeal joint. Further evaluation was concerning for a neoplastic process, leading to surgical intervention. Pathological examination demonstrated hemosiderotic synovitis, and hematologic evaluation led to a new diagnosis of von Willebrand disease. CONCLUSION: Hemorrhagic synovitis, involving mostly larger joints, has been well described. However, a literature search demonstrates no cases of this in the foot or toes. Presentation of hemarthroses and underlying coagulopathies can be subtle and must be considered i…

Osteochondral Lesion in Diffuse Pigmented Villonodular Synovitis of the Knee.

Pigmented villonodular synovitis (PVNS) is a rare benign condition that is locally aggressive and may destructively invade the surrounding soft tissues and bone causing functional loss of the joint and the limb. The knee is the most affected joint (range, 28% to 70%) but involvement of the bone is not a common feature seen at this site. We present a rare case of diffuse PVNS of the knee associated with subchondral cyst of the lateral femoral condyle. This posed a diagnostic dilemma because of bone invasion. The radiological image of synovitis was pathognomonic of PVNS but etiology of the osteo…

A Rare Incidence of Pigmented Villonodular Synovitis of the Ankle in an Adolescent.

Pigmented villonodular synovitis mostly affects the knee and other large joints such as the hip. Although the disease is most commonly found in adult patients aged 30 to 40 years, rare cases in children and the elderly have been reported. We present the case of an 11-year-old female who was found to have biopsy-proven pigmented villonodular synovitis in her subtalar joint in 2012. Five years after surgical excision, the patient has continued to be involved in competitive dancing and cheerleading without any pain in her ankle. Moreover, follow-up magnetic resonance imaging studies showed no evi…
